Hypersensitivity reactions to aluminum are rare. Sensitization has been reported during the continuous application of aluminum-containing antiperspirants or after the injection of aluminum-adsorbed vaccines and allergen extracts containing aluminum. We report a rare case of a nonhealing ulcer at the site of an accidental molten aluminum burn that showed changes of eosinophilic cellulitis and panniculitis histologically. This process was followed by a generalized vesicular and pustular id reaction. The reactions resolved after 1 month of systemic corticosteroid therapy. Two months later, there

BACKGROUND: Among the textile dyes, disperse dyes are common sensitizers.
OBJECTIVE: To investigate whether patch testing with a textile dye mix consisting of eight disperse dyes would be equivalent to testing with the separate ingredients of the mix at the concentrations used in the mix.
METHODS: Researchers tested 1,780 consecutive patients with a mix consisting of Disperse Blue 35, Disperse Yellow 3, Disperse Orange 1, Disperse Orange 31 (mislabeled as Disperse Orange 3), Disperse Red 1 and 17, all at 0.5%, and Disperse Blue 106 and 124, both at 0.1%, and with the ingredients at these con

In spite of the worldwide popularity of soccer among youth and the common requirement of the use of protective shin guards, reports of allergy to shin guards is sparse. This is surprising in light of the fact that this equipment is often made of materials that are reported to cause allergies, and that friction and moisture from the use of these shin guards during the sport would seem to predispose soccer players to the development of an allergic response. We present eight patients that presented for evaluation of dermatitis under their shin guards--some of which had allergy to their shin guard

BACKGROUND: Rising use of sunscreen products has led to increased reporting of adverse reactions to sunscreens.
OBJECTIVE: To investigate possible photoallergic reactions in patients who identified themselves as "being allergic" to sunscreens.
METHODS: Patients filled out questionnaires about types of sunscreens they used and timing of their "allergic" reactions. Next, they consented to be photopatch-tested with active sunscreen ingredients, including the new sunscreen Anthelios SX (containing Mexoryl SX) and the new ultraviolet filters Tinosorb M and Tinosorb S. Standard allergen patch test

Bisabolol is an active plant extract isolated from German chamomile and thought to have antiinflammatory and skin-soothing properties. It is a common additive in many products, including moisturizing creams and ointments, lotions, cleansers, sunscreens, antiperspirants, and makeup products. Contact dermatitis from bisabolol has been reported in Europe and is purported to occur in the United States. Patch testing with bisabolol-containing products or bisabolol may be useful in the work-up of patients with presumptive allergic contact dermatitis or potentially worsening atopic dermatitis. Patien

BACKGROUND: in Europe, methyldibromoglutaronitrile (MDGN) was banned because of excessive rates of contact allergy. However, it is unclear whether the concentrations used for testing MDGN are optimal, as different groups have used varying concentrations with quite different rates of positive reactions.
OBJECTIVES: to report patch-test results with MDGN in a localized US population, to compare these results with those of other studies, and to evaluate any association between contact allergy to MDGN and atopic dermatitis.
METHODS: a retrospective analysis of 1,753 patients tested with various

PEPPERMINT (Mentha piperita) is a popular herb that can be used in numerous forms (ie, oil, leaf, leaf extract, and leaf water). Peppermint oil has the most uses, and use data on the oil are considered relevant to the leaf extract formulations as well. This herbal preparation is used in cosmeceuticals, personal hygiene products, foods, and pharmaceutical products for both its flavoring and fragrance properties. Peppermint oil possesses a fresh sharp menthol odor and a pungent taste followed by a cooling sensation. It also has a variety of therapeutic properties and is used in aromatherapy, bat

BACKGROUND: Few US studies have reported results of patch testing with plastics and glues.
OBJECTIVE: To report our institution's results of testing patients suspected of allergy to plastics and glues with a comprehensive plastics and glues series and to compare these results with previously published data.
METHODS: Retrospective review of results of patch-testing with plastics and glues allergens at our institution between 2000 and 2007. In total, 444 patients were patch-tested with up to 56 plastics and glues allergens in the specialized series and up to five plastics and glues allergens i

Allergic contact dermatitis (ACD) from salicylates present in topical products is uncommon. Most publications about ACD from salicylates are case reports describing only a few patients. Cross-reactivity between salicylates is not commonly reported. This article describes allergic contact dermatitis from ethylhexyl salicylate used as an ultraviolet filter and fragrance compound and reviews the published literature on contact allergy to salicylates.

BACKGROUND: Allergic contact dermatitis (ACD) is a well-recognized immune-mediated disease often associated with the use of vulcanization accelerator-containing latex and nitrile gloves. Potential contact allergens in neoprene (polychloroisoprene, polychloroprene) gloves have not been reported.
OBJECTIVE: The objective was to analyze extracts of neoprene surgical and examination gloves for potential contact allergens.
METHODS: Four different brands of neoprene-type gloves were purchased, and dichloromethane extracts were derivatized and assayed by gas chromatographic mass spectrometry. A lat

Lichen-derived additives to commercial products are becoming a more frequent cause of allergic contact dermatitis. The most common lichen-derived additives are usnic acid and oak moss absolute. Both are useful for screening patients using "natural" or "botanical" products that may contain lichen-derived additives.

Ethyl chloride (EC) or chloroethane (C2H5Cl) is a volatile halogenated hydrocarbon. Reports of contact sensitivity to this gas are infrequent considering its widespread use as a local anesthetic, and it may have a relatively low sensitization potential. Benzocaine is another local anesthetic derivative of the ethyl ester of para-aminobenzoic acid, previously reported as a causative agent of delayed hypersensitivity reactions. We present a patient who developed a generalized itching dermatitis after the application of a medical aerosol containing EC, as well as facial angioedema and tongue swel

BACKGROUND: Diagnostic and predictive patch testing to determine contact allergy due to fragrance materials requires applying a fixed dose of material to the skin. This dose can be affected by the volatile nature of fragrances; little data exist on how the loss of fragrance dose due to volatility affects patch testing.
OBJECTIVE: (1) To evaluate pH dependence and evaporation rates of two fragrance chemicals, geraniol, citronellol, and a common fragrance solvent, diethyl phthalate (DEP) and (2) Assess implications for predictive patch-testing methods for fragrances.
METHOD: pH analysis of eac

A 34-year-old male experienced severe allergic contact dermatitis from 12-hydroxystearic acid in a lip balm and from hydrogenated castor oil in an underarm deodorant. He also had a positive patch-test reaction to bis-diglyceryl polyacyladipate-2, which is present in the implicated lip balm and which itself contains 12-hydroxystearic acid. He was also incidentally found to have contact allergy to ricinoleic acid and castor oil. Ricinoleic acid is the principal fatty acid in castor oil, whereas 12-hydroxystearic acid is the principal fatty acid in hydrogenated castor oil. These two fatty acids a

A rare case of contact allergic dermatitis from metal knee joint prostheses is presented. The low incidence of allergic contact dermatitis from metal prostheses is surprising, especially given the frequency of metal allergy, particularly to nickel and cobalt, in the general population. Even patients with known metal allergy often do not react to their metal prosthesis. Systemic allergic contact dermatitis is particularly uncommon with knee joint replacements where the articulation is metal on plastic rather than metal on metal as the former has decreased metal wear debris. In this particular c
