Reconstructive operations for the upper limb after brachial plexus palsy.

Limited function due to paralysis following brachial plexus lesions can be improved by secondary operations of the bony and soft tissue. Between April 1994 and December 2000, 109 patients suffering from arm-plexus lesions underwent a total of 144 reconstructive operations guided by our concept of integrated therapy. The average age at the time of surgery was 32 years (range: 15-59). The following operations were performed: shoulder arthrodesis (23), trapezius transfer (74), rotation osteotomy of humerus (9), triceps to biceps transposition (9), transposition of forearm flexors or extensors (8)

Semi-rigid vs rigid glass fibre casting: a biomechanical assessment.

OBJECTIVES: To determine if semi-rigid synthetic casts provide any measurable advantages compared to rigid synthetic casts. BACKGROUND: Glass fibre bandages are now commonly applied immediately post-injury to provide rigid immobilisation of the limb, for both weight bearing and non-weight bearing casts. However, composite casts that have inherent flexibility are also available and it is claimed they provide some functionality. METHODS: Five members of the orthopaedic department each applied a rigid and a semi-rigid below elbow (Colles) and a below knee walking cast to a single volunteer subj

TouchWGNN: spatio-temporal tactile perception for multimodal dexterous manipulation.

Dexterous in-hand manipulation requires robotic hands to estimate object-state reliably under frequent occlusions, contact-rich interactions, and fast dynamics. Tactile sensing provides high-frequency, contact-specific feedback, although extracting useful representations from raw tactile signals and integrating them with vision and proprioception remains challenging. In this article, we present TouchWGNN, a multimodal dexterous manipulation framework that explicitly models tactile signals as a spatio-temporal graph. We first develop a low-cost distributed tactile sensor array for a five-finger

Recovery of Grip Strength Over Time After Complete Resection of KRAS-Positive Lung Cancer With Hypertrophic Pulmonary Osteoarthropathy: A Case Report.

Hypertrophic pulmonary osteoarthropathy is a rare paraneoplastic syndrome affecting < 1% of patients with non-small cell lung cancer, characterized by clubbed fingers, periosteal proliferation, and arthritis. Although symptoms improve after treatment, objective functional recovery has not previously been reported. We present a 52-year-old male heavy smoker with right upper lobe adenocarcinoma and hypertrophic pulmonary osteoarthropathy causing profound grip strength impairment (5/2 right/left kilogram-force (kgf)). Genetic testing identified the KRAS G12C mutation. The patient underwent right

Evaluating corticokinematic coherence using electroencephalography and human pose estimation.

Objective. While peripheral mechanisms of proprioception are well understood, the cortical processing of its feedback during dynamic and complex movements remains less clear. Corticokinematic coherence (CKC), which quantifies the coupling between limb movements and sensorimotor cortex activity, offers a way to investigate this cortical processing. However, ecologically valid CKC assessment poses technical challenges. Thus, by integrating Electroencephalography (EEG) with Human Pose Estimation (HPE), this study validates the feasibility and validity of a novel methodology for measuring CKC durin

Hofmeister Effect-Driven Mussel-Inspired Hydrogel with Tunable Mechanics for Flexible Sensors.

Inspired by the adhesive proteins of mussels, we developed a dual-network hydrogel with robust adhesion and tunable mechanical properties for wearable sensors. A catechol-functionalized polymer (PAA-g-Dopa) was synthesized by grafting L-DOPA onto a poly(acrylic acid) backbone, introducing catechol groups capable of dynamic interfacial bonding. This adhesive component was copolymerized with acrylamide (AM) and poly(vinyl alcohol) (PVA) to fabricate a tough, stretchable, and highly adhesive PVA-DOPA hydrogel. The PVA-DOPA hydrogel adhered strongly to a wide variety of substrates, regardless of t

A Case of Penttinen Syndrome With Radiographic Acroosteolysis From Age 3 Years.

Premature aging syndrome, Penttinen type (Penttinen syndrome) is a progeroid syndrome with facial alterations (thin hair and progressive recession of the maxillozygomatic bones with pseudoprognathism), skin abnormalities (scleroderma with epidermal and dermal atrophy, lipoatrophy, chronic ulcers, and keloid-like hypertrophic lesions), corneal changes (vascularization and opacity), cerebral vascular anomalies, and acroosteolysis. This syndrome is caused by heterozygous, gain-of-function pathogenic variants in the PDGFRB gene. Only 10 affected individuals have been reported to date, and thus the

Highly Stretchable, Transparent, Self-Healing Ion-Conducting Elastomers for Long-Term Reliable Human Motion Detection.

The flexible electronic sensor is a critical component of wearable devices, generally requiring high stretchability, excellent transmittance, conductivity, self-healing capability, and strong adhesion. However, designing ion-conducting elastomers meeting all these requirements simultaneously remains a challenge. In this study, a novel approach is presented to fabricate highly stretchable, transparent, and self-healing ion-conducting elastomers, which are synthesized via photo-polymerization of two polymerizable deep eutectic solvents (PDESs) monomers, i.e., methacrylic acid (MAA)/choline chlor

Expanding the Phenotypic Spectrum of APMR4 Syndrome Caused by a Novel Variant in LSS Gene and Review of Literature.

Alopecia intellectual disability syndromes 4 (APMR4) is a very rare autosomal recessive condition caused by a mutation in the LSS gene present on chromosome 21. This syndrome has a clinical heterogeneity mainly exhibited with variable degrees of intellectual disability (ID) and congenital alopecia, as well. Eight families with 13 cases have been previously reported. Herein, we provide a report on an Egyptian family with two affected siblings and one affected fetus who was diagnosed prenatally. Whole-exome sequencing (WES) revealed a novel pathogenic missense variant (c.1609G > T; p.Val537Leu)

Secondary procedures for restoration of upper limb function in late cases of neonatal brachial plexus palsy.

Neonatal brachial plexus palsy is a devastating complication after a difficult delivery. The incidence of this injury has not significantly decreased over the past decades, despite all the advances in perinatal care. Although primary repair of the nerves with microsurgical techniques is the common treatment strategy nowadays, there are late cases in which secondary procedures in tendons or bones are necessary. Moreover, secondary procedures may be needed to improve the results of primary repair. A careful preoperative assessment of all the residual defects and deformities in upper limbs of the

A stimulation-driven exoskeleton for walking after paraplegia.

An untethered version of a stimulation-driven exoskeleton was evaluated for its ability to restore walking after paralysis from spinal cord injury. The hybrid neuroprosthesis (HNP) combined a passive variable-constraint exoskeleton for stability and support with functional neuromuscular stimulation (FNS) to contract the paralyzed muscles to drive limb movement. This self-contained HNP was operated by an onboard controller that sampled sensor signals, generated appropriate commands to both the exoskeletal constraints and integrated stimulator, and transmitted data wirelessly via Bluetooth to an

Pulling Harder than the Hamate Tolerates: Evaluation of Hamate Injuries in Rock Climbing and Bouldering.

OBJECTIVE: Hamate hook fractures are rare injuries, comprising 2% to 4% of all carpal fractures. Climbing athletes seem to be affected more frequently than others, as they strain the passive and active anatomical structures of their hands and fingers to maximum capacity during training or competing. This stress is transmitted to the hook of the hamate by tightened flexor tendons, which creates high contact pressure to the ulnar margin of the carpal tunnel. Injuries of the hamate hook, caused by contact pressure of the anatomical structures, are rare and occur nearly exclusively during climbing

Open proximal phalangeal shaft fractures of the hand treated by theta fixation.

BACKGROUND: Many implants and techniques are used for the treatment of open phalangeal fractures with varying grades of stability. The ubiquitous and simple Kirschner (K) wiring does not provide adequate stability to allow early mobilization of fingers. Lister described a combination of coronal interosseous wire and oblique K-wire technique for phalangeal fracture fixation with a stable construct that allowed early mobilization. Due to the fancied resemblance of this construct to the Greek alphabet θ (theta), we have referred to this as the theta fixation. MATERIALS AND METHODS: Ten patients

A multi-pad electrode based functional electrical stimulation system for restoration of grasp.

BACKGROUND: Functional electrical stimulation (FES) applied via transcutaneous electrodes is a common rehabilitation technique for assisting grasp in patients with central nervous system lesions. To improve the stimulation effectiveness of conventional FES, we introduce multi-pad electrodes and a new stimulation paradigm. METHODS: The new FES system comprises an electrode composed of small pads that can be activated individually. This electrode allows the targeting of motoneurons that activate synergistic muscles and produce a functional movement. The new stimulation paradigm allows asynchron

Medial plantar venous flap technique for volar oblique amputation with no defects in the nail matrix and nail bed.

BACKGROUND: Skin grafting is a simple technique used during volar oblique amputation; however, it is not appropriate to use this technique if the bone or tendon is exposed. Moreover, in volar oblique amputation, if the severed section is large and elongated, skin grafting makes the lack of volume conspicuous, and reconstruction with a V-Y advancement flap occasionally results in a nail deformity that resembles a parrot's beak. We used a medial plantar venous flap for the correction of large volar oblique amputation. METHODS: Reconstructive surgery was performed on patients with volar oblique
