When Silence Masks Autism: Delayed Autism Diagnosis in an Adolescent With Deafness.

A 13-year-old adolescent male was referred for an interdisciplinary autism assessment. His complex medical history included a traumatic delivery (with drop in heartrate and nuchal cord), neonatal hyperbilirubinemia, diagnosis of profound congenital hearing loss, diminished bile ducts on liver biopsy at 1 month of age, and a seizure disorder. Later, he was diagnosed with a significant global developmental delay with minimal communication in any modality. The youth received cochlear implants at the age of 2 years but was unable to tolerate them. Despite early intervention services and multiple s

The Reliability and Validity of Data Obtained With the Functional Gait Assessment Modified for Children.

PURPOSE: This study investigated the Functional Gait Assessment modified for Children (FGA-mC) in children with cochlear implant and with and without vestibular loss (VL), and the FGA-mC's reliability and validity. METHODS: Fifty-nine children completed the FGA-mC and vestibular and functional testing. Twenty children were video-recorded to assess the FGA-mC's inter- and intra-rater reliability, and 10 repeated the FGA-mC for test-retest reliability. Ten physical therapists reviewed the videos, scored the child's FGA-mC performance, and provided test administration feedback. RESULTS: Childre

Congenital deafness: from screening to management.

Congenital deafness represents a major global health challenge, impacting both communication skills and social integration. The results of epidemiological observations show variable prevalence rates, influenced by genetic and environmental factors. The pathophysiological processes involve abnormalities of the auditory system resulting from genetic mutations, prenatal infections, and exposure to ototoxic substances. Diagnostic approaches are multidisciplinary, combining audiological, genetic, and imaging assessments. Management options include the use of hearing aids, cochlear implants, therapi

Assessment and Management of Chemotherapy-Induced Ototoxicity in Children.

Chemotherapy-induced ototoxicity is a significant concern in pediatric patients with cancer, particularly those treated with platinum-based agents, such as cisplatin and carboplatin. This study reviewed its prevalence, risk factors, early diagnosis, and management strategies. A literature review was conducted to assess the effects of ototoxic chemotherapy, screening methods, and treatment approaches. Various grading scales and rehabilitation strategies were analyzed. Platinum-based chemotherapy causes ototoxic hearing loss in approximately 100% of cases, including high-frequency and delayed-on

Paediatric Cogan Syndrome masquerading as IgA vasculitis.

Paediatric Cogan Syndrome is a rare and underrecognised autoimmune vasculitis characterised by ocular inflammation and sensorineural hearing loss. Its etiopathogenesis, diagnosis, and management are not well defined. We report a 12-year-old girl who initially presented with symptoms of IgA vasculitis formerly called Henoch Schoenlein Purpura (HSP) and eventually developed anterior uveitis and bilateral sensorineural hearing loss leading to the diagnosis of atypical Cogan Syndrome. The workup for infectious etiologies and other systemic rheumatologic disorders was negative. The management was m

Shared decision-making for vaccines.

Shared decision-making is the process by which health care providers and patients work collaboratively to evaluate and select treatment options. In June 2019, this approach was included in the Advisory Committee on Immunization Practices' (ACIP's) updated recommendations for the 9-valent human papillomavirus (9vHPV) and 13-valent pneumococcal conjugate (PCV13) vaccines. Using shared decision-making, 9vHPV may be administered to individuals aged 27 years through 45 years who are not adequately vaccinated, and PCV13 may be administered to adults aged 65 years or older who do not have an immunoco

The Importance of Early Genetic Diagnostics of Hearing Loss in Children.

Hearing loss is one of the most common sensory deficits. It carries severe medical and social consequences, and therefore, universal newborn hearing screening was introduced at the beginning of this century. Affected patients can have hearing loss as a solitary deficit (non-syndromic hearing loss) or have other organs affected as well (syndromic hearing loss). In around 60% of cases, congenital hearing loss has a genetic etiology, where disease-causing variants can change any component of the hearing pathway. Genetic testing is usually performed by sequencing. Sanger sequencing enables analysi

The Promise-and Challenge-of Statistical Learning for Elucidating Atypical Language Development.

Statistical learning plays an important role in the acquisition of spoken and written language. It has been proposed that impaired or atypical statistical learning may be linked with language difficulties in developmental disabilities. However, research on statistical learning in individuals with developmental disabilities such as autism spectrum disorder, dyslexia, and specific language impairment, and in individuals with cochlear implants, has produced divergent findings. It is unclear whether, and to what extent, statistical learning is impaired or atypical in each of these developmental di

Current opinion: the management of tinnitus.

PURPOSE OF REVIEW: The purpose of this review is to describe our experience with management of chronic tinnitus and to review the recent literature on the best treatment options available for treating patients who are troubled by their tinnitus. In addition, we want to highlight our experience and approach to this very common problem. RECENT FINDINGS: Treatment options for patients are based on the severity of the tinnitus and any associated problems. The use of nutritional supplements has a place in the treatment of mild-to-moderate tinnitus. Ginkgo biloba and B-complex vitamins may have an

De novo mutation in X-linked hearing loss-associated POU3F4 in a sporadic case of congenital hearing loss.

OBJECTIVES: In this report, we present a male patient with no family history of hearing loss, in whom we identified a novel de novo mutation in the POU3F4 gene. METHODS: One hundred ninety-four (194) Japanese subjects from unrelated and nonconsanguineous families were enrolled in this study. We used targeted genomic enrichment and massively parallel sequencing of all known nonsyndromic hearing loss genes for identifying the genetic causes of hearing loss. RESULTS: A novel de novo frameshift mutation of POU3F4 to c.727_728insA (p.N244KfsX26) was identified. The patient was a 7-year-old male w

Treatment of neurological and psychiatric disorders with deep brain stimulation; raising hopes and future challenges.

The technology of Neural Stimulation in recent years has become the focus of the research and treatment, although it has been around for many years. The potential use of stimulating the brain and nerves ranges from the spinal cord stimulation to the implantations of cochlear and bionic eyes with a large discrepancy between the clinical readiness for these various uses. Electrical high-frequency Deep Brain Stimulation (DBS) was developed as an alternative option to treat a few neurological disorders. However, with advancing in surgical procedures, technologies and safeties, the applications of

The role of biofilms in otolaryngologic infections: update 2007.

PURPOSE OF REVIEW: Biofilms have been shown to play a role in otitis media, sinusitis, cholesteatoma, tonsillitis, adenoiditis, and device infections. This article is written to review recent advances in the field. RECENT FINDINGS: The role of biofilms in the persistence of chronic, mucosal-based ENT-related infections was first recognized in otitis media. Definitive proof was lacking until the demonstration of bacterial biofilms on the middle-ear mucosa of children, not only with chronic otitis media with effusion, but also with recurrent otitis media. Strains of Pseudomonas aeruginosa isola

Neuroplasticity in the auditory system.

An increasing interest on neuroplasticity and nerve regeneration within the auditory receptor and pathway has developed in recent years. The receptor and the auditory pathway are controlled by highly complex circuits that appear during embryonic development. During this early maturation process of the auditory sensory elements, we observe the development of two types of nerve fibers: permanent fibers that will remain to reach full-term maturity and other transient fibers that will ultimately disappear. Both stable and transitory fibers however, as well as developing sensory cells, express, and

Large vestibular aqueduct syndrome: a case study.

A 23-month-old female was referred for hearing aid fitting after failing newborn hearing screening and being diagnosed with significant hearing loss through subsequent diagnostic testing. Auditory brainstem response (ABR) and behavioral testing revealed a moderate-to-severe bilateral mixed hearing loss. Prior to the hearing aid evaluation, tympanostomy tubes had been placed bilaterally with little or no apparent change in hearing sensitivity. Initial testing during the hearing aid fitting confirmed earlier findings, but abnormal middle ear results were observed, requiring referral for addition

Suprathreshold measures of auditory function in the aging chinchilla.

The purpose of this study was to examine the relationship between aging and suprathreshold auditory function in the chinchilla, a rodent with a relatively long life span and a range of hearing similar to that of humans. Subjects were six 11-12-year-old chinchillas and six young controls. Tests of auditory function utilized evoked potential (EVP) recordings from electrodes implanted in the inferior colliculus and measurements of 2f1-f2 distortion product otoacoustic emissions (DPOAEs). Tests of frequency resolution (EVP tuning curves for a 2 kHz probe tone), temporal resolution (EVP forward mas

Convolutional neural network metal artifact reduction in ultra-high resolution photon counting detector CT of middle- and inner-ear implants.

BACKGROUND: Metallic inner- and middle-ear implants with submillimeter structures requiring ultra-high resolution (UHR) imaging invariably result in metal artifacts at CT, which can obscure adjacent microstructures and subtle pathologic findings. Current metal artifact reduction techniques can be less effective for small metal objects or metal in heterogeneous background anatomy, and can sometimes introduce new artifacts or noise. PURPOSE: To develop and evaluate an image-domain convolutional neural network to reduce metal artifacts near inner- and middle-ear implants in UHR photon-counting-d
