Tuberosity refixation in reverse proximal humerus fracture arthroplasty: a prospective cohort study with two-year follow-up.

INTRODUCTION: To evaluate the outcomes of a simplified tuberosity refixation technique using a two-suture construct in patients with complex proximal humerus fractures treated with reverse shoulder arthroplasty (RSA). The technique reduces the number of fixation sutures rather than the traditional four to five, aiming to decrease operative complexity while promoting greater tuberosity (GT) healing and improving the healing rate. A secondary exploratory analysis compared clinical and radiological outcomes between cemented and uncemented humeral stem fixation. MATERIALS AND METHODS: This prospe

The Hounsfield value for cortical bone geometry in the proximal humerus--an in vitro study.

INTRODUCTION: Fractures of the proximal humerus represent a major osteoporotic burden. Recent developments in CT imaging have emphasized the importance of cortical bone thickness distribution in the prevention and management of fragility fractures. We aimed to experimentally define the CT density of cortical bone in the proximal humerus for building cortical geometry maps. METHODS: With ethical approval, we used ten fresh-frozen human proximal humeri. These were stripped of all soft tissue and high-resolution CT images were then taken. The humeral heads were then subsequently resected to allo

Revision Reconstruction of a Humeral Simple Bone Cyst Using Fibular Strut Graft, Iliac Crest Bone Graft, and Posterior Plating in a Young Adult: A Case Report.

INTRODUCTION: Simple bone cysts, also known as unicameral or solitary bone cysts, are benign, fluid-filled lesions typically found in long bones, particularly in children and adolescents. The size, location, and symptoms of the cyst determine the course of treatment; approaches range from observation and periodic monitoring to surgical intervention. CASE REPORT: We report a case treated for a humeral simple bone cyst. A 15-year-old female patient has a history of left humerus titanium elastic nailing system (TENS) nailing for midshaft humerus fracture 9 years ago. Single TENS nail removal was

A biomechanical investigation of the surface strains on the acromion and scapular spine during simulated ex-vivo arm motion.

While several biomechanical investigations have measured acromion and scapular spine strains for various pathological conditions to better understand the risk factors for fracture, no study has measured strains in the native shoulder. The objective of this study was to use an ex-vivo shoulder motion simulator to measure principal strain during continuous, unconstrained, muscle-driven motion of the native shoulder. Eight cadaveric specimens (57 ± 6 years) were used to simulate scapular plane abduction (27.5 to 80° of humerothoracic elevation), forward flexion (27.5 to 72.5° of humerothoracic el

Implant retention in a rabbit model of fracture-related infection.

AIMS: Fracture-related infection (FRI) is commonly classified based on the time of onset of symptoms. Early infections (< two weeks) are treated with debridement, antibiotics, and implant retention (DAIR). For late infections (> ten weeks), guidelines recommend implant removal due to tolerant biofilms. For delayed infections (two to ten weeks), recommendations are unclear. In this study we compared infection clearance and bone healing in early and delayed FRI treated with DAIR in a rabbit model. METHODS: Staphylococcus aureus was inoculated into a humeral osteotomy in 17 rabbits after plate o

A new local muscle flap for elbow coverage-the medial triceps brachii flap: anatomy, surgical technique, and preliminary outcomes.

BACKGROUND: The medial triceps brachii is vascularized by the middle collateral artery and the arterial circle of the elbow. This vascularization allows a distal pedicled use to cover soft tissue defects of the elbow. We report our experience using this flap to cover traumatic and postsurgical wounds. METHODS: Patients who underwent a pedicled medial triceps brachii flap procedure between 2008 and 2015 were included. Data concerning characteristics of the patients, wound size, surgical technique, and complications were retrospectively reviewed. An independent observer examined patients and as

Triceps-sparing ulnar approach for total elbow arthroplasty.

To prevent insufficiency of the triceps after total elbow arthroplasty, we have, since 2008, used a triceps-sparing ulnar approach. This study evaluates the clinical results and post-operative alignment of the prosthesis using this approach. We reviewed 25 elbows in 23 patients. There were five men and 18 women with a mean age of 69 years (54 to 83). There were 18 elbows with rheumatoid arthritis, six with a fracture or pseudoarthrosis and one elbow with osteoarthritis. Post-operative complications included one intra-operative fracture, one elbow with heterotopic ossification, one transient ul

Prediction of Long Bone Fractures in Multiple Myeloma Patients in an Advanced Imaging World.

BACKGROUND: Multiple myeloma (MM) affects the long bones in 25% of patients. The advent of positron-emission tomography/computed tomography (PET/CT) scanners offers the possibility of both metabolic and radiographic information and may help determine fracture risk. To the best of our knowledge, no published study correlates these two factors with long bone fractures. OBJECTIVES: To evaluate the impact of PET/CT on fracture risk assessment in multiple myeloma patients. METHODS: We identified all bone marrow biopsy proven multiple myeloma patients from 1 January 2010 to 31 January 2015 at a si

Neurological and vascular injury associated with supracondylar humerus fractures and ipsilateral forearm fractures in children.

BACKGROUND: Approximately 5% of supracondylar humerus fractures in children are associated with an ipsilateral forearm fracture, often referred to as a floating elbow when both injuries are displaced. Historically, these patients have higher complication rates than patients with an isolated supracondylar humerus fracture. The purpose of this study was to review the acute neurologic and vascular injuries in patients with ipsilateral, operative supracondylar humerus and forearm fractures and compare the findings with a cohort of isolated, operative supracondylar humerus fractures. METHODS: We p

Bone Morphology and Strength in the Mid-Diaphysis of the Humerus and Metacarpus in Dairy Calves Prior to Weaning.

Calf growth rate in relation to future milk production has been thoroughly studied; however, the observation of growth arrest lines in bones from heifers with humeral fractures has highlighted the need to understand bone growth in relation to calf growth. The aim of this study was to describe the relationship of peripheral quantitative computed tomography (pQCT)-derived measures of bone strength and morphology with gross measurements of size and growth in pre-weaning dairy calves. Liveweight, height, body length, girth and leg length were measured at one, six and twelve weeks of age. At these …

Allograft reconstruction of the humerus: Complications and revision surgery.

BACKGROUND AND OBJECTIVES: Allograft reconstruction of the humerus after resection is preferred by many because of bone stock restoration and biologic attachment of ligaments and muscles to the allograft, theoretically obtaining superior stability and functionality. Our aim was to assess the prevalence of complications and the incidence and etiology for revision surgery in humeral allograft reconstructions. METHODS: We included patients 18 years and older who underwent wide resection and allograft reconstruction of the humerus for primary and metastatic lesions at our institution between 1990

Topical nitric oxide application in the treatment of chronic extensor tendinosis at the elbow: a randomized, double-blinded, placebo-controlled clinical trial.

BACKGROUND: Extensor tendinosis ("tennis elbow") is a degenerative overuse tendinopathy of the wrist extensors at their attachment to the lateral humeral epicondyle. No treatment has been universally successful. Topical application of nitric oxide has been used effectively to treat fractures and cutaneous wounds in animal models, presumably by stimulation of collagen synthesis in fibroblasts. PURPOSE: To determine whether topical nitric oxide can improve outcome of patients with extensor tendinosis. STUDY DESIGN: Prospective, randomized, double-blinded clinical trial. METHODS: Eighty-six pa

Natural history and treatment of fibrous dysplasia of bone: a multicenter clinicopathologic study promoted by the European Pediatric Orthopaedic Society.

A multicenter study on fibrous dysplasia of bone (FD) was promoted by the European Pediatric Orthopaedic Society in 1999 in order to gain insight into the natural history of the disease and to evaluate current diagnostic and therapeutic approaches. We collected and reviewed clinical, radiographic, pathological, and molecular genetic data when possible, from a total of 64 cases diagnosed as either monostotic FD (MFD), polyostotic FD (PFD), or McCune-Albright syndrome (MAS), evaluated or treated in 11 participating centers. Results from the initial analysis of the series indicate five main point
