BACKGROUND: Various corrective osteotomy techniques have been described in the literature for correcting paediatric cubitus varus. But we are still in search of the perfect technique that gives maximum possible deformity correction and cosmetic appearance that satisfies parents with minimal complications. We compared the outcomes of two technically sound osteotomy techniques having minimal postoperative lateral condyle prominence described in the literature.
RESEARCH QUESTION: Is modified reverse step-cut osteotomy (MRSO) better in terms of clinical, radiological, and cosmetic outcomes than Y

BACKGROUND: Fishtail deformity is a rare deformity of the humerus exhibiting concavity of the lateral trochlea, resulting in ulnotrochlear joint derangement. We wanted to share our experience that osteochondritis dissecans of the capitellum is a common associated complication.
OBJECTIVE: To summarize imaging of fishtail deformity in children centered on complications of the radiocapitellar joint.
MATERIALS AND METHODS: From the radiology information system, we identified all patients <18 years with the diagnosis of fishtail deformity. We included only patients with V-shaped deformity of the

OBJECTIVE: To investigate the operative method and clinical results of arthroscopic assisted treatment of shoulder dislocation combined with fracture of greater tuberosity of humerus.
METHODS: From February 2006 to June 2008, 12 cases of shoulder dislocation (6 left, 6 right) combined with greater tuberosity fractures were treated. There were 4 males and 8 females with an average of 58.5 years (range 34-79 years). Eleven cases fall down and one was crushed. The time from injury to hospital averaged 2.1 hours (range 30 minutes-24 hours). X-ray films revealed greater tuberosity fractures with a

In contrast to anterior dislocations, traumatic dorsal shoulder dislocation is a rare complication of upper extremity injuries. If the diagnosis is overseen and treated incorrectly, severe mobility restrictions might be the consequence for the injured individual. Hence, the aim of this article is to demonstrate the adequate diagnostic and therapeutic management and to critically discuss the literature.

BACKGROUND: The purpose of this study was to compare the outcomes of two different techniques of supracondylar osteotomy, French osteotomy and dome osteotomy, for the correction of posttraumatic cubitus varus.
METHODS: A comparative randomized study was undertaken of 25 patients (average age 10 years) with an established posttraumatic cubitus varus deformity (mean time from injury to corrective osteotomy, 1.7 years). Patients were followed-up with for 1 year, and carrying angle, Baumann's angle, and internal rotation deformity were measured. Postoperative complications were assessed.

Children are particularly prone to developing moderate to severe Volkmann's ischemic contracture following a supracondylar fracture of the humerus or its treatment. In order to treat such contractures, intensive hand therapy and a gracilis free muscle transfer to the digital flexors were used in 8 patients at an average age of 6 years 2 months. All transfers were successful. At follow-up (1 to 8 years, average 3 years), despite limited wrist extension in 5/8 patients and mean grip and pinch strengths between 22 and 43% of normal for age and dominance, all patients regained considerable finger

We have reviewed, at an average of 7 years, 14 children with flexion supracondylar fractures of the humerus. These fractures accounted for 6 per cent of the 288 displaced supracondylar fractures treated from 1981 to 1985. A total of 12 patients had typical fractures that were treated by manipulation and immobilization in an extension cast. Two patients had atypical fractures with anterior displacement and posterior angulation of the distal fragment; they were treated by manipulation, but were more stable when immobilized with the elbow flexed with strapping and collar and cuff support. Overall

18 patients with avulsions of the medial epicondyle 1 10/12 to 16 2/12 years after trauma are presented. Non-union occurs very often after conservative treatment involving avulsion with or without displacement. Although non-union of the medial epicondyle gives no or only little disability, the fixation with one or two Kirschner wires is recommended to avoid non-union. Open reposition is necessary in cases with injury of the n. ulnaris and in cases with incarceration of the epicondyle in the elbow joint.

The anterior interosseous nerve syndrome is a partial motor paralysis of the median nerve. Despite being well protected in the deep forearm, the nerve may be subject to traction and pressure phenomena occurring spontaneously or related to trauma. Forearm pain, fracture of an upper extremity, or the characteristic disturbance of pinch should alert the examiner to the possibility of a lesion of the anterior interosseous nerve. Function may be regained in some instances with conservative management. It is probably unwise to delay surgical exploration beyond eight to 12 weeks if the patient has sh

There are many patients who visit a clinic with some complaints at the elbow joint. In such cases, it is customary to evaluate radiographs in making diagnosis. However, it is often difficult to interpret the X-ray films due to structural complexity of the elbow joint. Although in some cases arthrography is performed, there is neither standard technique nor standard method of projection established, hence the evaluation of the arthrogram is often quite difficult. The purpose of this paper thereby is to study the role of the arthrography of the elbow joint as a diagnostic tool second to plain X-

A long term follow-up study of 54 fractures of the lateral condyle of the humerus was done to assess their special problems during the period of skeletal growth. It was demonstrated, that valgisation of the carrying angle will always be a result of an increasing displacement of the peripheral fragment. Furthermore it was shown that the most significant growth disturbance following this fracture is a partial stimulation of the radial part of the growth plate. This leads to radial overgrowth and thereby to varus deformity of the axis of the elbow joint. The extent of this varus deformity is sign

Fifty-six cases of anterior interosseous nerve paralysis including our thirteen cases were collected by reviewing the Japanese literature. The causes of the paralysis were similar to those in the cases reported in other countries. Paralysis is often only partial, and this is of importance in the differential diagnosis.

BACKGROUND: Lateral condyle fractures are associated with high morbidity due to their risk of nonunion and avascular necrosis (AVN). This study aims to assess the outcomes between closed reduction and the more traditional open techniques for operative fractures.
METHODS: All lateral condyle fractures that required operative fixation (pins or screws) over a ten-year period were included. We compared open versus closed reduction for OR time, infection rate, AVN, nonunion, premature physeal closure, ulnohumeral angle, and interepicondylar width (IEW).
RESULTS: 28 patients were identified in the

OBJECTIVES: The surgical treatment of comminuted fractures of the distal humerus remains a challenging problem. The aim of the present study was to compare the clinical outcomes of primary external fixation with second-staged open reduction and internal fixation (ORIF) and initial definitive internal fixation in surgically treated patients with comminuted distal humerus fractures.
DESIGN: Retrospective comparative study.
SETTING: Level one trauma center.
PATIENTS: A total of 24 patients (median age 52 years; range 14-84 years) were included: 15 patients were treated with initial definitive

BACKGROUND: The incidence of relevant posttraumatic functional deficits in the sense of elbow stiffness with less than 30 degrees in extension or flexion less than 120 degrees , is unknown. A differentiation can be made between intraarticular, extraarticular and combined causes. An open procedure is indicated in elbow stiffness after correct analysis of the situation and failure of conservative treatment.
MATERIALS AND METHODS: An open procedure was carried out on 35 patients between March 1995 and November 2001, 10 (3-24) months after the trauma. The mean age of the 24 men and 11 women was 4

The subclavius posticus muscle is a rare aberrant muscle that traverses from the costal cartilage of the first rib posterolaterally to the superior border of the scapula. We report a patient having persistent paralysis of shoulder abduction with wrist and finger extension after a humeral neck fracture. Electromyography (EMG) examination revealed injuries to several upper extremity peripheral nerves, including the radial, axillary, and musculocutaneous nerves. Magnetic resonance imaging (MRI) performed at 10 months post-injury showed severe entrapment of the left brachial plexus by the subclavi
