Elastic titanium nails are widely used for the established procedure of Elastic Stable Intramedullary Nailing (ESIN), but are costly. Thirty-five diaphyseal fractures in various long bones in children were treated by flexible nailing using stainless steel Kirschner (K) wires. The results were comparable with reports by other authors using titanium nails and stainless steel K wires. Cost-effective K wires and instrumentation are easily available. A stainless steel K wire is 92% cheaper than a Nancy nail and 84% cheaper than a Synthes titanium elastic nail. K wires can be used for flexible naili

INTRODUCTION: There is currently no universally accepted follow-up protocol for pediatric supracondylar humerus fractures (pSCHFs), but post-mobilization follow-ups are commonly scheduled to evaluate range of motion and the presence of deformities. Recent practice has trended towards enhancing efficiency by reducing unnecessary clinic visits for uncomplicated fractures. This study examines follow-up practices after pSCHFs at our multicenter institution, evaluating the utility of post-mobilization follow-up visits, and determining appropriate criteria for safe early discharge.

Various osteotomy methods have been proposed in the treatment of cubitus varus. We designed an improved stepped osteotomy to achieve improved deformity correction. We refer to this new approach as double-closed wedge broken-line osteotomy and report a series of clinical and imaging results (deformity correction, range of motion [ROM], function, osteotomy healing, and complications) of patients with cubitus varus treated with this technique.Between July 2014 and July 2019, we treated 9 cases of cubitus varus using the new technique. The study was conducted in accordance with the principles of t

OBJECTIVE: To investigate the effectiveness of three-dimensional (3D) navigation template in the adult cubitus varus osteotomy.
METHODS: Between April 2013 and September 2015, 17 patients with cubitus varus were admitted. There were 6 males and 11 females, aged from 19 to 38 years, with an average age of 26.9 years. There were 10 cases of left elbow joints and 7 cases of right elbow joints. The disease duration was 9-30 years (mean, 18 years). Based on the preoperative X-ray film, the humerus-elbow-wrist (HEW) angle was (-13.2 ±3.3)°, the anteversion angle was (-10.5±2.3)°. The preoperative r

STUDY DESIGN: Case report.
INTRODUCTION: Radial nerve injury can cause severe functional impairment due to paralysis of wrist and digit extensors. Various orthotic designs have been described, including static, dynamic, and tenodesis. All provide wrist stabilization or extension assistance. Some, but not all, also provide extension assistance to the wrist, thumb, and fingers.
PURPOSE AND METHODS: This article tells the story of Max, a 27-year-old male university student, who sustained a radial nerve injury after a left humeral shaft fracture. He was treated at a Brazilian tertiary hospital,

BACKGROUND: Heterotopic ossification (HO) is a known complication that can arise after total elbow arthroplasty (TEA). In most cases, it is … kely to develop in patients undergoing TEA because of ankylosis, primary osteoarthritis, and distal humeral fractures. Surgical intervention is more likely to be required in patients in whom HO develops after TEA performed for ankylosis and post-traumatic osteoarthritis.
CONCLUSION: HO is an uncommon complication after TEA, with most patients in whom HO develops b …

The best method of managing a fracture of the distal humerus in a frail low-demand patient with osteoporotic bone remains controversial. Total elbow arthroplasty (TEA) has been recommended for patients in whom open reduction and internal fixation (ORIF) is not possible. Conservative methods of treatment, including the 'bag of bones' technique (acceptance of displacement of the bony fragments and early mobilisation), are now rarely considered as they are believed to give a poor functional result. We reviewed 40 elderly and low-demand patients (aged 50 to 93 years, 72% women) with a fracture of

An original technique was developed to correct distal humerus malunion using a three-dimensional computer-assisted planning and a custom-made surgical guide. The technique was used in three cases (two with cubitus varus and one with cubitus valgus).A CT-scan of the distal humerus was obtained. The correction was simulated by software. A three-dimensional model of the patient's affected humerus was created by rapid prototyping, allowing creation of a surgical guide and premoulding of the osteosynthesis plate. At the time of surgery, the sterilised guide was placed on the surface of the bone to

INTRODUCTION: Five adult patients, with posttraumatic cubitus varus deformity underwent corrective surgery by the Ilizarov method of distraction osteogenesis.
METHOD: A standard technique was applied in all patients. The average follow-up was 28 (range, 24-38) months. Preoperative carrying angle ranged from 12 degrees to 22 degrees of varus (average 16.6 degrees ) and postoperative carrying angle ranged from 10 degrees to 14 degrees of valgus (average, 11.6 degrees ) equalized to the contralateral side.
RESULT: The outcome was rated as excellent in all patients. No complication was observed,

We report a case of complex reconstruction after traumatic loss of bone and muscle of the upper third of the humerus. A composite flap using the lateral border of the scapula and the pediculated latissimus dorsi muscle was used. The latissimus dorsi flap covered the bony reconstruction and enabled elbow flexion. Outcome was very satisfactory at three years follow-up. The lateral border of the scapula is rarely used for bony reconstruction of the humerus. Free composite flaps with the lateral portion of the scapula and the latissimus dorsi muscle have been used to reconstruct tissue loss of the

A retrospective review of 158 type 3 supracondylar humerus fractures was undertaken to determine whether any correlation exists between an increased time from injury to surgery and four unfavorable results: a longer operative time, an increase in hospital stay, an increase in the need to open the fracture, or an increase in unsatisfactory outcomes. The average age of the patients was 5.0 years. Five children had nerve injury on initial examination, and no arm was poorly perfused. The average time from injury to evaluation in the emergency department was 9.8 hours and the average time from the

The arterial blood supply of the arm is unique, as the collateral circulation is especially good. Because of this anatomic perculiarity it is possible to deal with lack of peripheral pulse without critical ischemia occurring in fractures in the elbow region. We present two children with supracondylar fractures of the humerus with damage to the brachial artery and the different management results. It is necessary to carry out the immediate diagnostic and therapeutic procedures in order to prevent late complications. We conclude that Volkmann's ischemic contracture can always be prevented.

Clinical features and X-rays of thirty-four cases with cubitus varus deformity were analysed in order to explain the occurrence of tardy ulnar nerve palsy caused by cubitus varus deformity. Cubitus varus cases with tardy ulnar nerve palsy, compared to cases without it, were older at the first visit to the clinic for cubitus varus deformity. There were no other differences between the two groups in clinical features. Computed tomography showed that the olecranon moved to the ulnar side against the olecranon fossa. Operative findings showed that the medial head of triceps brachii shifted mediall

Two methods of physiotherapy after fracture of the proximal end of the humerus were compared. The study was designed as a randomized, controlled and single-blind trial. Twenty patients were assigned to two groups 10-12 days after the injury. One group was treated conventionally, the other received instructions in self-training with follow-up control of results. Objective examination and subjective assessment were made at five intervals, up to one year after the injury. The greatest improvement in function was shown in both groups between 3 and 8 weeks. The patients deemed their daily life func

INTRODUCTION: Bicortical screw fixation, which penetrates and fixes the near and far cortex of bone, has been conventionally used to achieve compressive fixation for fracture using screws. Open reduction and internal fixation using the locking plate are widely used for treating proximal humerus fractures. However, minimal contact between the bone and the locking plate can lead to an insufficient reduction. Theoretically, a dual-lead locking screw with different leads for the screw head and body could enhance the reduction and fixation stability of fragments in proximal humeral fractures withou…

BACKGROUND: Bony defects are common injuries associated with anterior shoulder dislocation. It is generally thought that these bony defects are created at the time of dislocation. However, there have been no biomechanical reports demonstrating the exact time point when these lesions occur. The purpose of this study was to clarify when, how, and which types of bony defects were created during experimental dislocation in cadaveric shoulders.
METHODS: Fifteen fresh-frozen cadaveric shoulders (mean age at the time of death, 79 years) were fixed in a custom testing machine. First, the glenohumeral

BACKGROUND: Elbow tumors, though rare, pose significant reconstructive challenges due to the joint's complex anatomy and limited soft tissue coverage. This study evaluates the functional outcomes, complications, and implant survival rates of modern-design elbow tumor endoprostheses following resection of distal humerus or proximal ulna tumors.
METHODS: Conducted across 2 tertiary sarcoma centers, the retrospective study analyzed 18 patients treated from 2014 to 2023. Patients underwent primary surgery with consistent surgical and postoperative protocols. The study evaluated implant survival,

BACKGROUND: Limb salvage following resection of a tumor in the proximal part of the humerus poses many challenges. Reconstructive options are limited because of the loss of periarticular soft-tissue stabilizers of the glenohumeral joint in addition to the loss of bone and articular cartilage. The purpose of this study was to evaluate the functional outcome and survival of the reconstruction following use of a humeral allograft-prosthesis composite for limb salvage.
METHODS: An allograft-prosthesis composite was used to reconstruct a proximal humeral defect following tumor resection in thirty-
