Influence of breech presentation on the development of fetal arm posture.

BACKGROUND: In previous research, an age-related developmental trend towards increasing arm flexion has been found for cephalic fetuses. AI … n either group. Both breech and cephalic fetuses showed a clear preference for flexion in elbow and finger joints at all studied ages. After 36 weeks gestational age, the breech group showed significantly less wrist flexion when compared to the cephalic group (p = 0.037). A clear preference for location of the fetal hands near the fetal head could be observed …

Clinical and Molecular genetics of Stickler syndrome.

Stickler syndrome is an autosomal dominant disorder with characteristic ophthalmological and orofacial features, deafness, and arthritis. Abnormalities of vitreous gel architecture are a pathognomonic feature, usually associated with high myopia which is congenital and non-progressive. There is a substantial risk of retinal detachment. Less common ophthalmological features include paravascular pigmented lattice degeneration and cataracts. Non-ocular features show great variation in expression. Children with Stickler syndrome typically have a flat midface with depressed nasal bridge, short nose

Use of the free innervated dorsalis pedis tendocutaneous flap in composite hand reconstruction.

We used the free dorsalis pedis flap including the extensor digitorum longus or the extensor hallucis brevis, and/or the superficial peroneal nerve to reconstruct composite loss of skin and tendons on the dorsum of the hand. Between February 1992 and February 1996 we treated 7 patients with composite tissue loss on the dorsal hand caused by trauma or burn. Six men and 1 woman had an average age of 26 years (range, 19-42 years). Flap size ranged from 3 x 4 cm to 9.5 x 9 cm. The follow-up period ranged from 10 to 44 months. At 1 week postoperatively, active flexion and passive extension commence

Replantation and revascularization of the digits in a community microsurgical practice.

One-hundred and twenty-seven digits in Verdan zones I and II, and 24 thumbs in 106 patients were reviewed from 1979 to 1995. Patients were all followed for a minimum of 5 months to assess early results and complications, and to determine if a short course (minimum 3 days) of dextran 40 anticoagulation adversely affected digit survival. The total survival rate was 88 percent. Digits with only a single artery and vein repaired had a significantly higher rate of failure (p > .05). Index and small fingers also showed a trend toward lower survival. Vein grafts were used in 22 percent of cases. No v

[Parvovirus B19 infections. The cause of fifth disease-erythema infectiosum--can also cause aplastic crises, fetal damage and polyarthritis].

Human parvovirus B19 can be an important etiological factor in aplastic crises in patients with chronic hemolytic anemia, in fetal damage, and in acute polyarthritis. B19-virus can only be grown in cell cultures established from human bone marrow, where the virus production occurs in erythroblasts. Parvovirus can cause severe, often fatal, infections in various kinds of animals, also in their fetuses, and many cases of teratogenic changes have been described. The B19-infection is diagnosed by demonstration of either IgM antibodies or B19-DNA in serum samples obtained in the early phases of inf

Spontaneous arterial perforation: the Ehlers-Danlos specter.

The Ehlers-Danlos syndrome (EDs) is one of the most frequently inherited disorders of connective tissue. Type IV EDs, the arterial-ecchymotic type, is of concern to vascular surgeons because it is frequently associated with spontaneous catastrophic bleeding. This article summarizes our experience with five members of a family and reviews the 31 patients with type IV EDs described in the literature. The 22 male and 14 female patients had a mean age of 26 years. The 36 patients included 23 with easy "bruisability," 22 with hypermobility of their joints (especially the fingers), 13 with transpare

Neural Correlates of Visuomotor Co-regulation in Social Synchronization.

During social activities, people coordinate their movements by exchanging visuomotor information. Interpersonal coordination arises from two complementary processes, involving either voluntary (planned) or spontaneous (emergent) synchronization, whose neurofunctional underpinnings are currently unknown. We investigated the brain correlates of these two synchronization processes during an fMRI finger-tapping task. Thirty participants (17 females, 13 males) were scanned while reproducing a target tempo either synchronizing with (Joint Action, JA) or resisting (Non-interactive, NI) the partner's

Outcomes of Opponensplasty with Transfer of the Palmaris Longus Tendon to the Rerouted Extensor Pollicis Brevis Tendon for Severe Carpal Tunnel Syndrome.

Background: Camitz opponensplasty is a widely used reconstruction procedure for severe carpal tunnel syndrome. However, it has several drawbacks, including poor pronation during thumb opposition and a tendency for flexion at the thumb metacarpophalangeal (MP) joint. We developed a novel procedure to overcome these issues while preserving its advantages. Methods: Our procedure involves mini open carpal tunnel release (KnifeLight) and the transfer of the palmaris longus (PL) tendon to the rerouted extensor pollicis brevis (EPB) tendon using the distal part of the PL tendon as a pulley. We assess

Developing and Validating an Intelligent Mouth-Opening Training Device: A New Solution for Restricted Mouth Opening.

Restricted mouth opening (trismus) is one of the most common complications following head and neck cancer treatment. Early initiation of mouth-opening exercises is crucial for preventing or minimizing trismus. Current methods for these exercises predominantly involve finger exercises and traditional mouth-opening training devices. Our research group successfully designed an intelligent mouth-opening training device (IMOTD) that addresses the limitations of traditional home training methods, including the inability to quantify mouth-opening exercises, a lack of guided training resulting in temp

Extension Block Pinning Vs Single Kirshner Wiring To Treat Bony Mallet Finger. A Retrospective Study.

BACKGROUND AND AIM: Bony Mallet Finger or Mallet Fracture is a common injury of the hand, which follows a forced flexion of the extended distal interphalangeal joint, that leads to a bony avulsion of the distal phalanx. Depending on fracture extension and dislocation, those lesions can either be treated conservatively or surgically. Several surgical options have been described in the literature. The aim of this study is to compare retrospectively two percutaneous pinning techniques: the extension block technique according to Ishiguro vs an original single Kirshner wiring (Umbrella technique).

Topical Corticosteroids: Choice and Application.

Topical corticosteroids are an essential tool for treating inflammatory skin conditions such as psoriasis and atopic dermatitis. Topical corticosteroids are classified by strength and the risk of adverse effects such as atrophy, striae, rosacea, telangiectasias, purpura, and other cutaneous and systemic reactions. The risk of adverse effects increases with prolonged use, a large area of application, higher potency, occlusion, and application to areas of thinner skin such as the face and genitals. When prescribing topical corticosteroids for use in children, lower potencies and shorter duration

Lateral Toe Pulp Flap Used in Reconstruction of Distal Dorsal Toe Defect: Case Report and Review of the Literature.

BACKGROUND: When the distal dorsal part of the great toe is injured, especially with exposure of a tendon, bone, or joint, applying a free or local flap is difficult because of the lack of locally available tissue for reconstruction. Management of the distal dorsal part of a great toe soft tissue defect can be challenging for plastic surgeons. PATIENT AND METHOD: An 18-year-old woman presented with an injury to the dorsal aspect of her right great toe caused by a cobra bite. After fasciotomy, the wound showed exposure of the extensor hallucis longus tendon. After demarcation and infection con

A Kuramoto model of self-other integration across interpersonal synchronization strategies.

Human social behaviour is complex, and the biological and neural mechanisms underpinning it remain debated. A particularly interesting social phenomenon is our ability and tendency to fall into synchronization with other humans. Our ability to coordinate actions and goals relies on the ability to distinguish between and integrate self and other, which when impaired can lead to devastating consequences. Interpersonal synchronization has been a widely used framework for studying action coordination and self-other integration, showing that even in simple interactions, such as joint finger tapping

KLF2 Protects against Osteoarthritis by Repressing Oxidative Response through Activation of Nrf2/ARE Signaling In Vitro and In Vivo.

Osteoarthritis (OA) is a multifactorial and inflammatory disease characterized by cartilage destruction that can cause disability among aging patients. There is currently no effective treatment that can arrest or reverse OA progression. Kruppel-like factor 2 (KLF2), a member of the zinc finger family, has emerged as a transcription factor involved in a wide variety of inflammatory diseases. Here, we identified that KLF2 expression is downregulated in IL-1β-treated human chondrocytes and OA cartilage. Genetic and pharmacological overexpression of KLF2 suppressed IL-1β-induced apoptosis and matr
