A missed opportunity: a retrospective cohort study of alcohol use disorder pharmacotherapy in hospitalized patients.

INTRODUCTION: Alcohol use disorder (AUD) poses a significant public health challenge. Despite the availability of effective pharmacological treatments, their use during hospitalization remains limited. This study aimed to evaluate the prevalence of medication for AUD (MAUD) during inpatient admissions and assess its association with subsequent emergency department (ED) visits and hospital readmissions. MATERIALS AND METHODS: We conducted a retrospective cohort study using electronic health records from Stanford Health Care (SHC) between 2015 and 2023. Hospitalized adults with a documented dia

Medications for alcohol use disorder in alcohol-associated liver disease: The role of gastroenterologist and hepatologist in treatment and outcomes.

BACKGROUND: Alcohol-associated liver disease (ALD) is a major cause of liver-related morbidity and mortality, with alcohol abstinence remaining the cornerstone of management. Pharmacologic therapies for alcohol use disorder (AUD) can support abstinence but remain underutilized in patients with ALD. The impact of Gastroenterology (GI)/hepatology involvement on the use and outcomes of medications for alcohol use disorder (MAUD) remains understudied. METHODS: We conducted a retrospective chart review of patients with ALD and AUD identified by ICD-10 codes at a tertiary care liver transplant cent

Role of nucleus accumbens glutamatergic plasticity in drug addiction.

Substance dependence is characterized by a group of symptoms, according to the Diagnostic and Statistical Manual of Mental Disorders, 4th Edition, Text Revision (DSM-IV-TR). These symptoms include tolerance, withdrawal, drug consumption for alleviating withdrawal, exaggerated consumption beyond original intention, failure to reduce drug consumption, expending a considerable amount of time obtaining or recovering from the substance's effects, disregard of basic aspects of life (for example, family), and maintenance of drug consumption, despite facing adverse consequences. The nucleus accumbens

Ethanol effect on the ionic transfer through isolated human amnion. I. Preventive and antagonistic actions of some nutrients and of their synthetic congeners.

The inhibition of the placental transfer of nutrients was studied in the pathogenesis of the fetal alcohol syndrome. Ethanol reduces the ionic transfer through the human amnion. In this study, the preventive and antagonistic actions of natural nutrients (Mg2+, Ca2+, taurine, homotaurine, gamma-aminobutyric acid) and of their synthetic congeners (Ca2+-taurinate, Ca2+-acetyltaurinate, Ca2+-acetylhomotaurinate) were observed on the amniotic conductance which was modified by ethanol. All the molecules, except Mg, demonstrated protective actions on the fetal side. The effect varied with the molecul

Percentage of subjects with no heavy drinking days: evaluation as an efficacy endpoint for alcohol clinical trials.

BACKGROUND: Percent subjects with no heavy drinking days (PSNHDDs), an efficacy end point recommended by the Food and Drug Administration, considers abstinent individuals or those engaging in low-risk drinking behavior as successful responders to treatment. As PSNHDD has been used infrequently in previous alcohol clinical trials, we evaluated the utility and validity of the PSNHDD outcome measure in 2 large alcohol clinical trials. METHODS: Data sets from 2 alcohol trials, COMBINE and a multisite topiramate trial, were used to analyze PSNHDDs and other traditional end points for the topiramat

Pharmacologic management of metabolic and alcohol-associated liver disease.

Metabolic and alcohol-associated liver disease (MetALD) is an emerging phenotype within the steatotic liver disease spectrum, characterized by cardiometabolic risk factors coexisting with alcohol exposure, resulting in synergistic liver injury and fibrosis progression. Therapeutic development remains limited because most steatotic liver disease trials exclude patients with ongoing alcohol use. In contrast, alcohol-associated liver disease (ALD) trials have focused primarily on severe alcohol-associated hepatitis. Current management of patients with MetALD relies on an integrated approach that

Clinician Prescribing Practices Involving Medications for Alcohol Use Disorder.

INTRODUCTION: Despite the heavy toll of alcohol use disorder (AUD) in the U.S., efficacious medications for AUD (MAUD) are rarely used. Minimal research has explored clinician prescribing practices involving MAUD. METHODS: Using a large national database of electronic health records, this cross-sectional analysis, conducted in 2023-2024, identified clinicians with at least 1 prescription order for an FDA-approved MAUD (naltrexone, acamprosate, or disulfiram) for a patient with AUD during 2016-2021. Descriptive statistics captured clinician-level prescribing volume and type of medication presc

Factors Affecting Ethanol-Induced Conditioned Place Preference and Locomotor Sensitization in Mice.

The rewarding effects of alcohol can lead to progressively heavier and more frequent drinking. Since studies of reward have mainly focused on responses to higher alcohol doses, the relations between reward and moderate/sustained alcohol exposure remain unknown. Our objective was to evaluate factors affecting the reward value of low alcohol doses and risk factors for increasing alcohol doses due to reward progression caused by alcohol exposure patterns. We thus performed conditioned place preference (CPP) and ethanol (EtOH)-induced locomotor sensitization tests in mice. Low-dose EtOH (0.5 or 1

Pharmacotherapy for alcohol dependence: perceived treatment barriers and action strategies among Veterans Health Administration service providers.

Although access to and consideration of pharmacological treatments for alcohol dependence are consensus standards of care, receipt of these medications by patients is generally rare and highly variable across treatment settings. The goal of the present project was to survey and interview the clinicians, managers, and pharmacists affiliated with addiction treatment programs within Veterans Health Administration (VHA) facilities to learn about their perceptions of barriers and facilitators regarding greater and more reliable consideration of pharmacological treatments for alcohol dependence. Novel medication targets for the treatment of alcoholism: preclinical studies.

Alcoholism is a complex heterogeneous disease and a number of neurotransmitter and neuromodulator systems have been implicated in its manifestation. Consequently, it is unlikely that existing medications such as disulfiram (Antabuse®), naltrexone (ReVia®), acamprosate (Campral®)) can be efficacious in every individual. Thus, the development of novel therapeutic agents with greater selectivity and less unwanted effects for the treatment of this disease is one of the major objectives of alcohol research. This review summarizes the findings of five novel compounds with different neuronal targets

Glycine transporter-1 blockade leads to persistently reduced relapse-like alcohol drinking in rats.

BACKGROUND: Residual dysfunction of multiple neurotransmitter systems due to chronic alcohol use is likely responsible for the occurrence of compulsive alcohol seeking during abstinence and relapse behavior. There is increasing evidence that glycine, which activates both glycine and N-methyl-D-aspartate receptors, contributes to excessive alcohol consumption. We therefore hypothesized that the blockade of glycine transporter 1 might interfere with compulsive alcohol consumption and relapse behavior. METHODS: We used our animal model of alcoholism--long-term alcohol consumption with repeated d

Neuropharmacological treatments for alcoholism: scientific basis and clinical findings.

Preclinical studies have exploded our knowledge about the behavioral and biological underpinnings of alcoholism. These studies suggest that certain neurotransmitters, particularly those interacting with the opioid, N-methyl-D-aspartate, and monoamine systems, may play a critical role in the expression of alcohol-drinking and other behaviors associated with its abuse liability. Built upon this foundation, important advances have been made in the development of therapeutic medications for the treatment of alcoholism. Of the medications reviewed, acamprosate's potential appears to be the most wid
