The Unreamed Femoral Nail (UFN) has been used in Belgium since April 1995. Compared with the Reamed Femoral Nail (RFN) the new device had some advantages: it can also be used for subtrochanteric fractures and ipsilateral fractures of the femoral neck, there is less iatrogenic damage to the vascularization of the bone, the risk of fat embolism syndrome and ARDS is reduced, the operation is a less time consuming procedure, associated with less blood loss. During a 9 months period, 36 patients have been treated with the UFN: 10 patients with metastatic disease, 12 patients with a subtrochanteric

Nailing technique has changed in recent years in some important aspects which are not limited to the omitted reaming procedure. These changes concern patient positioning, reduction technique, the use of temporary stabilizers such as the 'Pinless', and determination of implant length and diameter. Approach and exposure techniques have been modified to new, less invasive procedures, in order to fulfill technical, functional and aesthetic requirements. Techniques and tricks have been developed for avoidance of fragment diastasis and axial and torsional malalignment. Finally, simple algorithms are

BACKGROUND: For complex subtrochanteric femoral fractures with lateral wall involvement, PFNA augmented with a lateral plate is commonly used; however, conventional reconstruction plates have limitations including poor anatomical conformity and insufficient anti-rotational capability. Whether a novel double-column bridging plate combined with PFNA can provide superior biomechanical performance under both normal and osteoporotic bone conditions requires validation.
METHODS: CT data of the left femur from one healthy adult volunteer were collected. Mimics 19.0, Geomagic Wrap 2017, and Creo 6.0

INTRODUCTION: In surgeries for unstable AO/OTA 31A3.3 fractures, surgeons use various lengths of intramedullary nails (IMNs). However, there is insufficient evidence regarding the appropriate nail length for these fractures. This study compared the biomechanical properties of IMNs of different lengths for AO/OTA 31A3.3 fractures.
METHODS: 30 synthetic femora of AO/OTA 31A3.3 fracture model were randomly assigned to the following three groups: short- (170 mm), mid- (235 mm), and long-length (300 mm) nail groups, and were performed fixation surgery. The translation patterns of the constructs we

INTRODUCTION: Elastic stable intramedullary nailing (ESIN) is currently the technique of choice for pediatric femoral fractures. Submuscular plating (SMP) allows reliable healing associated with an early range of motion. The following systematic review and meta-analysis was carried out to reveal the functional and surgical outcomes of SMP and ESIN for fixation of pediatric femoral fractures and to aid in the decision-making processes for those who perform these procedures.
METHODS: An extensive systematic literature review was implemented from inception to 23 February 2022. All clinical studi

INTRODUCTION: Intertrochanteric hip fractures are a common injury treated by orthopedic surgeons and the incidence rate is rising. Preoperative depression is a known risk factor for postoperative complications in orthopaedic surgery, however its effects on outcomes after geriatric hip fractures is relatively unknown. The purpose of this study was to investigate the relationship between preoperative depression and potential complications following open reduction internal fixation (ORIF) and intramedullary nailing (IMN) of geriatric hip fractures.
METHODS: In this retrospective study, the Truve

Introduction Nonunion, defined as the absence of progressive healing for more than six months after fracture, remains a major challenge in orthopedic practice, leading to chronic pain, impaired mobility, and reduced quality of life. Platelet-rich plasma (PRP), an autologous concentrate of platelets and growth factors, has been proposed as a biological adjunct for enhancing tissue regeneration. Although PRP has been demonstrated as beneficial for acute fracture healing, its efficacy in established nonunion is unclear. Methods We established a rat tibial nonunion model by transverse osteotomy wi

INTRODUCTION: Intertrochanteric (IT) hip fractures are increasingly common among older adults and are associated with significant morbidity and mortality. Cephalomedullary nailing is widely adopted for the treatment of intertrochanteric (IT) fractures; however, implant cut-out or cut-through (COCT) continues to be a significant complication. Polymethylmethacrylate (PMMA) augmentation has been introduced to mitigate this risk, though its use is associated with potential drawbacks including thermal necrosis and cement leakage into the joint or fracture site. Therefore, the aim of this study was

BACKGROUND: Elastic stable intramedullary nailing (ESIN) is an established treatment for pediatric radius fractures. However, the optimal surgical entry point remains debated due to differing complication profiles. The radial approach carries a risk of superficial branch of the radial nerve (SBRN) irritation, whereas dorsal approaches have been associated with extensor tendon injuries. This study evaluates the neurological complication profile of the radial approach and provides clinically relevant risk factors.
METHODS: A retrospective single-center study was performed including 285 pediatri

PURPOSE: Rotational malalignment and leg length discrepancy after intramedullary nailing of femoral shaft are frequent. This study has three objectives: evaluate the rate of femoral rotational malalignment and leg length discrepancy using EOS imaging after antegrade intramedullary nailing of femoral shaft fracture, find a relevant clinical examination to detect malrotation and identified risk factors.
METHODS: We performed a retrospective single-centre study between January 2014 and January 2022. Fifty-eight patients were clinically and radiographically assessed at a minimum of three months.

BACKGROUND: Proximal femoral fractures are a major public health problem because of the increasing proportion of elderly individuals in the general population. The mode of choice for anaesthesia in surgical treatment of these fractures is still debated in terms of better postoperative outcome. The aim of our study was to compare the effect of general over spinal anaesthesia on mortality in proximal femoral fracture surgery.
PATIENTS AND METHODS: This study was a retrospective analysis of 115 patients aged at least 70 years who underwent surgery for proximal femoral fracture. The survey was co

BACKGROUND: The femoral shaft fractures with large fragments makes anatomical reduction challenging and often results in non-union. In some studies, the degree of fragment displacement was reported to have affected non-union, but the association between the one fragment size and degree of displacement has not been fully clarified. Therefore we performed a retrospective study to assess: (1) the more influential factor of non-union: the degree of fragment displacement, or the fragment size? (2) the non-union rates according to different sizes and degrees of displacement.
HYPOTHESIS: The degree

We investigate the feasibility of arthroscopic fixation of comminuted glenoid fractures using cannulated screws and suture anchors. We retrospectively review 11 cases of closed comminuted glenoid fractures treated at our institution from August 2010 to May 2013. The 11 patients, 8 males and 3 females, had a mean age of 41 years (range: 27-55 years). The mechanisms of injury were traffic accidents in 9 cases and falls from height in 2 cases. The mean time from injury to surgery was 12 days (range: 3-28 days). All glenoid fractures were confirmed on x-ray and computed tomography. The major fractu

A treatment of fractures of distal metadiaphysis of the humeral bone remains an actual problem of modern traumatology at present time. This is associated with immediate proximity of the radial nerve and risk of iatrogenic injury in external fixation, presence of short distal fragment, comminuted nature of fracture, complexity of treatment method selection, need of extensible approach. Biomechanical features of different fixators were analyzed in consideration of presence of short distal fragment, traumatic of. external fixation and risk of iatrogenic injury of the nerve. The authors suggested

The treatment of basicervical femoral fractures remains controversial. The aim of this study was to examine the efficacy of intramedullary nail use in the surgical treatment of basicervical fractures. In total, 28 patients with basicervical fractures treated with proximal femoral nails were examined retrospectively. Fracture healing was observed in all patients, who were followed at least for 6 months. While the average radiological fracture healing timing was ~10.5 (8-14) weeks, clinical fracture healing occurred in 6 (5-9) weeks on average. Screw cut‑out, avascular necrosis, femur fracture,
