Multiple reflective channels in the spleen: a sonographic sign of portal hypertension.

We consistently observed small "reflective channels" (RCs) in the splenic parenchyma in patients with portal hypertension (PHT). The purpose of this study was to investigate the frequency of this sign in PHT and in splenic disorders unrelated to PHT compared to normal controls. The significance of this sign in the diagnosis of PHT and in differentiating PHT splenomegalies from others was also assessed. A total of 337 patients underwent sonographic examination of the spleen: 147 normal, 75 non-PHT splenic pathologies, and 115 with PHT. The RCs were scored from grade 0 to 3 by counting the numbe

Nonoperative management of major blunt renal trauma in children: in-hospital morbidity and long-term followup.

The management of 26 children with major renal injury secondary to blunt trauma was reviewed. Emergency computerized tomography (CT) was performed in all instances. Injury ranged from parenchymal laceration to vascular avulsion. Early surgical exploration was done in 5 children due to hemodynamic instability, renal pedicle injury or suspected malignancy. The remaining 21 children were observed. Of these children 5 had associated intra-abdominal organ injuries. The average length of hospitalization was 13.4 days and the average intensive care unit stay was 6.9 days. A third of the children were

The status of cardiovascular surgery in West Africa.

This report reviews the present status of cardiovascular surgery in West Africa and highlights some of the constraints of development in this field.Rheumatic heart disease is still endemic in the tropics, where it constitutes about 20 percent of all cases of cardiovascular disease (CVD) in Nigeria. Endomyocardial fibrosis is a disease of unknown etiology accounting for 10 to 20 percent of cases. Purulent pericarditis is a common complication of pyomyositis and osteomyelitis found in 5 percent of patients. Chronic constrictive pericarditis is a sequela of infective pericarditis found in 5 perce

The varied radiographic manifestations of retroperitoneal malignant fibrous histiocytoma revealed through 27 cases.

Malignant fibrous histiocytoma, a pleomorphic sarcoma, represents the most common soft tissue sarcoma of late adult life. Approximately 15 per cent of the cases of malignant fibrous histiocytoma arise within the abdominal cavity or retroperitoneum. Although tumors in this series arose in many different sites, their proximity to the kidney necessitated a nephrectomy in 44 per cent of the cases (12 of 27). In 3 cases calcification was noted, which varied from a few small speckled areas to extensive large, coarse densities. The masses tended to compress or displace adjacent structures and vessels

Para osteo arthropathy--ectopic ossification of healing tendon about the rodent ankle joint: histologic and type V collagen changes.

Ectopic bone formation, or para osteo arthropathy (POA), is commonly seen in humans following lesions to the central nervous system. The condition also occurs after severe trauma, burns, or tetanus, and, rarely, after poliomyelitis or cauda equina lesions. The lack of a suitable laboratory animal model has hampered study of its etiology and treatment. The lesion occurs just above the rodent ankle joint 7 weeks after repair of the severed achilles tendon. Tendon repair using a single silk suture resulted in ectopic calcification in 22 of 23 animals. Ectopic ossification, with extensive minerali

Mineralocorticoid receptor antagonism for non-diabetic kidney disease.

UNLABELLED: The use of mineralocorticoid receptor antagonists (MRAs) in preclinical models of non-diabetic chronic kidney disease (CKD) has … the kidney. Cardiovascular benefits, such as the prevention of cardiac fibrosis, hypoperfusion and vascular calcification, have also been observed. The translation of these preclinical findings into clinical practice has been difficult, mainly due to the lack of clinical studies testing the efficacy of steroidal MRAs in CKD patients due to their contraindication becaus …

Conical neck is strongly associated with proximal failure in standard endovascular aneurysm repair.

OBJECTIVE: Hostile proximal aortic neck (HN) challenges the suitability for standard endovascular aneurysm repair (EVAR) of patients at high risk for "open" repair. However, there has been little if any focus placed on the individual role of the "nonlength" HN features in EVAR outcomes. The aim of this study was to evaluate their individual and potentially predictive role in outcomes of EVAR under HN conditions. METHODS: Data of 156 consecutive EVAR patients with short (<15 mm) HN, treated with the Endurant device (Medtronic Cardiovascular, Santa Rosa, Calif) at three European academic vascul

Giant primary hepatic osteosarcoma: A rare case report and review of the literature.

OBJECTIVE: Primary hepatic extraskeletal osteosarcoma (PHOS) is an extremely rare and aggressive malignant tumor originating in the liver with no primary skeletal lesion. Less than 1% of all extraskeletal osteosarcomas occur in the liver, often presenting as a large abdominal mass with poor prognosis. CASE PRESENTATION: We report a rare case of a 64-year-old man diagnosed with a giant PHOS (163×128×160mm) accompanied by pulmonary and lymph node metastases. Multimodal imaging magnetic resonance imaging (MRI), computed tomography (CT), bone scintigraphy, positron emission tomography (PET)/CT de

Oral and maxillofacial clinical features of Ehlers-Danlos syndrome: a systematic review.

BACKGROUND: Ehlers-Danlos syndromes (EDS) are inherited connective tissue disorders with diverse clinical manifestations, complicating subtype classification. While joint hypermobility, skin hyperextensibility, and tissue fragility are key diagnostic features, oral and maxillofacial signs are often overlooked. OBJECTIVE: This review compiles dental and maxillofacial manifestations in EDS patients and identifies subtype-specific features to assist diagnosis. STUDY DESIGN: This systematic review followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and was regi

Anomalous vertebral arteries and recurrent ischemic stroke: Insights from a unique case (first documented case-report from Syria).

RATIONALE: The posterior circulation represented by the vertebrobasilar artery system is responsible for 20% of brain perfusion. Posterior circulation ischemia can cause a variety of symptoms due to the broad area supplied by the vertebrobasilar artery. In addition, stroke is the second leading cause of death globally, highlighting the importance of studying any potential contributing factors. PATIENT CONCERNS: A 68-year-old nonsmoker and nonalcoholic with a history of diabetes type 2 and a previous stroke, presented with 2 days of visual disturbance with a burning sensation associated with o

Reproducibility and concordance of Pedal Acceleration Time between students, residents and experts operators: Single-center prospective study.

Critical limb threatening ischemia (CLTI) is defined as peripheral artery disease (PAD) with rest pain, nonhealing wounds, or tissue loss. At this stage, this is a severe pathology with a high rate of ischemic events in the lower limbs and cardiovascular events. The ankle brachial pressure index (ABI) is the first line diagnostic method, but can be falsely high because of stiffened ankle arteries related to medial artery calcifications. The Pedal Acceleration Time (PAT) has been described as valid even in cases of diabetes or chronic renal failure. In order to evaluate this measurement in untr

Epididymo-orchitis with Epididymal Abscess in a Patient with Disseminated Tuberculosis: A Case Report.

Tuberculosis (TB) remains a global health concern, and while primarily affecting the lungs, extrapulmonary manifestations such as genitourinary tubercular epididymitis are rare and diagnostically challenging. Accurate differentiation is crucial for appropriate treatment. This case study presents a 34-year-old man with left scrotal swelling, backache, and fever, highlighting the importance of considering TB in genitourinary conditions. High-resolution sonography revealed an edematous left testis with increased vascularity and hypoechoic nodules. Further imaging, including high-resolution comput

A diminutive perivascular epithelioid cell tumor in the colon.

Perivascular epithelioid cell tumor (PEComa) is a rare mesenchymal tumor. Some papers have reported that colonoscopy could be used to treat PEComa with a predominantly pedunculated polyp, whereas surgical intervention is often required for cases with submucosal-type tumors. These findings suggest that the morphology of PEComa changes dramatically with disease progression. Because of the rapid progression of PEComa, endoscopic treatment remains challenging, and early-stage PEComa morphology is not well understood. A 64-year-old man presented to our hospital for a follow-up colonoscopy after und

Laparoscopic ischemic conditioning prior esophagectomy in selected patients: the ISCON trial.

Anastomotic leakage (AL) after esophagectomy is the most impactful complication after esophagectomy. Ischemic conditioning (ISCON) of the stomach >14 days prior to esophagectomy might reduce the incidence of AL. The current trial was conducted to prospectively investigate the safety and feasibility of laparoscopic ISCON in selected patients. This international multicenter feasibility trial included patients with esophageal cancer at high risk for AL with major calcifications of the thoracic aorta or a stenosis in the celiac trunk. Patients underwent laparoscopic ISCON by occlusion of the left

Calcific aortic valve stenosis and COVID-19: clinical management, valvular damage, and pathophysiological mechanisms.

Patients with corona virus disease (COVID)-19 are prone to a variety of myocardial and vascular complications. Recent studies suggest that cardiac valves are also potential targets for the Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2). Calcific aortic valve stenosis (CAVS) is the most common valvular heart disease. Severe COVID-19 has been associated with main risk factors for CAVS, including male sex, older age, cardiovascular co-morbidities, obesity, hypertension, diabetes, and chronic kidney disease. Prognostic implications for concomitant CAVS and SARS-CoV-2 infection have b

Total occlusion of abdominal aorta: still a deadly event in the very elderly.

Total occlusion of the abdominal aorta is an uncommon but potentially devastating event. Clinical symptoms are usually sudden and may vary depending on the level of the aortic occlusion. Associated morbidity and mortality remain substantial with high rates of limb loss, acute renal failure, rhabdomyolysis, and death. We report a case of a bedridden 91-year-old woman with history of hypertension, hyperlipidemia, anemia and dementia, carried to the emergency department because of bilateral lower limb pallor. She had been at the same department the week before because of the same problems and hig

'Solitary idiopathic choroiditis' or a tumour of scleral origin: A case report based hypothesis.

We here hypothesize the scleral origin of 'Solitary Idiopathic Choroiditis', a yellowish, nodular lesion that resembles other choroidal tumors. Systemic investigations were done to rule out causes like tuberculosis and sarcoidosis. Multimodal imaging was done for better characterization and to distinguish it from other choroidal lesions. Fundus imaging revealed a yellowish-orange, well-defined nodular lesion resembling solitary idiopathic choroiditis inferotemporal to the disc. Swept source optical coherence tomography (SSOCT) showed a nodular lesion arising from the sclera with compression of

Role of etelcalcetide in the management of secondary hyperparathyroidism in hemodialysis patients: a review on current data and place in therapy.

Secondary hyperparathyroidism (sHPT) is a frequently occurring severe complication of advanced kidney disease. Its clinical consequences include extraskeletal vascular and valvular calcifications, changes in bone metabolism resulting in renal osteodystrophy, and an increased risk of cardiovascular morbidity and mortality. Calcimimetics are a cornerstone of parathyroid hormone (PTH)-lowering therapy, as confirmed by the recently updated 2017 Kidney Disease: Improving Global Outcomes chronic kidney disease - mineral and bone disorder clinical practice guidelines. Contrary to calcitriol or other

Updating superficial siderosis of the central nervous system: bleeding of a dorsal osteophyte into the subarachnoid space from a perforating artery.

Superficial siderosis of the central nervous system (SSCNS) is an uncommon and often unrecognized disorder that results from recurrent and persistent bleeding into the subarachnoid space. Currently, there is no effective treatment for SSCNS. The identification and surgical resolution of the cause of bleeding remains the most reliable method of treatment, but the cause of bleeding is often not apparent. The identified sources of recurrent bleeding have typically included neoplasms, vascular malformations, brachial plexus or nerve root injury or avulsion, and previous head and spinal surgery. An
