Management of the pediatric postoperative cardiac surgery patient.

Optimal management of the postoperative pediatric cardiac surgical patient requires a thorough understanding of patient anatomy, physiology, surgical repair or palliation, and clinical condition. This necessitates a dedicated team of clinicians including skilled nurses, physicians, and respiratory therapists specialized in the care of patients who have complex congenital heart disease. This article provides an overview of the multisystemic risk factors and consequences associated with cadiopulmonary bypass and cardiac surgery. An evaluation of cardiac hemodynamics and a review of major organ s

Determining the effective dose of esketamine for mitigating pain during propofol injection by Dixon's up-and-down method: a double-blind, prospective clinical study of drug dose response.

BACKGROUND: Propofol is an intravenous (IV) anesthetic medication widely used for procedural sedation, operative anesthesia, and in intensive care unit (ICU), but the incidence of pain during IV infusion can reach 28-90%. Ketamine can attenuate pain associated with IV propofol injection through local and central analgesic effects. Ketamine is gradually being transitioned to its S-enantiomer, esketamine, which has a similar mechanism of action. The purpose of our study is to determine the half effective dose (ED50), 95% effective dose (ED95), and 99% effective dose (ED99) of esketamine for atte…

Comparison of pulmonary dynamic compliance between Laryngeal Mask Airway SupremeTM and endotracheal intubation during laparoscopic cholecystectomy: study protocol for a non-inferiority, randomized, controlled trial.

BACKGROUND: During laparoscopic cholecystectomy, pneumoperitoneum reduces pulmonary compliance, which may result in atelectasis and postoperative hypoxia. The efficacy of laryngeal mask ventilation during laparoscopic surgery is currently up for dispute. The objective of this study is to determine whether a Laryngeal Mask Airway Supreme™ (LMA-S) is non-inferior to an endotracheal tube (ETT) in maintaining lung compliance among low-risk patients undergoing laparoscopic cholecystectomy. The findings could be applied to implement novel airway management strategies for laparoscopic cholecystectomy

Holding on to bypass in endovascular times? A narrative review on the role of surgical LSA revascularization in zone 2 TEVAR and how to achieve optimal results.

Thoracic endovascular aortic repair (TEVAR) has become the preferred treatment for most descending thoracic aortic pathologies. In Zone 2 procedures, intentional coverage of the left subclavian artery (LSA) is frequently required to achieve an adequate proximal seal, but this carries a risk of stroke, spinal cord ischemia, and upper extremity ischemia. Preventive LSA revascularization is therefore recommended in current guidelines, although the level of supporting evidence remains limited. This narrative review summarizes the indications, timing strategies, techniques, and outcomes of surgical

A simple needle disc puncture in the cervical spine is a safe and efficient alternative to disc/vertebral biopsy: ten years' experience in a reference centre.

BACKGROUND: Disc/vertebral infection (DVI) often requires invasive sampling for microbiological diagnosis when blood cultures are negative. CT-guided disc/vertebral biopsy is the standard diagnostic procedure but is technically challenging and associated with neurological risks in the cervical region. The technique requires sedation or general anaesthesia. These factors may cause organizational complexity, longer processing times and further delay treatment. This study assesses the diagnostic value of simple CT-guided fine-needle disc puncture (CTFNP) under local anaesthesia as an alternative

Analysis of the analgesic mechanism of TENS-WAA in colonoscopy using the EEG-fNIRS system: a study protocol for a randomised controlled trial.

INTRODUCTION: Colonoscopy is an essential procedure for the early diagnosis of colorectal conditions; however, over 60% of patients undergoing non-sedated colonoscopy report moderate to severe pain. This study aims to investigate the central analgesic mechanisms of transcutaneous electrical nerve stimulation based on wrist-ankle acupuncture theory (TENS-WAA). A multimodal approach combining electroencephalography (EEG) and functional near-infrared spectroscopy (fNIRS) will be employed to assess pain-related brain activity, with artificial intelligence applied to model the relationship between

Anaesthetic management and complications of a Humboldt penguin (Spheniscus humboldti) undergoing diagnostic imaging.

BACKGROUND: The presence of a tracheal septum dividing the trachea into two makes intubation one of the main challenges of penguin anaesthesia. Differences in the length and location of the aforementioned tracheal septum have been described in some penguin species. However, to the best of the authors' knowledge, it has not been reported in Humboldt penguins (Spheniscus humboldti). Therefore, one of the aims of this publication is to report the septal position in this Humboldt penguin. Furthermore, this publication describes the anaesthetic protocol and complications encountered and discusses s

BACKGROUND AND OBJECTIVES: Spastic equinovarus foot (SEF) is a common complication of stroke and other upper motor neuron injuries. It is characterized by a plantigrade and inverted foot, often with toe curling, causing significant disability from pain, gait, and balance difficulties. Management includes physical therapy, antispasticity drugs, orthoses, chemical neurolysis, or botulinum toxin, all of which may be insufficient, sedating, or transient. Selective tibial neurotomy (STN) provides a surgical option that is effective and long-lasting. Our goal is to provide a concise description of o

Anteroposterior pacer pad position is better than anterolateral for transcutaneous cardiac pacing.

INTRODUCTION: Transcutaneous cardiac pacing (TCP) is a lifesaving procedure for patients with certain types of unstable bradycardia. We aimed to assess the difference in the pacing thresholds between the anteroposterior (AP) and anterolateral (AL) pacer pad positions. The second aim was to characterize the severity of chest wall muscle contractions during TCP. METHODS: In this prospective crossover trial, we enrolled patients presenting to the electrophysiology laboratory for elective cardioversion. After successful cardioversion, sedated participants were sequentially paced in both positions

Augmented Reality Assisted Endoscopic Transforaminal Lumbar Interbody Fusion: 2-Dimensional Operative Video.

Augmented reality (AR) is a novel technology for spine navigation. This tracking camera-integrated head-mounted display (HMD) represents a novel stereotactic computer navigation modality that has demonstrated excellent precision and accuracy with spinal instrumentation.1 Standard computer-assisted spine navigation systems have two major shortcomings: attention shift and line-of-sight limitations. The HMD allows visualization of the surgical field and navigation data concurrently in the same field of view.2,3 However, the use of AR in spine surgery has been limited to use for instrumentation, n
