Infective Endocarditis Presenting with Intracranial Bleeding.

BACKGROUND: Infective endocarditis (IE) can be complicated by intracranial bleeding (ICB) caused by different pathologic mechanisms. The occurrence of ICB in patients with IE significantly influences therapeutic decisions and has a negative impact on outcome. CASE REPORT: We describe the clinical courses of 3 patients with aortic prosthetic valve IE presenting with ICB. Patients 1 and 2 experienced subarachnoid hemorrhage (SAH) and intracerebral hemorrhage (ICH), respectively, caused by rupture of an intracranial infectious aneurysm (IIA). Both underwent endovascular treatment of IIA with goo

Posterior circulation hyperperfusion syndrome after superficial temporal artery-superior cerebellar artery bypass for vertebral artery dissection.

A 68-year-old man was diagnosed with infarction of the cerebellum and medulla oblongata caused by vertebral artery dissection manifesting as severe stenosis with poor collateral flow. He underwent superficial temporal artery (STA)-superior cerebellar artery (SCA) bypass for the prevention of fatal brain stem infarction. He had consciousness disturbance 2 days postoperatively. Single-photon emission computed tomography revealed hyperperfusion in the posterior circulation. His consciousness improved as hyperperfusion improved. We report the first case of posterior circulation hyperperfusion synd

Postoperative changes in the superficial temporal artery and the external carotid artery duplex sonography after extra-intracranial bypass surgery in European Moyamoya disease.

INTRODUCTION: Despite Duplex ultrasonography being a noninvasive, easily repeatable, readily available and economical tool, this examination and its normal ranges are rarely described in Moyamoya disease (MMD). METHODS: Duplex ultrasonography examinations of the superficial temporal artery (STA) and external carotid artery (ECA) were performed preoperatively, postoperatively (within 30 days) and as follow-up exams (6 months postoperatively) after 32 cerebral revascularization procedures in 20 European patients with MMD. RESULTS: A significant higher mean diastolic flow of STA compared to pre

Endovascular treatment of stenoses in a pediatric patient with incomplete aortic duplication, mesenteric ischemia, and renovascular hypertension.

Variations in abdominal aortic anatomy may have significant implications in various surgical procedures. We report here a pediatric patient with symptoms of chronic mesenteric ischemia, labile hypertension, and lower extremity claudication. Angiography revealed a partially duplicated aorta with the anterior aorta containing the splanchnic and renal arteries and the posterior segment perfusing the lower extremities. She was successfully treated with balloon angioplasty of two focal stenoses and is normotensive without abdominal symptoms at 1-year follow-up. To our knowledge, this is the first r

Visual grading system for vasospasm based on perfusion CT imaging: comparisons with conventional angiography and quantitative perfusion CT.

BACKGROUND: The purpose of this study was to compare simple visual grading of perfusion CT (PCT) maps to a more quantitative, threshold-based interpretation of PCT parameters in the characterization of presence and severity of vasospasm. METHODS: Thirty-three patients with acute subarachnoid hemorrhage were enrolled in a prospective study and underwent a total of 40 paired PCT and digital subtraction angiography (DSA) examinations. A neuroradiologist and a neurologist reviewed the PCT mean transit time (MTT), cerebral blood flow (CBF), and cerebral blood volume maps independently; they evalua

Pseudofenestration of the cervical internal carotid artery: a pathologic process that simulates an anatomic variant.

Fenestration of the cervical segment of the internal carotid artery (ICA) is considered an extremely rare anatomic variant, with, to the best of our knowledge, only four cases reported in the literature. We report six angiographic observations of apparent fenestration of the cervical ICA, which we believe are secondary to arterial dissection. We discuss the angiographic appearance of these pseudofenestrations and their potential clinical implications.

[Combination of endovascular procedures with open vascular surgery].

The combination of open surgery with endoluminal catheter techniques offers new chances in the treatment of multi-staged arterial occlusive disease. We report our experiences in 205 patients. In n = 125 patients TEA or bypass were undertaken in the groin with simultaneous catheter intervention proximally or distally. In 31 patients the operation was carried out above knee and in 49 cases below knee. 94 patients with an open operation in the groin and simultaneous iliac catheter interventions are in the follow up. For these interventions the combined secondary patency rate was 77 % after three

Impact of cerebral microcirculatory changes on cerebral blood flow during cerebral vasospasm after aneurysmal subarachnoid hemorrhage.

BACKGROUND AND PURPOSE: Cerebral microcirculatory changes during cerebral vasospasm after aneurysmal subarachnoid hemorrhage (SAH) are still controversial and uncertain. The aim of this study was to investigate the changes of cerebral microcirculation during cerebral vasospasm and to clarify the roles of microcirculatory disturbances in cerebral ischemia by measuring cerebral circulation time (CCT) and regional cerebral blood flow (rCBF). METHODS: In 24 cases with aneurysmal SAH, rCBF studies by single-photon emission CT and digital subtraction angiography (DSA) were performed on the same day

Angles between A1 and A2 segments of the anterior cerebral artery visualized by three-dimensional computed tomographic angiography and association of anterior communicating artery aneurysms.

OBJECTIVE: The angle of arteries at bifurcations, as well as the blood flow, are factors of hemodynamic stress on the apical region, where aneurysms often develop. Using images obtained with three-dimensional computed tomographic angiography, we sought to determine the angles between the A1 and A2 segments of the anterior cerebral artery of the anterior communicating artery (ACoA) complex associated with aneurysms. These angles cannot be detected by conventional cerebral angiography. METHODS: The course of the anterior cerebral artery was studied using three-dimensional computed tomographic a

Non-contrast ultrashort echo time magnetic resonance imaging-guided staging predicts in vivo lower extremity endovascular procedural complexity.

BACKGROUND: Our understanding of plaque morphology and its relationship with procedural complexity and immediate technical failure (ITF) in percutaneous vascular intervention is poorly understood, particularly in tibial disease due to significant limitations of computed tomographic angiography, digital subtraction angiography, and duplex ultrasound imaging. Ultrashort echo time magnetic resonance imaging (UTE MRI) is a non-contrast, 3T MRI technique that can identify and differentiate plaque morphology and characteristics. The aim of this study was to create a UTE MRI-defined staging system to…

Predicting Aneurysm Occlusion After Pipeline Embolization: an Ensemble Model Using Angiographic Parametric Imaging.

BACKGROUND AND PURPOSE: The Pipeline Embolization Device (PED) revolutionized endovascular treatment of intracranial aneurysms (IAs), especially complex wide-neck lesions, but incomplete occlusion remains common. This study aimed to develop an ensemble model integrating clinical, morphological, and hemodynamic features for predicting post-PED occlusion outcomes. MATERIALS AND METHODS: This retrospective study included 306 patients with IAs treated with PED from January 2016 to June 2023. Clinical, morphological, and hemodynamic features were extracted from clinical records, three-dimensional

Spontaneous Multiple Cervical Artery Dissections Detected with High-Resolution MRI: A Prospective, Case-Series Study.

Background: Cervical artery dissection (CAD) is a leading cause of acute ischemic stroke among young and middle-aged patients. Currently, the growing availability of high-resolution magnetic resonance imaging (MRI), particularly fat-saturated T1-weighted black-blood SPACE sequences, allows the non-invasive, rapid, and reliable diagnosis of multiple arterial dissections. Methods: We reported our experience from two tertiary stroke centers of patients diagnosed with spontaneous multiple cervical artery dissections, detected with high-resolution MRI, during a three-year period (2022-2025). Fatal Reversible Cerebral Vasoconstriction Syndrome: A Complication of Postpartum Haemorrhagic Shock.

Reversible cerebral vasoconstriction syndrome (RCVS) is a rare but serious cerebrovascular disorder characterised by transient narrowing of the cerebral arteries, triggered by internal or external stressors. We present a case of RCVS in a 28-year-old female patient 10 days postpartum, after a haemorrhagic shock. The initial acute management of the abdominal haemorrhagic shock syndrome included massive transfusion of blood products along with vasopressors and crystalloids. Although her condition stabilised initially, she later exhibited severe neurological deficits of unknown cause, prompting f

Visceral Arterial Pseudoaneurysms-A Clinical Review.

Background and Objectives: Visceral arterial pseudoaneurysms (VAPAs) are rare vascular lesions characterized by the disruption of partial disruption of the arterial wall, most commonly involving the intima and media. They have an estimated incidence of 0.1-0.2%, with the splenic artery most commonly affected. Their management poses unique challenges due to the high risk of rupture. Timely recognition is crucial, as unmanaged pseudoaneurysms have a mortality rate of 90%. This narrative review aims to synthesize current knowledge regarding the epidemiology, etiology, clinical presentation, diagn
