Prevalence of contact allergy at a dermatology clinic in China from 1990-2009.

BACKGROUND: The prevalence of contact allergy varies in different regions and populations. OBJECTIVE: To describe the frequency of sensitization in patients with dermatitis or eczema referred to Peking University First Hospital and analyze the trends in the prevalence of common allergens from January 1, 1990, to December 31, 2009. METHODS: A total of 1,858 patients were patch tested with the Chinese baseline series of contact allergens. Data were collected from retrospective charts and analyzed. RESULTS: Positive reactions to one or more allergens were shown in 1,374 patients (74.0%). The m

Textile dyes Disperse Orange 1 and Yellow 3 contain more than one allergen as shown by patch testing with thin-layer chromatograms.

BACKGROUND: It is known that some patch-test preparations containing disperse dyes contain impurities with unknown relevance for the development or elicitation of contact allergy. OBJECTIVE: To evaluate the significance of the impurities found in the commercial dyes Disperse Orange 1 (DO1) and Disperse Yellow 3 (DY3) regarding contact allergy in patients with known sensitivity to them. METHODS: Ten patients allergic to DY3 and/or DO1 were tested with a dilution series of commercial and purified DY3 and DO1 (with water-soluble parts prepared from the commercial dyes) and with naphthalene sulf

Allergic contact dermatitis from carmine in cosmetic blush.

Although there are many reported cases of immediate allergy after ingestion of foods containing cochineal, there are few reports of allergic contact dermatitis from carmine. We present a rare case of allergic contact dermatitis due to carmine. A 52-year-old female presented with an itchy erythema on her cheeks at the site where blush had been applied. Patch-tested with her cosmetics, she showed a positive reaction to the blush (30% in petrolatum) and to 0.2% (but not 0.1%) carmine in petrolatum. In this case, the optimum patch-test concentration of carmine was 0.2% in petrolatum.

Positive patch- and photopatch-test reactions to methylene bis-benzotriazolyl tetramethylbutylphenol in patients with both atopic dermatitis and chronic actinic dermatitis.

Ultraviolet filters are the most common topical photoallergens. Although currently not available on the US market, methylene bis-benzotriazolyl tetramethylbutylphenol (referred to as bisoctrizole on product labels) represents a new class of UV filters that have both organic and inorganic properties and are widely available in different preparations in Europe, South America, and Asia. We report two patients with atopic dermatitis and chronic actinic dermatitis who had positive patch- and photopatch-test reactions, which suggested both an allergic contact and a photoallergic contact dermatitis f

Antiirritant properties of polyols and amino acids.

BACKGROUND: Antiirritants are used in cosmetic products to prevent or to treat skin irritations that arise during daily life. Data were published earlier on the efficacy of the best-known humectant, glycerol, in reducing irritation. OBJECTIVE: The aim of the present study was to examine the effects of different polyols (including glycerol, xylitol, and mannitol) and the amino acids taurine and glycine on sodium lauryl sulfate (SLS)-induced skin irritation. METHODS: Healthy adult volunteers were patch-tested with 0.1% SLS in the presence or absence of one or another polyol or amino acid. Skin

Allergen content of patient problem and nonproblem gloves: relationship to allergen-specific patch-test findings.

BACKGROUND: Identification of putative contact allergen and source material is often done by a combination of patch testing and manufacturer-supplied product information. The accuracy of the identification of allergen-source material and level of allergen in that allergen-source material is not known. OBJECTIVE: The objectives of the study were to survey the chemical allergen content of glove allergic contact dermatitis (ACD) patient-identified problem and nonproblem gloves and to evaluate the ability of the patient to discriminate between problem and nonproblem gloves. METHODS: Gloves from

Sodium hydroxymethylglycinate.

Sodium hydroxymethylglycinate (SHMG) is a preservative used in many commercially available products, including shampoos, conditioners, soaps, moisturizers, body sprays, baby wipes, room sprays, cleaning agents, and pesticides. It is in a class of chemicals known as formaldehyde-releasing preservatives. Notably, members of this class have been associated with allergic contact dermatitis, possibly due to the agents themselves, the formaldehyde they release, or both. Studies on SHMG in animals have demonstrated potential for sensitization and dermatitis, and formaldehyde-allergic patients have be

Physician-patient agreement in the assessment of allergen relevance.

BACKGROUND: The efficacy of patch testing may be enhanced by data allowing the physician to estimate the likelihood that results of a patch-test reading are relevant to a patient's dermatitis. OBJECTIVE: The goal of this study was to compare the rates of agreement between the physician's assessment of relevance at the time of final reading and the patients' report 3 months to 3 years later in regard to whether avoidance of an allergen was needed to remain free of dermatitis. We hypothesize that the agreement rates between the physician and patient relevance assessments will vary based on prop

Local lymph node data for the evaluation of skin sensitization alternatives: a second compilation.

BACKGROUND: Development, evaluation and validation of alternatives to skin sensitisation testing require the availability of reliable databases with which comparative analyses can be conducted to establish performance characteristics. To facilitate this we have published previously a database comprising results from local lymph node assays (LLNAs) conducted with 211 chemicals. That database embraced a substantial range of chemistry, and of relative skin sensitising potency, and has found application in the assessment of new or refined methods. OBJECTIVE: In this paper we describe a second com

Positivity ratio and reaction index: patch-test quality-control metrics applied to the north american contact dermatitis group database.

BACKGROUND: The positivity ratio (PR) and reaction index (RI) characterize the ability of patch-test preparations to produce strong (++ or +++) reactions as opposed to weak (+), questionable, or irritant reactions. OBJECTIVE: This study evaluates these measures for North American Contact Dermatitis Group (NACDG) patch-test preparations. METHODS: The PR and RI were calculated for 79 NACDG standard allergens tested from 1994 to 2006 (n = 26,479 patients). The median values were used as cutoff values for "acceptable" versus "problematic" preparations. RESULTS: The top 10 "acceptable" patch-tes

Emergent and unusual allergens in cosmetics.

Allergic contact dermatitis from cosmetics is a common problem that is occasionally caused by new or rare allergens. When a patient has a positive patch test to a cosmetic product but to none of the common or commercially available allergens, it is important to further patch-test this patient to the ingredients of the product. Thorough testing with the breakdown of ingredients, usually obtained through cooperation with the manufacturer, often allows identification of the culprit allergen in the cosmetic product. In this article, we discuss emerging or rare allergens discovered by this method,

Vitamin E and allergic contact dermatitis.

Reports of vitamin E-induced allergic contact dermatitis (ACD) and frequent use of vitamin-E derivatives (tocopherol, tocopheryl linoleate, tocopherol acetate, etc) in skin care products deserves further investigation into tolerability and suitability of vitamin E in skin care preparations. A PubMed search was conducted to review the prevalence of vitamin E-induced ACD. It revealed 931 cases of vitamin E-induced ACD mainly from one large study. There were no reported deaths and only three patients required hospitalization for treatment. It appears that vitamin E-induced ACD is an uncommon phen
