[Gustilo type IIIb open fractures of the leg - Emergency treatment by internal fixation and simultaneous skin cover].

Sixteen patients were studied between 1986 and 1992. Fourteen patients were reviewed. One had died and one was lost to follow-up. The average was 35 years. There were 15 men and one woman. In 10 cases the fracture was isolated and in 6 there were multiple injuries. Two pre-operative arterial grafts were done. Four fractures were comminuted and in 3 there was a segmental loss of bone. Antibiotics were given for an average of 8 days using Penicillin and Metronidazole. Surgical treatment involved emergency debridement and fixation of the fracture by intramedullary nailing in 15 cases and by exter

A Unique Case of Bilateral Elbow Terrible Triad in a Polytrauma Patient: Associated Challenges in Management.

INTRODUCTION: Periprosthetic fractures (PPFs) are well-documented complications after total knee arthroplasty but are exceedingly rare following unicompartmental knee arthroplasty (UKA). Given UKA's design preserves native bone stock and maintains near-normal joint biomechanics, PPFs around well-fixed UKA components are infrequently encountered and poorly described in the literature. To the best of our knowledge, there are very few detailed reports of supracondylar femoral or proximal tibial PPFs following mobile-bearing UKA. This case series is important because it highlights not only the rar

Incidence and risk factors of fracture-related infection after SIGN nailing of open femoral fractures in Ethiopia.

Fracture-related infection is one of the most serious complications following open fracture management. Despite its preventable complications, Fracture-related infections continue to threaten public health with significant impacts on human and financial resources. The number of patients with open femur fractures increased in our hospital due to the war in the northern part of Ethiopia, but there was no study. This study aimed to assess the incidence of Fracture-related infection and its associated factors after intramedullary nailing of an open femur fracture using a Surgical Implant Generatio

Long hindfoot nail fixation using standard tibial nails for elderly ankle and distal tibia fractures.

INTRODUCTION: Fragility ankle and distal tibia fractures in the elderly population present a complex clinical situation, due to the poor bone quality, soft tissue condition and medical comorbidities in this age group. This study aims to assess the outcome of long hindfoot nail fixation using standard tibial nails in managing these injuries. This is the largest and possibly first study to date of this implant used in ankle trauma hindfoot fixation. METHODS: This was a retrospective observational study across two level 1 major trauma centres in the United Kingdom, from January 2019 to December

Extreme nailing: standardized definition and outcomes.

OBJECTIVES: The purpose of this study is twofold: (1) to reliably define the concept of "extreme tibial nailing" and (2) assess the feasibility of tibial intramedullary nailing when the fracture extends into the nail's locking bolt zone. METHODS: Design: A retrospective review. SETTING: A single academic center comprised of a specialty orthopedic hospital and a Level I Trauma Center. Patient Selection Criteria: 543 patients who sustained 555 tibia fractures between February 2014 and January 2024 were reviewed by two board-certified orthopedic surgeons. Cases were classified as "extreme naili

Unlocked versus dynamic and static distal locked femoral nails in stable and unstable intertrochanteric fractures. A prospective study.

INTRODUCTION: Few clinical studies have analyzed the utility of distal interlocking screws in stable and unstable intertrochanteric fractures treated with intramedullary devices. We performed a prospective analysis comparing short unlocked versus short dynamic and short static distal locked intramedullary nails. MATERIALS AND METHODS: Nine level-II trauma centres were involved in the study. 240 patients over the age of 65 with a stable (AO/OTA 31-A1) or unstable intertrochanteric fracture (AO/OTA 31-A2) were prospectively investigated. The same type of nail was used in every patient. Patients

Clinical experiences in the use of a gentamicin-coated titanium nail in tibia fractures.

Despite the improvement of surgical techniques surgical site infections (SSIs) still remain clinically challenging in high risk patients undergoing osteosynthesis for tibia fractures. The use of an antibiotic coated implant might reduce the adhesion of bacteria on the implant surface and could therefore reduce the rate of implant-related infection or osteomyelitis. A gentamicin-coated tibia nail was evaluated in a prospective study. Four centers enrolled 100 patients (99 treated) with fresh open or closed tibia fractures, or for non-union revision surgery and followed them for 18 months. Data

Influence of sagittal plane malpositioning of the patella on anterior knee pain after tibia intramedullary nailing.

PURPOSE: Anterior knee pain (AKP) is one of the most common complications after tibia intramedullary surgery. We evaluated changes in patellar tendon length after tibia intramedullary nailing surgery using a transtendinous approach and assessed the importance of nail position in relation to the anterior tibial cortex and joint line. METHODS: Two surgeons blinded to patients' pain status measured both the knee Insall-Salvati and Caton-Deschamps indexes on 30° flexion lateral knee X-rays of 33 patients. Superior nail prominence (the distance from the proximal tip of the nail to the tibial plate

Minimally invasive clamp-assisted reduction and cephalomedullary nailing without cerclage cables for subtrochanteric femur fractures in the elderly: Surgical technique and results.

INTRODUCTION: The treatment of subtrochanteric fractures in the elderly remains technically challenging, due to instability and osteoporosis, with high reoperation rates. Even if intramedullary nailing is the most reliable treatment, reduction is difficult and cerclage wiring remains controversial. The purpose of this study was to evaluate 26 consecutive subtrochanteric fractures in elderly patients treated with a minimally invasive clamp-assisted reduction and cephalomedullary nailing without cerclage wiring. PATIENTS AND METHODS: A retrospective analysis was conducted between January 2010 a

The Fixion expansion nail in the surgical treatment of diaphyseal fractures of the humerus and tibia. Our experience.

The authors present their experience in the application of a Fixion expansion intramedullary nail for the treatment of diaphyseal fractures of the humerus and tibia, in a total of 40 surgeries. The features of the nail are: stability and flexibility of the instrumentation, speed of surgery, minimal exposure to radiation. The results obtained up to now are encouraging; there are no significant problems either during or after surgery, and mean consolidation time is 3 months for fractures of the humerus, and 4 months for those of the tibia. The Fixion nail is a versatile type of instrumentation t

Treatment of low-energy tibial shaft fractures: plaster cast compared with intramedullary nailing.

We analyzed data from 87 patients who had displaced closed or open grade I simple or spiral wedge tibial shaft fractures caused by low-energy impact. Fifty-four patients were treated with plaster cast and 33 with intramedullary locking nail (IMLN). Delayed union only occurred in 8 patients after plaster cast treatment. Forty-two patients in the IMLN group and one in the plaster cast group suffered from anterior knee pain. Final treatment outcome, healing time, hospitalization time and duration of sick leave were assessed on the basis of 25 matched pairs of patients. Mean healing time, hospital

[Surgical stabilization of per- and subtrochanteric femoral fractures with the gamma nail].

BACKGROUND: From 1992 through 1997 96 patients with per- or subtrochanteric femur fractures were treated with a Gamma nail. We retrospectively evaluated the influence of patient age, additional diseases, type of fracture, time of operation, type of implant (short/long Gamma nail) and surgical approach (open/closed reduction) on the mobility of the patients, healing of the fractures on radiographs, and possible complications. PATIENTS AND METHODS: The average patient age was 72.5 years (range 27 to 101). There were 27 male and 69 female patients. Surgery was performed 1.19 (0-10) days after in

Subtrochanteric fractures of the femur treated with the Zickel nail.

In a retrospective study of 41 subtrochanteric fractures treated with Zickel intramedullary nails (with a follow-up period of at least 1 year) there was a union rate of 100 per cent (39/39). Union occurred between 4 and 7 months (average healing time 5.2 months). There were no implant failures. Four patients have had their Zickel nails successfully removed. In 48 per cent of the cases we had some difficulties with the stability of the fracture, and in 14 per cent, all of them long oblique comminuted fractures, skeletal traction was needed for 6 weeks. In nine cases there was a shortening of 1.

A Clinical Study on Mobility Outcomes After Surgical Fixation of Intertrochanteric and Shaft Femur Fractures.

BACKGROUND: Femoral fractures are a major orthopedic burden with comprehensive implications on both the mobility and health care resources of the patient. Early rate of recovery and functional locomotion is the most significant in its ability to reduce hospitalization and maximize postoperative quality of life after surgical repair. This study was performed to compare early postoperative mobility, complications, and length of stays of intertrochanteric and femur shaft fracture patients undergoing surgery. METHODS: The prospective study consisted of 90 adults with isolated intertrochanteric or
