[Severe nutritional deficiencies in young infants with inappropriate plant milk consumption].

Over the past few years, we have observed increasing consumption of inappropriate plant milks as an alternative to infant milk formula. Some families believe that foods labeled as natural are the most healthy and an appropriate nutritional choice. However, their composition does not respect European recommendations. They are always hypocaloric and protein, vitamin, and mineral concentrations are inadequate. The aim of this study was to report severe nutritional complications after inappropriate plant milk consumption. Between 2008 and 2011, we studied severe nutritional deficiencies caused by

Differences in sensitivity to the convulsant pilocarpine in substrains and sublines of C57BL/6 mice.

In rodents, the cholinomimetic convulsant pilocarpine is widely used to induce status epilepticus (SE), followed by hippocampal damage and spontaneous recurrent seizures, resembling temporal lobe epilepsy. This model has initially been described in rats, but is increasingly used in mice, including the C57BL/6 (B6) inbred strain. In the present study, we compared the effects of pilocarpine in three B6 substrains (B6JOla, B6NHsd and B6NCrl) that were previously reported to differ in several behavioral and genetic aspects. In B6JOla and B6NHsd, only a small percentage of mice developed SE indepen

A 5-year PICU experience of disseminated staphylococcal disease, part 2: management, critical care needs and outcome.

Staphylococcus aureus causes an impressive spectrum of disease in tropics and subtropics. Scanty data are available regarding disseminated staphylococcal disease (DSD) in children, especially on their critical care needs. It is important to recognize and prioritize patients who may benefit most from Pediatric Critical Care. The objective of this article is to review the, critical care needs, management and outcome of patients with DSD and to identify clinical indicators for need of critical care. The study setting is a Pediatric Intensive Care Unit of an urban tertiary care teaching hospital i

Intractable temporal lobe epilepsy with rare spikes is less severe than with frequent spikes.

OBJECTIVE: To analyze clinical, electrophysiologic, and neuroradiologic characteristics of a group of patients with nonlesional intractable temporal lobe epilepsy (TLE) and rare or absent interictal epileptiform abnormalities (IEA). METHODS: Between 1990 and 2000, 31 patients (11 men; mean +/- SD age 34.3 +/- 11.7 years) with nonlesional intractable TLE were consecutively selected on the basis of the absence or paucity of IEA (<1/h) on serial scalp EEG recording; these were defined as "oligospikers." The clinical and laboratory characteristics of oligospikers were compared with those of a gro

Intractable seizures of frontal lobe origin: clinical characteristics, localizing signs, and results of surgery.

PURPOSE: We analyzed the clinical characteristics of seizures of frontal lobe (FL) origin with particular emphasis on establishing different categories and determining if these categories had any localizing or lateralizing value. In addition, results of surgery are reported. METHODS: Seizure characteristics were established by historical review and electroencephalographic/videotape analysis of 449 seizures in 26 adult patients with refractory seizures of FL origin. RESULTS: No outstanding risk factor was identified for seizures of FL origin. Seizures were frequent (7.1 per week), brief (mean

FLAIR hyperintense cortical lesions in myelin oligodendrocyte glycoprotein-associated encephalitis with seizures in children: a retrospective single-center case series.

BACKGROUND AND PURPOSE: In recent years, the number of case reports concerning fluid-attenuated inversion recovery (FLAIR) hyperintense cortical lesions in myelin oligodendrocyte glycoprotein (MOG) -associated encephalitis with seizures (FLAMES) has been gradually increasing. However, within the pediatric demographic, there remains a lack of related serial reports. This study was designed to characterize the clinical features and prognosis of FLAMES in pediatric patients. METHODS: We reviewed the medical records of children diagnosed with FLAMES from January 2019 to July 2024 and retrospectiv

Benefits and Risks of Epilepsy Surgery in Patients With Focal Cortical Dysplasia Type 2 in the Central Region.

BACKGROUND AND OBJECTIVES: Focal cortical dysplasia type 2 (FCD2) in the central region can cause drug-resistant epilepsy for which surgery remains challenging because of subsequent functional deficits. Advances in imaging and surgical techniques have progressively improved outcome. We aimed to assess the benefits on epilepsy and the functional risks after FCD2 resections in these highly eloquent areas. METHODS: We retrospectively studied all consecutive patients with histologically confirmed FCD2 located in the central region operated on between 2000 and 2019 at a single center. We analyzed

Neurologic Assessment of the Neurocritical Care Patient.

Sedation is a ubiquitous practice in ICUs and NCCUs. It has the benefit of reducing cerebral energy demands, but also precludes an accurate neurologic assessment. Because of this, sedation is intermittently stopped for the purposes of a neurologic assessment, which is termed a neurologic wake-up test (NWT). NWTs are considered to be the gold-standard in continued assessment of brain-injured patients under sedation. NWTs also produce an acute stress response that is accompanied by elevations in blood pressure, respiratory rate, heart rate, and ICP. Utilization of cerebral microdialysis and brai

Case Report: Reduced CSF Orexin Levels in a Patient With Sepsis.

Sepsis is a potentially lethal condition characterized by systemic inflammation and multiple organ failure, and sepsis-associated encephalopathy (SAE) is an independent risk factor for mortality in patients with sepsis. We previously reported that orexin improved survival in an animal model of sepsis by acting in the brain. Peripherally administered orexin entered the brain under the conditions of systemic inflammation because of BBB dysfunction and produced survival-related effects. As a therapeutic concept, we hypothesized that orexin treatment enhances recovery from sepsis by restoring redu

Transient increase of P-glycoprotein expression in endothelium and parenchyma of limbic brain regions in the kainate model of temporal lobe epilepsy.

Several recent studies have shown that the multidrug transporter P-glycoprotein (PGP) is over-expressed in endothelial cells from brain blood vessels of patients with refractory temporal lobe epilepsy (TLE), suggesting that altered drug permeability across the blood-brain barrier (BBB) may be involved in pharmacoresistance to antiepileptic drugs (AEDs). Furthermore, over-expression of PGP has been found in astrocytes of epileptogenic tissue. However, it is not known in which regions of the temporal lobe PGP over-expression occurs and whether the over-expression is a result of uncontrolled seiz

Phenotypic characterisation, quality of life and prevalence of idiopathic epilepsy and epilepsy of unknown cause in the nine Dutch dog breeds.

INTRODUCTION: Canine epilepsy is a common and complex neurological disease that occurs in almost all dogs. In the Netherlands, there are nine pedigree dog breeds, and all of them have numerous small populations, with moderate to high inbreeding coefficients. The goal of this study is to characterise epilepsy within these breeds and to estimate the prevalence for each of these nine breeds: Dutch Partridge dog, three variants of the Dutch Shepherd, Dutch Smoushound, Kooiker dog, Markiesje dog, Saarloos Wolfdog, Schapendoes dog, Stabyhoun, and Wetterhoun. METHOD: An online questionnaire was dist

Treatment administration during a seizure in home-settings: Time to treat (TT).

BACKGROUND AND OBJECTIVE: Early intervention on prolonged and cluster seizures can avoid serious consequences such as irreversible neuronal damage, late onset epileptogenesis, sudden unexpected death, and premature death, among others. In addition, it can prevent progression to status epilepticus, which has a mortality rate of 20%. However, prehospital diagnosis and treatment of seizures is often delayed, as patients receive treatment 30minutes past seizure onset even though most seizures last less than 2minutes. The aim of this consensus was to determine the time at which rapid and early seiz

Dogs as a Natural Animal Model of Epilepsy.

Epilepsy is a common neurological disease in both humans and domestic dogs, making dogs an ideal translational model of epilepsy. In both species, epilepsy is a complex brain disease characterized by an enduring predisposition to generate spontaneous recurrent epileptic seizures. Furthermore, as in humans, status epilepticus is one of the more common neurological emergencies in dogs with epilepsy. In both species, epilepsy is not a single disease but a group of disorders characterized by a broad array of clinical signs, age of onset, and underlying causes. Brain imaging suggests that the limbi

Head Rules Over the Heart: Cardiac Manifestations of Cerebral Disorders.

UNLABELLED: The Brain-Heart interaction is becoming increasingly important as the underlying pathophysiological mechanisms become better understood. "Neurocardiology" is a new field which explores the pathophysiological interplay of the brain and cardiovascular systems. Brain-heart cross-talk presents as a result of direct stimulation of some areas of the brain, leading to a sympathetic or parasympathetic response or it may present as a result of a neuroendocrine response attributing to a clinical picture of a sympathetic storm. It manifests as cardiac rhythm disturbances, hemodynamic perturba

Simple Changes to Emergency Department Workflow Improve Analgesia in Mechanically Ventilated Patients.

INTRODUCTION: In 2013 the Society for Critical Care Medicine (SCCM) published guidelines for the management of pain and agitation in the intensive care unit (ICU). These guidelines recommend using an analgesia-first strategy in mechanically ventilated patients as well as reducing the use of benzodiazepines. Benzodiazepines increase delirium in ICU patients thereby increasing ICU length of stay. We sought to determine whether a simple educational intervention for emergency department (ED) staff, as well as two simple changes in workflow, would improve adherence to the SCCM guidelines. METHODS:

The problem of epilepsy and its care in rural Ethiopia.

BACKGROUND: Jimma University Hospital (JUH) in south west Ethiopia has been running an integrated rural chronic disease programme since 1999, focusing on treatment of epilepsy, diabetes and heart disease. OBJECTIVE: The purpose of this review is to compare clinical data of the epilepsy patients with those previously published from a similar programme in Gondar university, in the north west of the country. METHOD: In November 2005, we identified a total of 1,250 patients with epilepsy from the JUH study with case records for analysis. Clinical data are documented in the case records using a s

Retrospective investigation of medico-legal autopsy cases involving mentally handicapped individuals.

Mentally handicapped individuals were involved in some way in 64 (17.7%) of the total of 362 medico-legal autopsy cases during the past 13 years. Among these 64 cases, 32 were suicides committed by mentally handicapped persons (18 males and 14 females). The most common method of suicide was drowning (65.6%). Five cases were homicides suddenly and unexpectedly committed by mentally handicapped persons. In four of the five cases, the victim was a member of the assailant's family; this was considered to be a characteristic of homicides committed by mentally handicapped persons. On the other hand,

[Epileptic activity of the occipital lobe. Clinico-electroencephalographic contribution].

Our analysis of the course of illness in 14 patients, whose common electroencephalographic characteristic was epileptogenic activity in the occipital area, showed very different clinical symptoms. The first group comprised patients who presented bilateral amaurosis. In four of these cases, the occipital hypersynchronous EEG activity was merely a secondary symptom of either ischaemic hypoxia or of a degenerative process in the occipital visual cortex and was not responsible for the genesis of the actual blindness. In two further cases of monosymptomatic temporary loss of vision, it was difficul
