A 10-year retrospective study on palladium sensitivity.

BACKGROUND: Palladium has become an important contact allergen because of increased use in industry, jewelry, and dentistry. OBJECTIVE: To determine the frequency of palladium allergy in a US patch-test population tested to palladium. METHODS: A 10-year retrospective review (1997-2006) was performed on patients sensitive to palladium at the Department of Dermatology, Mayo Clinic, Rochester, MN. RESULTS: A total of 910 patients were tested to a series that included palladium chloride 2% in petrolatum. A palladium-positive patch-test result was noted in 110 patients (12.1%). Of the 106 patien

Questionnaire study of the prevalence of allergic contact dermatitis from cosmetics in Israel.

BACKGROUND: The prevalence of cosmetics-related contact dermatitis is rising, owing mainly to the wider use of cosmetics by the public and the routine diagnostic use of patch tests. OBJECTIVE: To determine the prevalence of cosmetics allergy in Israel. METHODS: A questionnaire was distributed to a random sample of 360 female customers of pharmacies and beauty salons in two areas of the country. Items included general health profile, family history of atopy, occurrence of rash due to patch-test-proven cosmetics allergy, anatomic sites of the rash, subjective aspects regarding the rash, and co

Late patch-test reaction to acrylates: a biochemical hypothesis.

Acrylates (acrylic and methacrylic compounds) are commonly used in dentistry as restorative materials, and their use is progressively increasing because of the concern for mercury-related health problems (allergic and nonallergic) due to their metal equivalents. However, acrylates are not completely safe from an allergologic point of view. We report a case of multiple patch-test reactions to acrylates and methacrylates that occurred at various times between 24 hours and 7 days after test application.

Eyelid dermatitis: contact allergy to 3-(dimethylamino)propylamine.

We present the case of a 42-year-old woman with intractable eyelid dermatitis. Patch testing revealed sensitization to 3-(dimethylamino)propylamine (DMAPA). DMAPA is an important etiology of allergic contact dermatitis of the eyelids and face but is easily missed even with expanded-series patch testing. We also review the most common causative allergens in eyelid dermatitis cited in the literature over the past decade. DMAPA is a reagent used in the formation of cocamidopropyl betaine (CAPB), a common additive to liquid soaps, shampoos, and other cleansing products because of its utility as a

Preservatives and skin sensitization quantitative risk assessment.

BACKGROUND: Preservatives are an unfortunately common cause of allergic contact dermatitis (ACD). Often, this is in association with exposure to cosmetics or medicaments. Recently, a quantitative risk assessment (QRA) approach to the quantitation of safe exposure levels for sensitizers has been promulgated as a more effective tool for the identification of acceptable levels of potential sensitizers in consumer products. OBJECTIVE: To assess this QRA approach, which facilitates the prediction of acceptable exposure levels to skin sensitizers in consumer products, levels that are normally below

Allergens in corticosteroid vehicles.

BACKGROUND: Whereas allergy to vehicle ingredients (ie, excipients and preservatives) in topical steroid vehicles is well recognized, there are no data regarding which vehicle ingredients are in common use or on which vehicles and active molecules are associated with which ingredients. OBJECTIVE: To produce descriptive data on the use of allergenic vehicle ingredients in prescription topical corticosteroids. METHODS: The package insert for every steroid in widespread use in the United States was obtained from the manufacturer and used to generate an ingredient list for the product. RESULTS:

Frequency and main sites of allergic contact dermatitis caused by nail varnish.

BACKGROUND: Liquid nail varnish has been used since 1919, and allergic contact dermatitis (ACD) has been recognized for at least 80 years, but it is difficult for nonspecialists to identify this condition. OBJECTIVES: (1) To verify the frequency of ACD from nail varnish in patients with a presumptive diagnosis of contact dermatitis seen at an outpatient clinic, (2) to characterize the groups studied according to site of skin disorder, and (3) to determine the main sensitizer related to varnish. METHODS: Patients with a final diagnosis of ACD caused by nail varnish were assessed by means of r

Systemic contact dermatitis.

Systemic exposure to allergens resulting in a cutaneous eruption is known as systemic contact dermatitis (SCD). Once sensitization occurs, varying exposures to antigens via multiple routes (including transepidermal routes, intravenous or intramuscular routes, inhalation, and ingestion) can result in systemic flare. This article highlights the different categories of common contactants, metals, medications, and plants, exposure to which leads to SCD. A comprehensive approach that takes into account all possible routes of exposure is essential in diagnosing SCD and in helping patients successful

Allergic contact dermatitis in children: the Ottawa hospital patch-testing clinic experience, 1996 to 2006.

BACKGROUND: Allergic contact dermatitis in children is a significant clinical problem. Patch testing is a diagnostic tool for the evaluation of patients with suspected allergic contact dermatitis. OBJECTIVES: To determine the frequency and relevance of positive patch-test results in children and to identify the most common allergens in children at our clinic. METHODS: Retrospective chart review of 100 children and adolescents, aged 4 to 18 years, who were patch-tested at the Ottawa Hospital patch-testing clinic between 1996 and 2006. The children were patch-tested with the North American Con
