OBJECTIVES: We evaluated the radiographic and functional results of locked intramedullary nailing in the treatment of subtrochanteric fractures of the femur.
METHODS: Nineteen patients (14 men, 5 women; mean age 51.8 years; range 20 to 87 years) underwent locked intramedullary nailing for subtrochanteric fractures of the femur. The fractures were on the right in nine patients and on the left in 10 patients. The injuries were caused by a simple fall (n=12), motor vehicle accident (n=4), falling from a height (n=2), and gun shot (n=1). Six patients had associated fractures and nine patients had

During a two-year period 237 patients were admitted for intertrochanteric fracture of the femur. 196 patients were included in this study with an average age of 79 years. Surgery comprised 163 nailings (Ender-Kempf), 31 blade plates and 2 prostheses. In this study the mortality rate and general complications were essentially related to the age and general condition of the patient; the general complications did not differ between methods. Walking ability at 6 months chiefly depended on the patient's general condition before the fall. The techniques allowing immediate weight-bearing diminished t

Fifty femur fractures were treated with the Brooker-Wills intramedullary locking nail. The indications included subtrochanteric, subisthmal, segmental or comminuted acute fractures or nonunions, and intramedullary shortening procedures. Eighteen technical problems in 13 (26%) patients were encountered during insertion of the nail. The incidence of technical problems was high early in the series and in procedures done with the patient in the supine position. Adverse clinical results from technical errors occurred in two patients. Most technical errors seem avoidable with careful technique. The

One hundred twenty-seven patients with pertrochanteric femoral fractures were transfixed with the Ender nail. The mean age of the patients was 78.9 years. Thirty-five percent of the fractures were of the stable type, and 65% were of the unstable type. All patients had clinical and radiologic follow-up examinations every three months for one year. Successful closed reduction was achieved in 97.6% of the patients. Perioperative supracondylar femoral fractures occurred in two patients. The overall stay in hospital was 19 days. Technical failures occurred early in five patients, and late manifesta

BACKGROUND: Femur shaft factures (FSF) are common injuries following high-energy mechanisms mainly involving motor vehicle crashes (MVC). We evaluated the timings of nailing management and analyzed the pattern of fracture union and outcome in a level 1 trauma center.
METHODS: This was a retrospective observational study of all the admitted trauma patients who sustained femoral fractures between January 2016 and September 2020. Data were analyzed and compared based on time to Intramedullary Nailing (IMN) (<12 h, 12-24 h and >24 h) and outcomes of FSF (union, delayed union and nonunion).

INTRODUCTION: Paget's disease is a metabolic bone disorder characterized by abnormal patterns in bone remodeling, resulting in variable degrees of chronic bone pain, deformation of the long bones and rarely, and pathologic fracture. These issues can pose difficult surgical challenges, particularly in the elderly frail population, where the benefits of orthopedic intervention must be balanced with minimizing inherent surgical risks. Such considerations often include reducing operative time and blood loss, allowing for early mobilization, stabilizing an impending fracture, and providing symptom

BACKGROUND: The role of simulation in orthopaedic surgical training is becoming increasingly evident, as simulation allows repeated sustained practice in an environment that does not harm the patient. Previous studies have shown that the cognitive aspects of surgery are of equal if not greater importance in developing decision making than the practical aspects.
AIM: To observe construct, content and face validity of four IFN modules on a cognitive simulator, Touch Surgery™.
METHODS: 39 novices and 10 experts were recruited to complete four simulation modules on surgical decision-making that

BACKGROUND: Osteonecrosis of the talus is a well-recognized pathology, which can result in significant hindfoot collapse resulting in poor function and pain. Treatment with intramedullary tibiotalocalcaneal arthrodesis (IMTCA) using a retrograde intramedullary nail is widely utilized for severe concomitant tibiotalar and subtalar pathologies. This study reports the results of ankle and hindfoot arthrodesis in patients with arthritis and deformity caused specifically by talar osteonecrosis.
METHODS: Fourteen ankle and hindfoot arthrodeses with retrograde intramedullary nail were studied, with

Fractures of the clavicle in the middle third usually undergo closed treatment by external fixation, because they heal spontaneously in most cases. However, a correct reposition and fixation by external manner is sometimes impossible, and consolidation in malposition with functional and esthetic disturbance occurs. External fixation can be painful and inconvenient to the patient, and congestion and paresthesia of the upper limbs may occur. Therefore the indication for internal fixation has been extended on fractures in persisting severe malposition despite external fixation and painful congest

INTRODUCTION: The primary purpose of this study was to report the indications for and the rate of polyethylene exchange (PE) during retrograde intramedullary nailing (rIMN) in periprosthetic distal femur fractures. This study also explores the role of polyethylene-related etiologies of patients experiencing prefracture TKA instability.
METHODS: Retrospective review was done on patients older than 18 years treated with rIMN or nail-plate combination (NPC) for periprosthetic fracture (PPFx) of the femur above a total knee arthroplasty with well-fixed components from 2013 to 2023. Patient demogr

BACKGROUND: Treatment of ankle and pilon fractures in the setting of diabetes mellitus (DM) is challenging due to a propensity for postoperative complications. Limb salvage is a primary concern following these injuries, as below knee amputation (BKA) occurs at an unacceptably high rate. Primary retrograde tibiotalocalcaneal (TTC) joint nailing without articular preparation has emerged as a solution to treat diabetics with ankle and pilon fractures to mitigate surgical complications and prevent BKA. The technique minimizes surgical dissection and has previously demonstrated utility in fragility

PURPOSE: Fracture models in animals are essential to analyze bone healing in musculoskeletal research fields. Especially in small animals, fractures are difficult to simulate and stabilize. Therefore, a fracture model is desirable with a short operation time, high safety of the model without stabilization failure and low costs. Aim of this study is the evaluation of a new open tibial shaft model in mice for musculoskeletal research.
METHODS: In 12 eight week-old wild type mice, an open tibial shaft fracture was simulated and stabilized with a retrograde over the fracture inserted intramedulla

PURPOSE: Open tibia fracture is prone to infection, consequently causing significant morbidity and increasing the hospital stay, occupational loss and onset of chronic osteomyelitis. Intramedullary nailing is one choice for treating tibia shaft fractures. To improve the delivery of antibiotics at the tissue-implant interface, many methods have been proposed as a part of prophylaxis against infection. This study was conducted to study the role of gentamicin-impregnated intramedullary interlocking (IMIL) nail in the prevention of infection in Gustilo type I and II open tibia fractures and to com

Stabilization of diaphyseal long-bone fractures using elastic stable intramedullary nails (ESIN) in children promises early mobilization and rapid resumption of full weight bearing. We evaluated the duration of postoperative functional rehabilitation after ESIN, measured by the time from stabilization until first partial weight bearing, full weight bearing, and resumption of school sports. Fifty children with unstable, displaced fractures of the femur or lower leg treated with ESIN between 2002 and 2012 were included in this retrospective analysis. We classified fractures according to the pedi

INTRODUCTION: Pertrochanteric fractures after low-energy trauma are common among osteoporotic patients. Although the use of intramedullary devices to treat such fractures is becoming increasingly popular, there is a paucity of data comparing the outcomes of the use of short cephalomedullary nails (SCN) with the use of long cephalomedullary nails (LCN). This study aimed to compare the outcomes of treatment using LCN with treatment using SCN for patients with osteoporotic pertrochanteric fractures.
METHODS: A retrospective review of 64 patients with osteoporotic pertrochanteric fractures who we
