Immediate repair and early mobilization of the extensor pollicis longus tendon in zones 1 to 4.

We present the results of repair and early mobilization of 100 extensor pollicis longus (EPL) tendon injuries in zones 1 to 4 in 100 patients using a dynamic outrigger splint which controlled metacarpophalangeal joint movements but allowed free movement of the interphalangeal joint. Eighty-two were complete divisions of the tendon and 18 were 80% to 99% tendon divisions. Analysis of measurements obtained routinely at 8 weeks showed 81% excellent and good results using the TAM system. There were 90% excellent and good results in the 72 patients, who were followed-up and received therapy for 12

Early motion in the treatment of fractures and dislocations in the hand and wrist.

The concept of early motion is valuable in the treatment of fractures of the hand and wrist. It is a means to an end, however, and should be employed principally when the benefits outweigh the risks. The goal is a hand that functions as near to normally as possible, considering the injury. All treatments need to be tailored to the individual patient.

An autosomal dominant facio-audio symphalangism syndrome with Klippel-Feil anomaly: a new variant of multiple synostoses.

A family is reported in which at least the propositus, his mother and his grandfather suffer from proximal symphalangism, conductive hearing loss due to stapes fixation, Klippel-Feil anomaly and abnormality of the nose with lack of alar flare. It is noteworthy that the first metacarpal bone is not abnormal. This association is delineated from two other "facio-audio-symphalangism" syndromes and from Wildervanck syndrome. Our observation confirms one previous description only by Pierson et al. (1981).

Medulloblastoma in association with the Coffin-Siris syndrome.

An 8-year old patient with Coffin-Siris syndrome is presented, who was found to have a medulloblastoma during the evaluation of apnea and ventilator dependency. Although several cases of Dandy Walker cysts and one case of brain-stem heterotopia have been described in this rare syndrome, this is the first report of medulloblastoma in a patient with Coffin-Siris syndrome. Possible pathogenetic mechanisms are briefly discussed.

Bone marrow lesions and collateral ligament lesions are associated in interphalangeal joints with osteoarthritis: The Hand OSTeoArthritis in Secondary care cohort.

OBJECTIVES: To investigate whether bone activity adjacent to collateral ligaments is present and results in collateral ligament lesions (CLLs) of the proximal and distal interphalangeal joints in patients with hand osteoarthritis (OA), and vice versa. METHODS: We used data measured on baseline, year two and year four from the Hand OSTeoArthritis in Secondary care cohort. MR images of the right hand were scored at the radial and ulnar 1/3rd of each joint ("=joint side") for bone marrow lesions (BMLs) and CLLs (=non-visible or non-continuous ligament). Odds ratios (ORs) with 95% confidence inte

How may complementary magnetic resonance imaging findings facilitate the diagnosis of inflammatory hand arthritis involving the distal interphalangeal joint? A prospective cohort study.

PURPOSE: In this study, we aimed to characterize the role of magnetic resonance imaging (MRI) in making a specific diagnosis of inflammatory hand arthritis (IHA), particularly in early stages or ambiguous cases. METHODS: Patients aged ≥18 years with suspicious IHA in at least one joint were enrolled in this single-center prospective study. Three Tesla MRI (3T-MRI) with a fine-tuned protocol was utilized, whereby differential diagnoses were made by radiologists according to the predominant involvement of synovium or synovioentheseal complex (SEC) and/or specific degenerative findings. The Use of Relative Motion Flexion Orthoses for Chronic Boutonniere Deformity.

PURPOSE: This study investigated the effectiveness of a relative motion flexion orthosis (RMFO) for increasing the range of motion for boutonniere deformity. METHODS: We included 28 patients aged 13-62 years with chronic boutonniere deformity who could complete 0° proximal interphalangeal (PIP) joint extension with the pencil test and were stage 1 according to the Burton classification of boutonniere deformity. At the initial hand therapy appointment, the RMFO was made. The duration of the orthosis usage at the initial therapy session, after stopping the use of the orthosis (posttreatment), a

A novel treatment for idiopathic knuckle pads with cantharidin-podophylotoxin-salicylic acid.

Knuckle pads are benign subcutaneous nodules that appear most frequently on the small joints of the hands. In children, they are often idiopathic, and no universally effective treatment has been reported. We present the case of an adolescent successfully treated with a combination of topical cantharidin -podophylotoxin -salicylic acid.

Sick Leave After Surgery for Thumb Carpometacarpal Osteoarthritis: A Population-Based Study.

PURPOSE: Patients undergoing surgery for thumb carpometacarpal (CMC1) osteoarthritis often require sick leave owing to postoperative immobilization, pain, and decreased function. Our goal was to evaluate the amount of sick leave after surgery for 2 common CMC1 arthroplasty procedures. METHODS: Using registry data from the Skåne region of southern Sweden, cross-linked with employment data showing person-specific sick leave, 2 cohorts of CMC1 surgical patients, between ages 40 and 59 years, were examined. These comprised all persons undergoing soft tissue arthroplasty and prosthetic implant art

Van der Woude and Popliteal Pterygium Syndromes.

: Van der Woude syndrome is the most generic form of syndromic orofacial cleft, present in approximately 2% of all cleft patients. The lower lip pits with or without cleft lip and/or palate is typical of this syndrome. Popliteal pterygium syndrome (PPS), also known as popliteal web syndrome or fasciogenito-popliteal syndrome, was first described by Trelat in 1869, the incidence is approximately 1 in 300,000 live births. The term PPS was coined by Gorlin et al in 1968 based on the most unusual anomaly, the popliteal pterygium. Popliteal pterygium syndrome shares features with van der Woude synd

Popliteal Pterygium Syndrome With Syngnathia.

Popliteal pterygium syndrome is a condition characterized by skin webs on the popliteal fossa, which may impair mobility unless surgically repaired. Affected individuals may also have syndactyly on the fingers and/or toes. Most people with this disorder present cleft lip and cleft palate and they can have syngnathia, that is a congenital adhesion between maxilla and mandible by fibrous bands, which affects the opening of the mouth. The case that we report is about a 2-month-old male, who presented skin webs bilaterally on the popliteal fossa, syndactyly between the IV and the V toe of the righ

Corticoplasty for improved appearance of hands with Ollier disease.

PURPOSE: Only a few treatment options are available for Ollier disease, especially for improving appearance. The purpose of this study was to evaluate the efficacy of partial resection of the cortical bone with curettage of the tumor (corticoplasty) for treating hand deformity in Ollier disease. METHODS: Five hands (3 patients, 30 metacarpals and phalanges) were examined. Windows of thin cortical bone were elevated, the tumors were excised, and the cortex was trimmed and replaced to enhance the appearance. Artificial bone substitute was used as the graft material for 17 lesions to fill the va

[Littler tenodesis for correction of swan neck deformity in rheumatoid arthritis].

PURPOSE: The aim of this study was to assess the results of operative treatment for rheumatoid swan neck deformity using Littler's technique consisting in the reconstruction of the oblique retinacular ligament. PATIENTS AND METHOD: From 2004 to 2007 twenty rheumatoid patients with 30 PIP-joints affected by swan neck deformity underwent surgical correction. In all cases the tenodesis described by Littler was used. Modification of the operative procedure because of insufficiency of the Cleland ligament or the A2-pulley was in no case necessary. Twenty six PIP-joints in 17 patients could be exam

A 10-year experience with the surgical treatment of radial polydactyly.

PURPOSE: Authors' clinical and surgical experience in correction of the radial polydactyly in its various degrees of severity is presented. MATERIAL AND METHODS: Nineteen patients were operated due to thumb duplication in a 10-year period from 1996 to 2006. The cases were classified according to Wassel 7-type classification. The surgical techniques adopted were the Bilhaut-Cloquet and the method of exclusion of the more hypoplasic thumb with preservation of the more functional and aesthetic one. RESULTS: In 95% of the cases, the performed procedures meant to sacrifice the finger that was mor

A clinical report of the effect of mechanical stress on functional results after fasciectomy for Dupuytren's contracture.

Early postoperative treatment after Dupuytren's fasciectomy traditionally has included the application of mechanical stress to digital extension with splints and exercise. This study examines the effect of mechanical stress, which may compromise nutrient delivery to the tissues, on inflammation, flare, hypertrophic scar, digital range of motion (ROM), and therapy visits. The authors compared functional outcomes in operated digits treated postoperatively with tension applied (TA) and no tension applied (NTA), retrospectively from 1983 to 1993 (TA only) and prospectively from 1993 to 1999 (TA an

[The dynamic treatment of intraarticular fractures of the base of the middle phalanx with the Suzuki dynamic fixator].

Between 1994 and 1998, we have treated eleven patients with intraarticular fractures of the base of the middle phalanx including impaction, dislocation, and pilon types of injuries. All patients were evaluated after a median follow-up period of 25.8 (8 to 57) months. Treatment was carried out according to Suzuki's technique with a dynamic PIP-joint distraction fixator consisting of Kirschner wires and rubber bands. In five cases, there was additional osteosynthesis (Kirschner wires, resorbable hemicerclage) or cancellous bone-grafting for reconstruction of the joint surface. Early mobilisation

[Kapandji enlargement plasty of A1 pulley. Results in 15 primary trigger fingers with a 5 year (2-8 years) follow-up].

We retrospectively studied 15 enlargement plasties of the A1 pulley performed in a university orthopaedic department for trigger finger. The study objectives where: 1)to evaluate whether closure of the enlarged A1 pulley led to recurrent trigger finger; 2) to determine whether the complexity of the procedure was responsible for complications. Fifteen trigger fingers operated by enlargement of the A1 pulley according to Kapandji, instead of opening of the A1 pulley, were reviewed after 5 years of follow-up (2-8 years) by an observer who did not participate in surgery. These 15 fingers (6 thumbs

Aplasia cutis congenita, cleft palate, epidermolysis bullosa, and ectrodactyly: a new syndrome?

Aplasia cutis congenita (ACC) is a skin disorder in which localized or widespread areas of skin are absent at birth. It has been associated with numerous anomalies and recognizable syndromes. We report a newborn infant with ACC of the scalp, multiple facial abnormalities including cleft palate but not cleft lip, hypoplasia of the distal phalanges of the hands, ectrodactyly of the feet, and epidermolysis bullosa of the extremities and lower trunk. Although this patient had some features that overlapped with ectrodactyly-ectodermal dysplasia-clefting syndrome, the absence of cleft lip and presen
