Modified interventional obliteration for variceal hemorrhage from elevated jejunum after pylorus-preserving pancreatoduodenectomy.

Ectopic variceal hemorrhage caused by sinistral portal hypertension after splenic vein ligation during a pyloric-preserving pancreatoduodenectomy is a rare entity. We report the case of a 58-year-old man with symptoms of refractory melena. The varices could not be treated endoscopically and surgery was considered unsuitable due to severe adhesions and altered anatomy. Following clinical failure of partial splenic embolization, an alternative obliteration method by a retrograde trans-portal-venous approach was successfully performed and resolved the problem. It seems to be an effective method f

Dissecting aneurysm at the A1 segment of the anterior cerebral artery presenting as visual loss and visual field defect.

Intracranial dissecting aneurysms mainly occur in the territory of the vertebrobasilar system. Dissecting aneurysms confined to the anterior cerebral artery are rare, and the presentations are usually of either subarachnoid hemorrhage or cerebral infarction. Here, we report a unique case of a dissecting aneurysm of the anterior cerebral artery presenting as a visual field defect. After surgical decompression, visual symptoms recovered.

Delayed presentation and management of blunt traumatic inferior gluteal artery pseudoaneurysm with associated arteriovenous fistula.

Disruption of arterial wall integrity as a result of trauma, iatrogeny, inflammation, or infection may result in pseudoaneurysm formation. Gluteal artery aneurysms are rare and represent less than 1% of all arterial aneurysms. Pseudoaneurysms of the inferior gluteal artery (IGA) following blunt trauma are exceptionally rare with only 6 reported cases in the English literature. We describe an 82-year-old female with a remote history of a fall presenting with an enlarging buttock mass. Imaging confirmed an IGA pseudoaneurysm with associated arteriovenous fistula that was successfully treated wit

Novel endovascular treatment of enlarging facial artery pseudoaneurysm resulting from molar extraction: a case report.

PURPOSE: A novel endovascular therapy to treat a pseudoaneurysm as a complication of molar tooth extraction is described. PATIENTS AND METHODS: A 20-year-old man presented 2 weeks after third molar tooth extraction with an enlarging pulsatile jaw mass, identified as a facial artery pseudoaneurysm. Endovascular embolization with microcoils and a liquid embolic agent, Onyx, was performed. The use of Onyx in this manner is an off-label use. RESULTS: Immediately after treatment, the pulsatile mass resolved and remained obliterated at 2 months of follow-up. CONCLUSIONS: Onyx embolization of faci

An innovative exclusion technique for ductus arteriosus with a novel stent graft.

A 72-year-old woman who had a patent ductus arteriosus that was anatomically unsuitable for catheter coiling was treated with a pre-curved fenestrated stent graft. This graft was custom-made to configure the patient's whole aortic arch, and was capable of accurately adjusting its fenestrations to the arch branch orifices. The advantage of this fenestrated stent graft is close sealing, especially on the lesser curvature of the arch. This device could be an excellent option to treat an otherwise normal aortic arch with such a localized lesion.

Fatal reperfusion injury after thrombolysis for phlegmasia cerulea dolens.

Reperfusion injury after venous thrombolysis for phlegmasia cerulea dolens (PCD), despite its potentially rapid, fatal course, has not been well described in the literature. Two cases of fatal reperfusion injury after venous thrombolysis are reported, and the clinical manifestations and treatment strategies of PCD and the aspects of reperfusion injury relevant to the interventionalist are reviewed.

Portal venous gas: a clinical finding in obstructed infracardiac total anomalous pulmonary venous connection.

Total anomalous pulmonary venous connection is a cyanotic congenital heart defect that presents with a multitude of clinical presentations. We report a case of obstructive total anomalous pulmonary venous connection (TAPVC) which presented in a way that has not previously been described. An infant with mixed type obstructive TAPVC was found to have pulmonary interstitial emphysema and portal venous gas on a scout radiograph obtained prior to computed tomography scan. The presence of portal venous gas indicated a connection between the pulmonary venous system and the portal venous system infrad

Intra-arterial application of nimodipine in reversible cerebral vasoconstriction syndrome: a diagnostic tool in select cases?

INTRODUCTION: Differential diagnoses of the reversible cerebral vasoconstriction syndrome (RCVS) include all forms of intracranial stenotic disease, such as primary or secondary vasculitis of the central nervous system. Here, we tested the hypothesis that angiographic response to intra-arterial nimodipine application may be helpful in differentiating between RCVS and other entities. METHODS: A digital subtraction angiographic (DSA) series of nine consecutive patients with suspected RCVS that were treated by intra-arterial nimodipine due to clinical worsening were retrospectively analyzed. Microcoil embolisation of mycotic cystic artery pseudoaneurysm: a viable option in high-risk patients.

Mycotic cystic artery pseudoaneurysm is a rare complication of cholecystitis, of which the main treatment has been cholecystectomy plus ligation of the cystic artery. We highlight our experience with successful coil embolisation of this condition without the need for surgical intervention. This is followed by a comprehensive review of the literature regarding management of this unusual condition.

Coil embolization of an unruptured intracranial aneurysm associated with Behcet's disease: case report.

A 36-year-old male patient with Behcet's disease consulted our hospital for treatment of an unruptured intracranial aneurysm. He was neurologically intact. He had undergone surgery for an aneurysm of the superior mesenteric artery 2 years ago and had been followed up because of the aneurysm of the left radial artery for 1 year. He had been receiving steroid treatment for 4 years. Three-dimensional computed tomography angiography identified an aneurysm of the left intracranial internal carotid artery. The lesion was treated by coil embolization. He was discharged without complications. Intracra

Treatment of metastatic spinal tumors by percutaneous vertebroplasty versus percutaneous vertebroplasty combined with interstitial implantation of 125I seeds.

BACKGROUND: As the most frequent bone metastasis, spinal metastases cause severe pain and damage to vertebral bodies such as spinal osteolytic destruction and compression fractures. To avoid the trauma and complications of open surgery, a minimally invasive procedure, percutaneous vertebroplasty (PVP), has recently been developed to treat metastatic spinal tumors. PURPOSE: To analyze the treatment outcomes of metastatic spinal tumors by percutaneous vertebroplasty (PVP) alone or PVP combined with interstitial implantation of 125I seeds. MATERIAL AND METHODS: 80 patients with metastatic spina

Time management in acute vertebrobasilar occlusion.

Acute vertebrobasilar occlusion (VBO) is associated with a high risk of stroke and death. Although local thrombolysis may achieve recanalization and improve outcome, mortality is still between 35% and 75%. However, without recanalization the chance of a good outcome is extremely poor, with mortality rates of 80-90%. Early treatment is a fundamental factor, but detailed studies of the exact time management of the diagnostic and interventional workflow are still lacking. Data on 18 patients were retrospectively evaluated. Time periods between symptom onset, admission to hospital, time of diagnos

Microneurosurgical management of distal middle cerebral artery aneurysms.

BACKGROUND: Distal middle cerebral artery aneurysms originate from branches of MCA distal to its main bifurcation or the peripheral branches. Distal middle cerebral artery aneurysms are the least frequently seen among the middle cerebral artery aneurysms. The purpose of this article is to review the practical anatomy, preoperative planning, and avoidance of complications in the microsurgical dissection and clipping of MdistAs. METHODS: This review, and the whole series on intracranial aneurysms, are mainly based on the personal microneurosurgical experience of the senior author (JH) in 2 Finn

Traumatic pericallosal artery aneurysm: a rare complication of transcallosal surgery. Case report.

Traumatic intracranial aneurysms are rare in adults but account for up to 33% of all aneurysms encountered in a pediatric population. The most common location of such lesions in children is the pericallosal or adjacent branch of the anterior cerebral artery, where a head impact exerts sudden decelerating shearing forces on the arteries tethered on the brain surface against an immobile falx cerebri, weakening the arterial wall. This action can lead to dissection of the damaged vascular layers, with resultant expansion of the affected site into a fusiform aneurysm. Pericallosal aneurysms followi
