Tree-based models for survival data with competing risks.

OBJECTIVE: Tree-based models belong to common, assumption-free methods of data analysis. Their application in survival data is narrowed to univariate models, which partition the feature space with axis-parallel hyperplanes, meaning that each internal node involves a single feature. In this paper, I extend the idea of oblique survival tree induction for competing risks by modifying a piecewise-linear criterion function. Additionally, the use of tree-based ensembles to analyze the competing events is proposed. METHOD AND MATERIALS: Two types of competing risks trees are proposed: a single event

Explainable machine learning and unsupervised anatomical phenotyping of mitral valve prolapse in patients with severe mitral regurgitation.

BACKGROUND: Mitral annular disjunction (MAD) is increasingly recognized in mitral valve prolapse (MVP), but its anatomical embedding within degenerative MVP remains incompletely defined. We assessed whether explainable supervised learning and unsupervised clustering could identify MAD-associated MVP phenotypes. METHODS: We analyzed a public surgical cohort of 979 patients with MVP and clinically significant mitral regurgitation. Etiology was classified as Barlow disease (BD) or fibroelastic deficiency (FED). Logistic regression, Random Forest and XGBoost were evaluated for MAD prediction usin

Frailty and aortic valve disease.

Frailty is a common occurrence in elderly persons and is present in approximately half of the patients being screened for transcatheter aortic valve replacement (TAVR) therapy. Accurate assessment of the likelihood of benefit from intervention in the older patient with aortic stenosis is critical with both surgical aortic valve replacement and TAVR now available. Whereas risk algorithms are available that are helpful in predicting outcomes in patients undergoing surgical procedures, measures of frailty are not included in the algorithms. When considering intervention in the elderly patient, th

Investigating the effect of remodelling signal type on the finite element based predictions of bone remodelling around the thrust plate prosthesis: a patient-specific comparison.

The resorption of bone in the human femur following total hip arthroplasty is recognized to be related to the loading in the bone surrounding the prosthesis. However, the precise nature of the mechanical signal that influences the biological remodelling activity of the bone is not completely understood. In this study, a validated finite element modelling methodology was combined with a numerical algorithm to simulate the biological changes over time. This was used to produce bone remodelling predictions for an implanted thrust plate prosthesis (Centerpulse Orthopedics Limited) in a patient spe

A double-edged sword: unpacking the functional heterogeneity of mobile social media use and its differential associations with exercise identity and social-media-specific sedentary time among older adults.

BACKGROUND: As digital life prevails among older adults, this study investigates how mobile social media functions affect their exercise identity and social-media-specific sedentary time. METHODS: Using the S-O-R framework, a dual-path model was tested with SEM (AMOS 20.0) on data from 452 older adults in China. RESULTS: Overall, mobile social media functions were not significantly associated with exercise identity or social-media-specific sedentary time. However, specific functions showed divergent effects: information retrieval (Z = 6.06, 95% CI [0.14, 0.27]), online guidance (Z = 3.32, 95

[Optimizing follow-up of pregnancies of unknown location: a simplified clinical algorithm for ectopic pregnancy risk].

OBJECTIVES: Management of pregnancies of unknown location (PUL) relies on close monitoring with serial human chorionic gonadotropin (hCG) measurements and repeated ultrasound examinations to exclude ectopic pregnancy (EP). Although safe, this strategy leads to multiple consultations and significant organizational and economic burden. This paper aims to develop and preliminarily validate a simplified clinical algorithm to stratify the risk of EP in women presenting with PUL, in order to adapt follow-up frequency. METHODS: The prospective monocentric OPTIGLI study was conducted at Nantes Univer

Long-term Evaluation of Machine Learning Based Methods for Air Emission Monitoring.

Machine learning (ML) techniques have been researched and used in various environmental monitoring applications. Few studies have reported the long-term evaluation of such applications. Discussions regarding the risks and regulatory frameworks of ML applications in environmental monitoring have been rare. We monitored the performance of six predictive models developed using ML and statistical methods for 28 months. The six models used to predict NOx emissions were developed using six different algorithms. The model developed with a moderate complexity algorithm, adaptive boosting, had the best
