Lateral gastrocnemius myocutaneous flap transposition to the midlateral femur: extending the arc of rotation.

We report the successful use of an extended lateral gastrocnemius myocutaneous flap for coverage of the midlateral femur using successive delayed elevations. A 62-year-old man underwent wide resection of a liposarcoma of the right anterior thigh with free flap reconstruction and subsequent radiation therapy 10 years before. Four years later, the patient fractured his irradiated femur and was treated with a retrograde intramedullary nail, which subsequently became infected, causing osteomyelitis of the distal femur, septic arthritis of the knee joint, and nonunion of his pathologic fracture. Risk factors for overgrowth after flexible intramedullary nailing for fractures of the femoral shaft in children.

We analysed retrospectively the risk factors leading to femoral overgrowth after flexible intramedullary nailing in 43 children (mean age 7.1 years (3.6 to 12.0)) with fractures of the shaft of the femur. We reviewed their demographic data, mechanism of injury, associated injuries, the type and location of the fractures, the nail-canal diameter (NCD) ratios and femoral overgrowth at a mean follow-up of 40.7 months (25.2 to 92.7). At that time, the children were divided into two groups, those with femoral overgrowth of < 1 cm (Group 1), and those with overgrowth of ≥ 1 cm (Group 2). The mean fe

Flexible intramedullary nailing in simple bone cysts of the proximal humerus: prospective study for high-risk cases of pathologic fracture.

UNLABELLED: We designed and evaluated a modified cyst index using the Picture Archiving and Communication System software for treating simple bone cysts of the proximal humerus by preventive intramedullary flexible nail insertion in symptomatic children or adolescents. From January 2007 to December 2009, cases with a cyst index of at least 4 were enrolled in the study. Of 20 total cases, 19 were prospectively followed-up for at least 2 years. The mean patient age was 10.6 years (range, 7-15 years) and the mean modified cyst index was 6.56 (range, 4.95-8.25). The mean cyst healing period was 8.

Treatment strategies for intramedullary nailing of femoral shaft fractures.

Intramedullary nailing has become the gold standard to treat femoral shaft fractures. It is unknown which nailing technique orthopedic surgeons prefer. The goal of this study was to determine current techniques and perioperative complications of intramedullary nailing of diaphyseal femoral fractures. Fifty-one institutions in 26 countries participated in an international survey to assess detailed descriptions of preferred operative strategies and perioperative complications. Altogether, 517 cases of diaphyseal femoral fractures were collected. The Internet-based survey incorporated information

Metastatic fractures of the tibia.

Pathologic fractures of the tibia due to a metastasis are rare. The treatment of an established fracture is sometimes conservative, but more often surgical. The purpose of the surgical procedure is to improve the quality of life and the ambulatory status, to relieve pain and to facilitate activities of daily living and nursing care. Four cases of operatively managed metastatic fractures of the tibia are presented with emphasis on the surgical technique. The scarce literature on metastatic tibial fractures in reviewed. The operative technique to be used does not only depend on the location of t

Conversion Total Hip Arthroplasty Following Prior Internal Fixation: A Comparison With Primary and Revision Total Hip Arthroplasty.

BACKGROUND: With rising life expectancy, the incidence of total hip arthroplasty (THA) following prior femoral and/or acetabular fracture fixation is increasing. Conversion THA (cTHA) following these procedures is technically complex and associated with unique challenges. However, direct comparisons between primary THA (pTHA), cTHA, and revision THA (rTHA) remain limited. METHODS: This was a retrospective cohort study comparing intraoperative and postoperative outcomes among patients undergoing cTHA from April 2012 to October 2022. This was defined as THA following previous fixation with hard

Intercalary defects reconstruction of the femur and tibia after primary malignant bone tumour resection. A series of 13 cases.

INTRODUCTION: Performing intercalary segment reconstruction after malignant bone tumour resection results in both mechanical and biological challenges. Fixation must be solid enough to avoid short-term or mid-term mechanical failure. The use of an allograft or autograft must ensure long-term survival of the reconstruction. The goal of this study was to analyse the clinical and radiological outcomes of these reconstructions. PATIENTS AND METHODS: Thirteen patients were operated on eight femurs and five tibias. The median age was 20 years old (range 14-50). The most common diagnosis was osteosa

Treatment of rigid hypertrophic posttraumatic pseudarthrosis of the tibia in children using distraction osteogenesis.

Posttraumatic tibial pseudarthrosis is a relatively uncommon complication of tibia fractures in children. Although reported as a successful method of treating tibial nonunions in adults, distraction osteogenesis through a nonunion site via ring external fixation has not been described in children. The authors report three consecutive cases of distraction through an angulated, shortened, hypertrophic, posttraumatic nonunion to achieve successful union and concurrent correction of deformity. Distraction was carried out using a ring fixator with computer-guided correction. Union was achieved in e

A method for fixation of femoral fractures below previous hip implants.

Previously applied plates and screws or endoprosthetic implants of the proximal femur create a recurrent problem in surgery for fractures below such implants. Of 13 elderly women (mean age, 81 years) six had had a hip arthroplasty, five a nail and plate, and two a nail or a plate only. The fresh fractures were stabilized from the condylar region by intramedullary nails connected to a coupling-piece with two transverse screws preventing extrusion or intrusion of the nails. All fractures healed uneventfully without rotatory and angulatory malalignment. The maximum shortening was 2 cm in two pati

Flexible intramedullary nailing in the treatment of paediatric forearm fractures: 15-years' experience at a level one trauma centre, comparison of implant retention versus removal and cost-effectiveness analysis.

INTRODUCTION: Flexible intramedullary nailing (FIN) is a common treatment method for paediatric forearm fractures. They are routinely removed after fracture healing. This study aims to assess the 15-years' experience of managing paediatric forearm fractures with FIN, with comparison between the outcomes of implant retention versus removal and cost analysis. This is the first study on FIN retention in paediatric forearm fractures. METHODS: This was retrospective cohort study for paediatric forearm fractures treated with FIN between 2009 and 2024 at Leeds General Infirmary, a level one trauma c

A Comparative Analysis of Distal Femur Plating and Intramedullary Nailing in the Management of Distal Femoral Fractures: A Retrospective Cross-Sectional Study.

BACKGROUND: Distal femoral fractures constitute a significant proportion of all femoral fractures and are increasingly common due to the aging population and high-energy trauma in younger individuals. Surgical management is preferred for most cases, with the two predominant modalities being distal femoral locking plates (DFLP) and retrograde intramedullary nailing (RIMN). The choice between plating and nailing remains controversial, with ongoing debate regarding optimal treatment strategies based on fracture characteristics, patient factors, and surgeon preference. OBJECTIVE: The primary obje

Combined approach for intrapelvic abscess drainage among adults.

UNLABELLED: Obturator internus muscle (OIM) and adductor brevis (AB) abscess occurs rarely in adults. This condition mainly affects children experiencing trauma. This paper presents and discusses a unique case of OIM and AB abscess in an adult male that was treated operatively with a combined surgical approach (modified Stoppa and Smith-Peterson) after multiple failed drainage procedures. CASE: A 23-year-old male underwent reduction and fixation of his right tibiae shaft fracture with an intramedullary nail. After several referrals to the emergency room due to recurrent symptoms of fever and

Tibiotalar joint arthrodesis for the treatment of severe ankle joint degeneration secondary to rheumatoid arthritis.

The technical aspects of fusion of the rheumatoid ankle do not deviate from those in the post-traumatic or osteoarthritic ankle. Screw fixation can usually be achieved, and rarely is fixation failure a problem in rheumatoid ankle arthrodesis. If fixation is difficult because of deformity or bone quality, external fixation or locking intramedullary nails should be used. The placement of cannulated screws and adequacy of screw fixation has not been a problem (Fig. 13). Screw fixation provides compression and prevents rotation. The surgeon, however, needs to be assured that no screws invade the s

Combined two-centre experience of single-entry telescopic rods identifies characteristic modes of failure.

AIMS: The Fassier Duval (FD) rod is a third-generation telescopic implant for children with osteogenesis imperfecta (OI). Threaded fixation enables proximal insertion without opening the knee or ankle joint. We have reviewed our combined two-centre experience with this implant. METHODS: In total, 34 children with a mean age of five years (1 to 14) with severe OI have undergone rodding of 72 lower limb long bones (27 tibial, 45 femoral) for recurrent fractures with progressive deformity despite optimized bone health and bisphosphonate therapy. Data were collected prospectively, with 1.5 to 11

Conservative Management with Functional Brace Versus Various Surgical Fixation Techniques for Humeral Shaft Fractures: A Network Meta-Analysis.

BACKGROUND: Historically, humeral shaft fractures have been successfully treated with nonoperative management and functional bracing; however, various surgical options are also available. In the present study, we compared the outcomes of nonoperative versus operative interventions for the treatment of extra-articular humeral shaft fractures. METHODS: This study was a network meta-analysis of prospective randomized controlled trials (RCTs) in which functional bracing was compared with surgical techniques (including open reduction and internal fixation [ORIF], minimally invasive plate osteosynt
