UHR as a systemic sensor of cumulative heat exposure and subclinical cardiovascular injury: evidence from 64,088 adults.

BACKGROUND: The increasing frequency and intensity of heatwaves threaten global cardiovascular (CV) health. While the link between acute heatwaves and clinical CV events is known, the effect of cumulative heat exposure on early, subclinical CV injury is poorly understood. We investigated the dose-response relation between sustained thermal burden and subclinical CV damage and identified the main metabolic-inflammatory pathways involved. METHODS: In this large-scale, population-based retrospective cross-sectional study of 64,088 adults from Sichuan, China, we quantified cumulative heat exposur

A longitudinal cohort study of damp and mouldy housing and children's respiratory health in Bradford, UK.

Damp and mould compromise indoor air quality (IAQ) in millions of European homes. Epidemiological evidence links damp and mould exposure to increased risk of respiratory diseases and symptoms in children. However, longitudinal evidence from the UK remains limited, despite its older housing stock and wet, humid climate. We investigated associations between damp and mould and respiratory health in the Born in Bradford birth cohort (Bradford, UK) among 2594 children aged 3-5, with follow-up for new disease development age 7-11 (n = 1543). We report descriptive analyses of damp and/or mould by hou

Large mammalian herbivores influence the deployment of microbial extracellular enzymes to control soil carbon cycling.

UNLABELLED: Large mammalian grazers have regulated biogeochemical cycles across 40% of the Earth's terrestrial surface for millennia. While grazer effects on organic matter entering the soil are becoming clear, their effect on the downstream microbial foraging through extracellular-enzymes (EE), which link processes at the cellular level to ecosystem-level flow of matter remain less clear. We assessed how large mammalian grazers influence microbial foraging in soil using two lines of evidence. First, utilising a 14-year-old long-term grazer-exclusion experiment in the Trans-Himalayan ecosystem

Agrivoltaic systems in a tropical region: Influence on thermal environment, vaginal temperature, and behavior of dairy heifers.

This study aimed to evaluate the influence of different agrivoltaic system configurations on the thermal environment, vaginal temperature, and behavior of dairy heifers raised on pasture in a tropical climate. The study was carried out between July and August 2025, covering the winter season in the Southern Hemisphere. Microclimatic variables were measured, and the black globe-humidity index (BGHI) was calculated to determine thermal comfort. Additionally, vaginal temperature and behavior of 21 dairy heifers were measured in a crossover experimental design and compared among 3 treatments: an a

Extreme heat and cause-specific risk of hospital admission in the adult population in England: a case time series analysis.

OBJECTIVES: This study investigated the impact of heat on the risk of hospital admission due to a range of health conditions in England. DESIGN: We used records of over 4 million hospital admissions in the summer months between 2008 and 2019, to construct daily time series of admissions in 32 837 census areas. Coupled with high-resolution environmental data, we conducted a case time-series analysis using distributed-lag non-linear models to measure the lagged relationship between summertime temperature and risk of admission for a broad set of health conditions. We derived the relative risks o

Associations between extreme weather events, mobility, and resource insecurities with HIV vulnerabilities among older adults in Moshi, Tanzania: Cross-sectional findings.

BackgroundEnvironmental and social hazards shape HIV vulnerabilities in Sub-Saharan Africa. Although older adults (>55) have an HIV prevalence higher than the national average in Tanzania, they are understudied in HIV research. To address this knowledge gap, we examined associations between extreme weather event exposure, mobility, resource insecurities, and HIV vulnerabilities among older adults in Moshi, Tanzania.MethodsWe conducted a cross-sectional survey in Moshi with adults aged ≥50 randomly sampled from the Prospective Urban Rural Epidemiology cohort. We conducted multivariable linear a

Ageing, health system implications, and a changing epidemic: organizational transitions and stigma dynamics in Philippine HIV care.

BACKGROUND: The global HIV epidemic is undergoing a demographic transition as expanded access to antiretroviral therapy (ART) has increased life expectancy among people living with HIV (PLWH). As a result, the population of older adults living with HIV (OPLWH) is rapidly growing, introducing new clinical, psychosocial, and public health challenges. In the Philippines, one of the fastest-growing HIV epidemics in the Asia-Pacific region, this shift is occurring alongside rising incidence, creating a dual burden of ongoing transmission and emerging survivorship needs. METHODS: This study employe

Transforming public mental health: a review on global trends, challenges, and pathways to change.

This review synthesizes global trends, persistent challenges, and actionable pathways for overhauling public mental health systems, with a particular focus on low- and middle-income countries (LMICs). Thematic analysis of our review revealed that mental health disorders now affect nearly one in eight people worldwide, yet up to 75% of those in LMICs receive no treatment due to stigma, underfunding, workforce shortages, and fragmented systems, perpetuating a widening "treatment and care gap." Social inequities, harmful cultural norms, conflict, climate change, and gender disparities further amp

Hostility in the ICU Waiting Room: Extrapunitive and Intropunitive Reactions Among Family Members.

Background/Objectives: Families of ICU patients endure intense psychological strain. While anxiety and depression are well documented, less attention has been given to hostility-expressed both outwardly as anger and inwardly as guilt or self-criticism. Hostility, however, often shapes the climate of the ICU waiting room and the collaboration between families and staff. This study examined the levels and forms of hostility among ICU relatives, focusing on demographic predictors that may influence extrapunitive and intropunitive reactions. Methods: A cross-sectional study was conducted between J

Enhanced crisis resilience of general practitioner-centred care: a retrospective cohort study of patients with coronary artery disease during the COVID-19 pandemic in Germany.

BACKGROUND: Structured, comprehensive provision of primary care services has been shown to provide better outcomes in chronic disease management. In 2004, Germany introduced a programme of general practitioner (GP)-centred healthcare to strengthen the primary care sector. Crises such as pandemics, world conflict and climate events can result in significant challenges for the provision of routine healthcare requiring rapid reorganisation of existing models of care provision. The objective of this study was to assess the impact of the COVID-19 pandemic on the provision of chronic disease surveil

Estimating the Exposure-Response Relationship between Fine Mineral Dust Concentration and Coccidioidomycosis Incidence Using Speciated Particulate Matter Data: A Longitudinal Surveillance Study.

BACKGROUND: Coccidioidomycosis, caused by inhalation of Coccidioides spp. spores, is an emerging infectious disease that is increasing in incidence throughout the southwestern US. The pathogen is soil-dwelling, and spore dispersal and human exposure are thought to co-occur with airborne mineral dust exposures, yet fundamental exposure-response relationships have not been conclusively estimated. OBJECTIVES: We estimated associations between fine mineral dust concentration and coccidioidomycosis incidence in California from 2000 to 2017 at the census tract level, spatiotemporal heterogeneity in

Extreme Temperatures and Stroke Mortality: Evidence From a Multi-Country Analysis.

BACKGROUND: Extreme temperatures contribute significantly to global mortality. While previous studies on temperature and stroke-specific outcomes presented conflicting results, these studies were predominantly limited to single-city or single-country analyses. Their findings are difficult to synthesize due to variations in methodologies and exposure definitions. METHODS: Within the Multi-Country Multi-City Network, we built a new mortality database for ischemic and hemorrhagic stroke. Applying a unified analysis protocol, we conducted a multinational case-crossover study on the relationship b

Longitudinal assessment of psychological distress and its determinants in a sample of firefighters based in Montreal, Canada.

INTRODUCTION: Firefighters face elevated risks of common mental health issues, with distress rates estimated at around 30%, surpassing those of many other occupational groups. While exposure to potentially traumatic events (PTEs) is a well-recognized risk factor, existing research acknowledges the need for a broader perspective encompassing multidimensional factors within the realm of occupational stress. Furthermore, this body of evidence heavily relies on cross-sectional studies. This study adopts an intensive longitudinal approach to assess psychological distress and its determinants among

Regional variation in the landscape ecology of West Nile virus sentinel chicken seroconversion in Florida.

How landscape composition and configuration impact the distribution of multi-vector and multi-host mosquito vector-borne disease systems, such as West Nile virus (WNV), remains challenging because of complex habitat and resource requirements by hosts and vectors that affect transmission opportunities. We examined correlations between landscape composition and configuration and 2018 WNV sentinel chicken seroconversion in Florida, USA across the state and within five National Oceanic Atmospheric Administration (NOAA) bioclimatic regions to understand strength and variation of landscape effects d
