Reverse Total Shoulder Arthroplasty for Proximal Humerus Fractures Using the Biomet Comprehensive Shoulder System: One-Year Results Reflect Clinically Meaningful Recovery.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is an established treatment for complex proximal humerus fractures in elderly patients, providing reliable pain relief and functional improvement. Outcomes are well described at one and two years, but whether changes observed between these timepoints are clinically meaningful remains unclear. METHODS: We conducted a retrospective analysis of prospectively collected data on 64 patients who underwent reverse total shoulder arthroplasty using the Comprehensive Shoulder System (Biomet, Warsaw, IN, USA) for acute proximal humerus fractures wit

Lateral closing wedge supracondylar osteotomy of humerus for post-traumatic cubitus varus in children.

Cubitus varus deformity following mal-union of a supracondylar fracture of the humerus in children causes no functional disability, but surgical correction is often requested to improve the appearance of the arm. Maintaining the correction after supracondylar osteotomy is a difficult aspect of the operative treatment and remains controversial. Nine children aged between 6 and 12 years (average 8 years and 11 months) underwent lateral closing wedge supracondylar osteotomy of the humerus, for deformity ranging between 10 and 20 degrees (average 13 degrees). The correction required ranged between

Low-intensity X-ray-imaging scope (Lixiscope). Its applications in sports medicine and orthopaedic surgery.

The low-intensity x-ray imaging scope (Lixiscope) is a hand-held, totally portable fluorescopic device utilizing a low-energy, low-intensity gamma source of 125I. The unit was developed at the Goddard Space Flight Center for the National Aeronautics and Space Administration. It has become apparent that there is increasing use of this tool, especially in sports medicine, to diagnose and treat injuries of the hand, foot, ankle, wrist and elbow at the time that they occur. The Lixiscope enables the orthopaedic surgeon to carry out the immediate screening at the scene of the injury with a hand-hel

Cementless Long-Stem Reverse Total Shoulder Arthroplasty as Primary Treatment for Metadiaphyseal Humeral Shaft Fractures.

BACKGROUND: The use of cementless diaphysis-fitting stems has been shown to be an effective treatment option for cases of metadiaphyseal humeral fracture. Complex metadiaphyseal fractures are those that extend below the surgical neck into the diaphysis, which can compromise the metaphyseal fixation of typical arthroplasty designs. The presently described surgical technique circumvents the potential risks associated with the use of cement while also permitting the treatment of common concomitant pathologies, such as arthritis and rotator cuff tendinopathy. Evidence supports the efficacy of this

Surgical Timing and Approach in Gartland Type III Supracondylar Humerus Fractures in Children: Does After-Hours Surgery Influence Clinical and Radiological Outcomes? A Retrospective Cohort Study.

Background/Objectives: Gartland Type III supracondylar humerus fractures (SCHFs) represent the most surgically challenging pediatric elbow injuries, yet controversy persists regarding whether the timing of surgery, specifically after-hours versus working-hours operations, influences clinical and radiological outcomes. This study aimed to compare the functional, cosmetic, and radiological outcomes of Gartland Type III SCHFs managed during working hours versus after hours, with a secondary analysis incorporating the surgical approach (open vs. closed reduction). Methods: A retrospective cohort s

Early clinical and radiographic outcomes of an augmented baseplate in reverse shoulder arthroplasty for glenohumeral arthritis with glenoid deformity.

BACKGROUND: Glenoid deformity is commonly encountered in patients undergoing reverse shoulder arthroplasty (RSA). Augmented baseplates can correct glenoid deformity while potentially avoiding certain complications encountered with structural bone graft. Limited evidence exists to support the use of metallic augmented baseplates in RSA. METHODS: We performed a retrospective review to identify all patients treated with an augmented baseplate during primary RSA with a minimum of 1 year of clinical and radiographic follow-up. Preoperative radiographs and advanced imaging were used to determine gl

[The use of the sequential mathematical analysis for the determination of the driver's seat position inside the car passenger compartment from the injuries to the extremities in the case of a traffic accident].

The objective of the present study was to determine the diagnostic coefficients (DC) for the injuries to the upper and lower extremities of the vehicle drivers inflicted inside the passenger compartment in the case of a traffic accident. We have analysed the archival expert documents collected from 45 regional bureaus of forensic medical expertise during the period from 1995 to 2014 that contained the results of examination of 200 corpses and 300 survivors who had suffered injuries in the traffic accidents. The statistical and mathematical treatment of these materials with the use of sequentia

Three-dimensional suitability assessment of three types of osteochondral autograft for ulnar coronoid process reconstruction.

BACKGROUND: Osteochondral autografts with use of the olecranon tip, lateral radial head, or proximal radial head have been employed for coronoid process reconstruction. However, it is unclear which autograft is most suitable for coronoid articular configuration. We assessed 3-dimensional articular facet suitability of 3 osteochondral autografts for coronoid process reconstruction. METHODS: We performed 3-dimensional computed tomography of 20 elbows to compare the articular facet configuration between the coronoid process and the ipsilateral olecranon tip, lateral radial head, and proximal rad

Fractures of the distal third of the humerus with palsy of the radial nerve: management using minimally-invasive percutaneous plate osteosynthesis.

Fractures of the distal third of the humerus may be complicated by complete lesions of the radial nerve which may be entrapped or compressed by bone fragments. Indirect reduction and internal fixation may result in a permanent nerve lesion. We describe the treatment of these lesions by insertion of a bridge plate using the minimally-invasive percutaneous technique. Six patients were operated on and showed complete functional recovery. Healing of the fractures occurred at a mean of 2.7 months (2 to 3) and complete neurological recovery by a mean of 2.3 months (1 to 5). In one patient infection

Treatment of cubitus varus using the Ilizarov technique of distraction osteogenesis.

Seven children with a post-traumatic cubitus varus deformity were treated using the Ilizarov technique of distraction osteogenesis. The outcome was rated as excellent in each case and all were satisfied with the cosmetic appearance. No complications had been encountered by the latest follow-up at a mean of 66.7 months. This technique seems reliable for the treatment of such deformities, provided that it achieves full correction by gradual distraction. Nerve palsy and unsightly scars are avoided, and the range of movement of adjacent joints is preserved.

The reverse Sauvé-Kapandji procedure for the treatment of (posttraumatic) proximal radioulnar synostosis.

Proximal radioulnar synostosis is a rare but highly disabling posttraumatic complication in periarticular elbow injuries. Surgical treatment is an option for functionally limiting proximal radioulnar synostosis; however, the approach can endanger local neurovascular structures, especially if the synostosis affects the level of the bicipital tuberosity. We report two cases of proximal radioulnar synostosis with a preoperative prono-supination range of motion of 0° and 15° treated by a reverse Sauvé-Kapandji procedure resecting a 1-cm section of the radial shaft distal to the bicipital tuberosit
