Poor correlations between measurements of bone quality by quantitative ultrasound sonography and dual energy X-ray absorptiometry in patients with beta-thalassaemia major.

OBJECTIVES: Osteopenia/osteoporosis is a major component of morbidity even in young patients with beta-thalassaemia major. Dual energy X-ray absorptiometry (DXA) is the reference method for determining bone mineral density (BMD). Quantitative ultrasound sonography (QUS) for bone measurement is a relatively new, inexpensive and radiation-free method that could serve as an alternative to DXA. Our aim was to assess bone status in thalassaemic patients both with QUS and DXA and, consequently, to investigate the degree of correlation between the two methods. METHODS: Thirty-three patients (15 male

Changes in scaphoid bone density after acute fracture.

The radiographic density of the proximal fragments of 16 scaphoid fractures was assessed on scaphoid series radiographs taken at 6 to 12 weeks. In addition, dual energy X-ray absorptiometry measurements of bone mineral density in the distal radius and proximal and distal fracture fragments were performed at 1 to 2 weeks and 6 to 12 weeks. Median reductions of 9% and 10% were observed in bone mineral density in the proximal fracture fragment and the distal radius respectively, but these did not correlate with the radiographic density of the proximal fragment. A greater median reduction in bone

Microfracture and changes in energy absorption to fracture of young vertebral cancellous bone following physiological fatigue loading.

STUDY DESIGN: Fifty-five human thoracolumbar vertebrae were randomly fatigue loaded and analyzed. OBJECTIVES: The purpose of this study was to explore the relationship between fatigue loading, trabecular microfracture, and energy absorption to fracture in human cadaveric thoracolumbar vertebrae. BACKGROUND: Although trabecular microfractures are found in vivo and have been produced by fatigue loading in vitro, the effect of the level of physiologic fatigue loading on microfracture and energy absorption has not been investigated. METHODS: Fifty-five human thoracolumbar vertebrae (T11-L4) wer

Relationship between absorptiometry and ultrasound measurements of abdominal subcutaneous fat tissue in postmenopausal women.

The purpose of this study was to describe the association of abdominal subcutaneous adipose tissue (SAT) as assessed by ultrasound with fat tissue in the abdomen, trunk, and other areas as measured by DXA in 101 postmenopausal Caucasian women (62.5 years; 27.3 kg/m(2); 43.0% body fat). Ultrasound SAT thickness was calculated with electronic calipers positioned at the skin-fat and fat-muscle computer screen interface, at the suprailiac (SUPT) and abdominal (ABDT) sites. Pearson correlation showed significant ( p<0.001) coefficients between SAT by DXA at both ABDT ( r=0.644) and SUPT ( r=0.537).

Identifying and managing morphologic complications of HIV and HAART.

Morphologic complications continue to be observed in patients receiving HAART. Body habitus changes include fat accumulation, lipoatrophy, and HIV-associated wasting. Anthropometry, dual-energy x-ray absorptiometry, and imaging methods (CT, MRI) are most useful for detecting fat redistribution syndromes, while bioelectric impedance analysis is useful for determining and monitoring wasting. Various clinical interventions, including diet and exercise, switching antiretroviral agents, the use of lipid-lowering and insulin-sensitizing agents, recombinant human growth hormone therapy, and plastic s

Bone mineral density in adults with Down's syndrome.

The objective of the study was to elucidate if individuals with Down's syndrome (DS) are likely to experience an increased risk of osteoporosis with advancing age, in addition to precocious aging and their skeletal anomalies. Bone mineral density (BMD) was measured in 22 home-reared adults (9 males and 13 females; age 26.22 +/- 4.45 and 23.65 +/- 3.23 years, respectively) by dual energy X-ray absorptiometry (DXA). The BMD of the second to fourth lumbar vertebrae was measured in posteroanterior projection and the mean density expressed as grams per square centimetre. The BMD of DS individuals w

Skeletal changes in the ovariectomised ewe and subsequent response to treatment with 17 beta oestradiol.

The ewe has been suggested as a suitable large animal model for assessment of therapeutic agents for the treatment of osteoporosis. In this study we asses the response of ewe bone mineral parameters, using dual energy x ray absorptiometry (DXA), after ovariectomy. Regular DXA analysis was performed over a period of 75 weeks followed by a period of dosing with 17 beta oestradiol. Total body bone mineral density (BMD) was reduced in ovariectomised animals 15 weeks post operatively. BMD remained lower than control animals for the entire pre dose period. Subsequently, dosing with 17 beta oestradio

Osteopenia in men with a history of delayed puberty.

BACKGROUND AND METHODS: The effect of delayed puberty on peak bone mineral density in men is unknown. To determine whether such a delay reduces normal peak bone density and leads to osteopenia during adulthood, we measured radial bone mineral density by single-photon absorptiometry and spinal bone mineral density by dual-energy x-ray absorptiometry in 23 men who had a history of constitutionally delayed puberty and 21 men who underwent normal puberty. Their mean ages were 26 and 24 years, respectively. The groups were matched for other factors known to affect bone mass. RESULTS: The mean (+/-

Microdensity of solitary bone cyst after steroid injection.

The change of bone density in five patients with solitary bone cysts (SBC) was directly measured by microdensitometry (MD). Standard plain radiographs could not always predict the strength of the bone, but with MD, smaller changes in bone density could be found quantitatively. The density of the lesion temporarily decreased just after steroid injection and began to increase to maximum level in approximately 4 or 5 months. However, in two patients, the density decreased gradually to the first MD levels and refractured. In this article we stress the usefulness of direct MD in detecting the stren

Truncal adiposity and lung function in older black women.

The increase in adiposity associated with aging is a concern in older adults, especially as it relates to the risk for ventilatory complications. Therefore, the specific aim of this study was to determine the association of various measures of abdominal adiposity with lung function in a sample of older healthy black women. Participants (n=27) had no history of diabetes or respiratory disease. The mean age was 67 years. Lung function was measured by spirometry using percent of predicted values for forced vital capacity (FVC) and forced expiratory volume in 1 second (FEV1). Body fat was measured

[Diagnosis, prevention and therapy of osteoporosis].

Osteoporosis is a generalized osteopathy manifesting itself mainly in women beyond 60 years of age. We still lack a uniform definition of osteoporosis, generally defined as a decrease in bone mass in comparison with an age- and sex-related norm with normal mineralization of the remaining bone tissue. Of great importance for diagnosis are, above all, radiological procedures (plain X-rays of the spine, photon absorptiometry, computerised tomography), besides the medical history (backache, fractures) and the physical examination (kyphosis, shortening of the trunk, decrease in height). Furthermore

The effect of hemiplegia on bone mass and soft tissue body composition.

The content of bone mineral (BMC), lean tissue, and fat tissue were measured by single and dual photon absorptiometry in both the paretic and the non-paretic limbs of 15 patients, hemiplegic due to cerebrovascular accident 23-38 weeks earlier. Compared with the non-paretic arm, the paretic arm had approximately 10% lower (P less than 0.01) BMC. This difference was largest at the measuring site with the highest ratio of trabecular to compact bone. The paretic leg had a 4% (P less than 0.001) lower BMC than the non-paretic leg. For both the arms and the legs, the lean content was lower (P less t

Metabolic bone disease in primary biliary cirrhosis at presentation.

Metabolic bone disease, particularly osteoporosis, is a complication of advanced primary biliary cirrhosis, but the extent of the problem is unclear. We present 33 patients who were investigated for bone disease at the time of diagnosis of their liver disease and who had received no prior treatment likely to influence their bones. Iliac crest bone biopsy showed no patient with osteoporosis, and mild osteomalacic changes in 1 patient. Slight elevations in appositional rate, osteoid volume, and resorption surface were compatible with a state of high bone turnover. Photon absorptiometry revealed
