Western Ontario Osteoarthritis of the Shoulder Index (WOOS) - a validation for use in proximal humerus fractures treated with arthroplasty.

BACKGROUND: The Swedish shoulder and Arthroplasty Registry (SSAR) use the Western Ontario Osteoarthritis of the Shoulder Index (WOOS) as their shoulder-specific score in the follow-up. WOOS is not yet validated for use as the Patient Reported Outcome Measurement (PROM) for proximal humerus fractures (PHF) treated with shoulder hemiarthroplasty (SHA) in the Swedish registry. The aim of this study was to examine the validity, the reliability and the responsiveness of WOOS as a PROM for proximal humerus fractures treated with shoulder arthroplasty. METHODS: Data was collected from the SSAR from

Skateboarding injuries.

Skateboarding has experienced intermittent periods of popularity since the 1960s. Along with this popularity, there have been concomitant increases in numerous types of injuries. Most documented cases occur in boys aged from 10 to 14 years, with injuries ranging from minor cuts and abrasions to multiple fractures and, in some cases, even death. Although head injuries account for approximately 3.5 to 9% of all skateboarding injuries, fractures of both upper and lower extremities account for 50% of all musculoskeletal trauma. Not surprising is the fact that 33% of those injured on skateboards ar

Management of Seymour (Distal Phalangeal) Fractures with Nail Bed Involvement.

BACKGROUND: Seymour fractures are a unique type of pediatric distal phalangeal fracture that can be easily misdiagnosed as a simple nail-bed injury or mallet finger1-3. Because of this potential for misdiagnosis, clear communication with consulting physicians regarding physical examination findings, such as nail plate avulsion and radiographic findings, is necessary. Seymour fractures involve the open physis and the germinal matrix, which become interposed in the fracture site, increasing the likelihood of infection. Open fractures are often displaced as a result of the distracting forces of t

Is Arthroscopic Technique Superior to Open Reduction Internal Fixation in the Treatment of Isolated Displaced Greater Tuberosity Fractures?

BACKGROUND: Arthroscopic double-row suture-anchor fixation and open reduction and internal fixation (ORIF) are used to treat displaced greater tuberosity fractures, but there are few data that can help guide the surgeon in choosing between these approaches. QUESTIONS/PURPOSES: We therefore asked: (1) Is there a difference in surgical time between arthroscopic double-row suture anchor fixation and ORIF for isolated displaced greater tuberosity fractures? (2) Are there differences in the postoperative ROM and functional scores between arthroscopic double-row suture anchor fixation and ORIF for

Is Parkinson Disease a Risk Factor After Total Elbow Arthroplasty? Midterm Survivorship in a Matched Cohort.

PURPOSE: Parkinson disease (PD) has been associated with elevated rates of periprosthetic fracture, periprosthetic joint infection, and reduced implant survivorship following total hip and knee arthroplasty; however, it remains unclear whether these risks extend to total elbow arthroplasty (TEA). The purpose of this study was to evaluate whether PD is associated with worse midterm survivorship and higher complication risk after TEA compared with matched controls. METHODS: A retrospective, matched cohort study was performed using the PearlDiver administrative claims database. Adult patients un

Guide-wire and K-wire readjustment jig for osteotomy and osteosynthesis: An instrument.

INTRODUCTION: Numerous orthopaedic procedures including dynamic hip screw plating and various osteotomies require placement of a reference guide pin or K wire to direct bone cuts or for drilling screw holes. Appropriate positioning of these wires is a critical component of surgery. Irrespective of whether one is a seasoned surgeon or an apprentice, these wires often need repositioning and readjustment. Parallel wire positioning is needed even during procedures like osteotomies or patellar fracture fixation. Though there are several tools available to help with accurate placement of wires and s

Treatment outcome of tardy ulnar nerve palsy associated with traumatic cubitus valgus by supracondylar shortening wedge rotary osteotomy and ulnar nerve in situ tension release.

BACKGROUND: Tardy ulnar nerve palsy is a common late complication of traumatic cubitus valgus. At present, the treatment of tardy ulnar nerve palsy associated with traumatic cubitus valgus is still controversial, whether these two problems can be corrected safely and effectively in one operation is still unclear. To investigate the supracondylar shortening wedge rotary osteotomy combined with in situ tension release of the ulnar nerve in the treatment of tardy ulnar nerve palsy associated with traumatic cubitus valgus. METHODS: Between 2012 and 2019, 16 patients who had traumatic cubitus valg

Relationship of cubitus varus and ulnar varus deformity in supracondylar humeral fractures according to the age at injury.

BACKGROUND: We investigated the relationships of ulnar varus deformity and the degree of cubitus varus according to the age at injury in patients with cubitus varus. METHODS: Basic demographic factors were investigated in group 1 (injured at age younger than 5 years) and group 2 (injured at age 5-10 years). The uninjured side ulnar angle (UA) at the time of injury and final follow-up and the humeral-elbow-wrist angle (HEW-A) at final follow-up were measured. ΔUA (injured side - uninjured side UA at final follow-up) and ΔHEW-A (injured side - uninjured side HEW-A at final follow-up) were calcu

Magnetic Resonance Arthrography of the Wrist and Elbow.

Magnetic resonance (MR) arthrography of the wrist and elbow is useful for detecting a variety of intra-articular pathologies. MR dictations should address whether intrinsic ligament tears of the wrist are partial-thickness or full-thickness, and involve the dorsal, membranous, and/or volar components of the ligaments. With regard to elbow soft tissue pathology, partial-thickness tears of the anterior band of the ulnar collateral ligament in overhead-throwing athletes are well evaluated with MR arthrography. MR arthrography also is helpful in staging osteochondritis dissecans of the capitellum,

Medial approach for corrective osteotomy of cubitus varus: a cosmetic incision.

Osteotomies described previously to correct cubitus varus had been associated with unsatisfactory results such as a prominent lateral scar and condyle and injury to the triceps. The authors evaluated the results of a medial approach for the corrective osteotomy of 14 consecutive children (mean age 8.4 years) with cubitus varus. The incision was made medially and the ulnar nerve was identified and protected. An image intensifier was used to identify the wedge. Lateral closing wedge osteotomy was performed and secured with cross K-wires, and the distal fragment was translated medially to reduce

Peripheral nerve repairs and their results in children.

It seems that lesions of nerves in children account for 10% to 15% of the total cases seen in this, as in other, specialist units. The prognosis for recovery after repair is defined by the severity of the original injury. The authors' worst results were seen in complete lesions of the supraclavicular brachial plexus and in the untidy wounds of war, in which penetrating missiles destroyed the proximal femur and the adjacent nerves. The outcome for injuries to the lumbosacral plexus, from fractures of the pelvis or from penetrating missile injury, are as bad. The most significant variable in det

Comminuted fractures of the radial head. The role of silicone-implant replacement arthroplasty.

Radial head resection is the accepted treatment of comminuted radial-head fractures in adults, but the results are not always satisfactory. This study examines the results of silicone replacement arthroplasty of the radial head in acute fractures and as a salvage procedure after failed resections of the radial head. The first group of patients was evaluated both before and after silicone replacement arthroplasty had been done as a salvage procedure following a previous radial head fracture. This group comprised twelve patients with an average follow-up of 3.8 years. Improvement was found in a

Distal humeral fractures: impact of lateral approach and fracture-specific plating on radial nerve palsies.

We retrospectively reviewed supracondylar distal humeral fractures that had been treated with fracture-specific plating and definitive fixation through a lateral approach with a medial triceps mobilization technique. We determined the incidences of preoperative and postoperative radial nerve palsies (RNP) to evaluate the impact of the plating and fixation technique on RNP. Seventy-three patients treated at our institution from 2006 through 2009 were included in the study. The patients were assigned to 2 groups: 57 to the blunt injury group (motor vehicle collisions, falls, struck pedestrians,

Plate vs. intramedullary screw fixation of chevron olecranon osteotomies: a biomechanical study.

BACKGROUND: Intra-articular distal humerus fractures frequently require olecranon osteotomies for adequate exposure, but fixation of olecranon osteotomies is associated with high rates of hardware-related complications requiring subsequent reoperation for removal. Intramedullary screw fixation is an attractive option to attempt to minimize hardware prominence. The purpose of this biomechanical study is to directly compare intramedullary screw fixation (IMSF) with plate fixation (PF) of chevron olecranon osteotomies. It was hypothesized that PF would be biomechanically superior to IMSF. METHOD

The long-term results of shoulder hemiarthroplasty in irreducible four-part fracture-dislocation of the proximal humerus without rotator cuff tear arthropathy.

PURPOSE: Although there are concerns about nonunion of the greater tuberosity or shoulder weakness, hemishoulder arthroplasty (HSA) is a viable treatment option for irreducible four-part proximal humerus fractures-dislocation without rotator cuff tear arthropathy. This study aims to analyze the long-term radiological and functional outcomes of HSA. METHODS: This study enrolled 36 patients who underwent HSA due to irreducible four-part proximal humerus fracture-dislocation without rotator cuff tear arthropathy between March 2005 and May 2020. The exclusion group included 10 reverse total shoul
