Complications of olecranon osteotomy in the treatment of distal humerus fracture.

anaDistal humerus fractures (DHFs) are challenging to treat due to the locally complex osseous and soft tissue anatomy. Adequate exposure of the articular surface of the distal humerus is crucial when performing an anatomical reconstruction of the elbow. Even though "triceps-on" approaches are gaining popularity, one of the most commonly used surgical treatments for DHF is olecranon osteotomy. The incidence of complications related to this approach is unclear. This review was performed to assess the type and frequency of complications that occur with the olecranon osteotomy approach in the tre

Humerus Lengthening With the PRECICE Internal Lengthening Nail.

BACKGROUND: Deformity and growth arrest of the humerus in children may result as sequelae of proximal humerus fractures and unicameral bone cysts, or as complications of their treatment. As approximately 80% of the growth of the humerus arises from the proximal physis, the resultant upper limb-length discrepancy can be substantial. Benefits to lengthening the shortened arm have been previously demonstrated with the use of external fixation devices. To our knowledge, no reports have been published on the use of intramedullary implants for this purpose. METHODS: A 15-year-old girl with humeral

Cable versus cerclage wire in weber tension band fixation for transverse olecranon fractures: a biomechanical study in a synthetic ulna model.

Stable fixation of displaced simple transverse olecranon fractures is required to maintain reduction under triceps traction and enable early elbow motion. Although Weber tension band fixation is widely used, the biomechanical influence of tension band material (cable vs cerclage wire) across different pin/anchorage designs remains unclear. This study compared resistance to fracture-line separation among six Weber-based constructs in a standardized synthetic ulna model. Sixty commercially available synthetic ulnae were osteotomized to create a transverse olecranon fracture model (Mayo type IIA)

Biomechanical comparison of 2.4-mm locking compression plating radial head plate and tripod fixation in Mason type III radial head fractures: a human cadaveric study.

BACKGROUND: Proximal radius fractures, particularly Mason type III, represent a challenge in elbow surgery because of their complexity and impact on joint function. Using the conventional radial neck plate is a reliable treatment option; however, it can cause soft tissue irritation and restricted range of motion. A novel tripod fixation approach, using 3 crossed screws, should minimize such complications and preserve range of motion. This study aims to compare the biomechanical competence of conventional radial neck plating using an locking compression plate Radial Head Neck Plate 2.4 mm (John

Allograft-Prosthetic Composite Reconstruction for Proximal Humerus Bone Loss: A Systematic Review and Meta-Analysis of Outcomes and Complications.

BACKGROUND: In smaller studies, allograft-prosthetic composite (APC) has been used for proximal humerus bone loss with some success, although with notable complication risk. This systematic review and meta-analysis sought to describe outcomes and complications after proximal humerus APC and how major APC complications are defined in the literature. METHODS: A systematic review was performed per Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. PubMed/MEDLINE, EMBASE, Web of Science, and Cochrane were queried for articles on APC for proximal humeral bone loss secon

[Scaphoid percutaneous osteosynthesis by screw using computer assisted surgery: an experimental study].

Scaphoid fractures are sometimes difficult to diagnose and even more difficult to fix. Recent progress such as miniaturization of osteosynthesis material, adoption of the percutaneous route, and widening of the indications to include undisplaced fractures has still not abolished complications. In this context, computer assisted surgery (CAS) may be useful and deserves further study. To apply it to the scaphoid, it is initially necessary to immobilize the "wrist hand fingers" unit in a device adapted to make it a rigid unit. It is then necessary to choose the correct configuration of CAS system

Operative vs Nonoperative Management of Fractures of the Humeral Diaphysis: The Humeral Shaft Fracture Fixation Randomized Clinical Trial.

IMPORTANCE: Humeral shaft fractures are routinely managed nonoperatively, but this approach is potentially associated with higher nonunion rates and inferior functional outcomes when compared with operative fixation. OBJECTIVE: To assess whether there is any difference in outcome between surgery and functional bracing for adults with an isolated, closed humeral shaft fracture. DESIGN, SETTING, AND PARTICIPANTS: This prospective, superiority, parallel-group randomized clinical trial was conducted between September 2018 and October 2023 and took place at an academic major trauma center in the

Clinical outcomes of reverse total shoulder arthroplasty for elective indications versus acute 3- and 4-part proximal humeral fractures: a systematic review and meta-analysis.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) has continued to increase in clinical utility and popularity as an effective treatment for cuff tear arthropathy (CTA), irreparable rotator cuff tears (RCTs), osteoarthritis, and acute 3- and 4-part proximal humeral fractures. Performing RTSA for acute proximal humeral fractures presents the unique challenges of tuberosity management, bone loss, and instability compared with elective indications such as CTA or irreparable RCTs. The purpose of this study was to compare the clinical outcomes, active range of motion (ROM), radiographic outcom

Percutaneous pin removal in the outpatient clinic--do children require analgesia?: a randomized controlled trial.

BACKGROUND: Percutaneous pins used in the surgical fixation of fractures in children are often removed in the outpatient clinic without the administration of analgesia. Pin removal can be a cause of anxiety for children, parents, and caregivers. Relatively little is known about the requirement of analgesia for this procedure. In a randomized controlled trial, we evaluated whether oral acetaminophen or ibuprofen reduced the pain experienced during pin removal. METHODS: Participating in the study were 240 children between the ages of five and twelve years who had two or three percutaneous pins

The burden of chronic elbow dislocations in Cambodia and early results of a cost-effective surgical approach.

Chronic elbow dislocation presents a surgical challenge and there is difficulty in balancing stability with early mobilisation. We present a series of 103 patients treated with open reduction via a posterior approach and provide early results of an alternative combined medial and lateral approach (Soddo technique, Anderson et al.). Of the 103 patients, 81% initially consulted a traditional healer and the mean dislocation period was 11 weeks. There was significant loss to follow-up. Only 12 patients having undergone the posterior approach had complete datasets. The mean preoperative arc of move

Surgical Methods for the Management of Intra-articular Distal Humerus Fracture: A Systematic Review and Bayesian Network Meta-analysis.

BACKGROUND: Intra-articular distal humerus fractures (DHF) carry a technical challenge for orthopedic surgeons, owing to the complex geometry of the distal humerus. Surgical management is the standard of care for these fractures. The aim of this network meta-analysis was to compare the clinical outcomes of the different open reduction and internal fixation (ORIF) and arthroplasty techniques for the management of intra-articular DHFs. METHODS: The databases Medline, Embase, and CENTRAL were searched for relevant studies. Randomized controlled trials (RCTs) were included if they matched the fol

A High Level of Fibrinogen Degradation Product on Arrival as the Only Clue Suggesting Deterioration in a Blunt Trauma Patient.

We report the case of an 89-year-old woman who was struck by a car while walking and fell to the ground. She had hypertension, dyslipidemia, and cerebral infarction requiring medication. She was transported to a nearby acute critical care center. Upon arrival, her vital signs were stable. A physical examination showed right facial and hip contusion, right shoulder tenderness, a right elbow contusional lacerated wound, and bilateral knee abrasion wounds. She vomited when her face moved. Radiological studies showed a right proximal humerus fracture and a right minor ischial fracture. Her injury

Short-term clinical and radiographic outcomes in patients treated with a modern intramedullary nailing system for 2-part proximal humerus fractures.

OBJECTIVES: The optimal surgical treatment of displaced proximal humerus fractures (PHFs) remains controversial. There are advocates for both open reduction and internal fixation with plate and screws (ORIF) and intramedullary nailing (IMN). The purpose this study was to evaluate the early-term clinical and radiographic outcomes of IMN for isolated, displaced 2-part surgical neck PHFs using a modern, straight nail system and to determine the effect of preoperative patient and fracture characteristics on outcome. METHODS: This was a case series of 23 patients with displaced 2-part surgical nec

Is cross-screw fixation superior to plate for radial neck fractures?

This study compares the outcomes of two methods of fixation of displaced fractures of the radial neck. The 58 patients with a mean age of 38.5 years (18 to 56), were treated in a non randomised study with screws (n = 29) or a plate and screws (n = 29) according to the surgeon's preference. The patients were reviewed at one year. Radiographs and functional evaluations were carried out up to one year post-operatively, using the Broberg and Morrey functional evaluation score, range of movement, and assessment of complications. The mean functional scores did not differ significantly between groups
