From Approximation to Validation: Rethinking Frequency-Matched Acoustic Models of Cochlear Implants.

OBJECTIVES: This study aims to validate vocoders as acoustic models of cochlear implants by determining whether they capture both perceptual sound quality and speech performance. We hypothesize that valid acoustic models of cochlear implants require listener-specific amounts of frequency mismatch between input filters and output tones or noise bands. DESIGN: Forty-four adult single-sided deaf cochlear implant users were tested 1 to 5 times for a total of 73 sessions at different time points after initial stimulation. Participants had a cochlear implant in one ear and normal or near-normal hea

Production and perception of volitional laughter across social contexts.

Human nonverbal vocalizations such as laughter communicate emotion, motivation, and intent during social interactions. While differences between spontaneous and volitional laughs have been described, little is known about the communicative functions of volitional (voluntary) laughter-a complex signal used across diverse social contexts. Here, we examined whether the acoustic structure of volitional laughter encodes social contextual information recognizable by humans and computers. We asked men and women to produce volitional laughs in eight distinct social contexts ranging from positive (e.g.

Effect of Probe-Tone Frequency on Ipsilateral and Contralateral Electrical Stapedius Reflex Measurement in Children With Cochlear Implants.

OBJECTIVES: The upper loudness limit of electrical stimulation in cochlear implant patients is sometimes set using electrically elicited stapedius reflex thresholds (eSRTs), especially in children for whom reporting skills may be limited. In unilateral cochlear implant patients, eSRT levels are measured typically in the contralateral unimplanted ear because the ability to measure eSRTs in the implanted ear is likely to be limited due to the cochlear implant surgery and consequential changes in middle ear dynamics. This practice is particularly limiting in the case of fitting bilaterally implan

Acoustic-reflex adaptation: morphology and half-life data for subjects with normal hearing.

Acoustic-reflex adaptation was studied in 35 subjects with normal hearing using nine pure-tone activators (250-6000 Hz) and a broadband-noise activator. The individual subject data generated by the 31-s activators presented 10 dB above the reflex threshold were digitized, corrected for baseline drift and ear canal volume, and analyzed in terms of the acoustic-admittance change in acoustic mmhos and in terms of the percentage of maximum-reflex magnitude. Reflex adaptation increased as a function of frequency. The adaptation functions for the lower frequencies (less than or equal to 1500 Hz) wer

Intraoperative continuous monitoring of evoked facial nerve electromyograms in acoustic neuroma surgery.

BACKGROUND: Preservation of facial nerve function is one of the most important goals in acoustic neuroma surgery. We have been using intraoperative continuous monitoring of evoked facial nerve electromyograms (EMGs) since 1997 in acoustic neuroma surgery. We therefore investigated surgically treated patients to clarify the usefulness of this monitoring, and to determine safety criteria for preserving facial nerve function. METHODS: This intraoperative continuous monitoring of evoked facial nerve EMG is a method for checking the EMG evoked by continuous direct electrical stimulation of the fac

Behavioral reactions of harbor porpoises to impact pile driving noise are predicted by the auditory frequency weighted sound pressure level.

Offshore impact pile driving is a major source of high level underwater noise that can disturb marine mammal behavior tens of kilometers away. Projects involving pile driving are therefore subject to environmental impact assessments, which include modelling of the spatial extent of the behavioral disturbance. Reliable predictions about behavioral reaction distances require robust estimates of the minimum received levels of noise above which animals are likely to respond. Studies of reactions of harbor porpoises to pile driving noise in the wild and playback in captivity were identified, and re

Mouse middle-ear forward and reverse acoustics.

The mouse is an important animal model for hearing science. However, our knowledge of the relationship between mouse middle-ear (ME) anatomy and function is limited. The ME not only transmits sound to the cochlea in the forward direction, it also transmits otoacoustic emissions generated in the cochlea to the ear canal (EC) in the reverse direction. Due to experimental limitations, a complete characterization of the mouse ME has not been possible. A fully coupled finite-element model of the mouse EC, ME, and cochlea was developed and calibrated against experimental measurements. Impedances of

On the calculation of reflectance in non-uniform ear canals.

Ear-canal reflectance is useful for quantifying the conductive status of the middle ear because it can be measured non-invasively at a distance from the tympanic membrane. Deriving the ear-canal reflectance requires decomposing the total acoustic pressure into its forward- and reverse-propagating components. This decomposition is conveniently achieved using formulas that involve the input and characteristic impedances of the ear canal. The characteristic impedance is defined as the ratio of sound pressure to volume flow of a propagating wave and, for uniform waveguides, the plane-wave characte

Contralateral suppression of TEOAE. Attempts to find a latency.

Latency of the contralateral suppression of transient-evoked otoacoustic emissions (TEOAE) has been investigated using the ILO88 system for ipsilateral stimulation and recording, and a laboratory computer interface producing contralateral noise bursts. Reduction of rms amplitude in the last 10.24 ms of the response was found to produce the most consistent indication of suppression. Onset latencies varied from less than 40 ms to 140 ms, offset latency from 20 ms to more than 80 ms. Acoustic reflex latencies have not been reported with such short latencies and this makes it unlikely that this re

Differential impact of thalamic versus subthalamic deep brain stimulation on lexical processing.

Roles of subcortical structures in language processing are vague, but, interestingly, basal ganglia and thalamic Deep Brain Stimulation can go along with reduced lexical capacities. To deepen the understanding of this impact, we assessed word processing as a function of thalamic versus subthalamic Deep Brain Stimulation. Ten essential tremor patients treated with thalamic and 14 Parkinson׳s disease patients with subthalamic Deep Brain Stimulation performed an acoustic Lexical Decision Task ON and OFF stimulation. Combined analysis of task performance and event-related potentials allowed the de

Vestibular evoked myogenic potentials in acoustic tumor patients with normal auditory brainstem responses.

In order to clarify the utility of the vestibular evoked myogenic potential (VEMP) in detecting acoustic tumors, we report two patients who were found to have normal auditory brainstem responses (ABRs) and abnormal VEMPs. To record VEMPs, electromyographic responses to brief loud clicks (0.1 ms at 95 dBnHL) were amplified and averaged on the sternocleidomastoid muscle ipsilateral to the stimulated side. The stimulation rate was 5 Hz and the analysis time 50 ms. The first case was a 54-year-old woman in whom VEMPs were absent on the affected side while caloric tests and ABRs were normal. The se

Cochlear implantation as a successful rehabilitation for radiation-induced deafness.

Radiotherapy of the head and neck can be associated with conductive and/or sensori-neural hearing loss. We report the case of a 67-year-old man who developed complete bilateral deafness caused by labyrinthitis and radiation-induced neuritis of the acoustic nerve after postoperative radiotherapy of a nasopharyngeal carcinoma. Two years postoperatively extensive clinical workup including computed tomography and magnetic resonance imaging showed no recurrence or secondary brain tumors. To facilitate sound perception a Combi 40 cochlear implant was implanted. Because of fibrosis the insertion dept

Pointwise encoding time reduction with radial acquisition (PETRA) to pseudo-CT (computer tomography) mapping for precision focused transcranial ultrasound applications.

Ultrasound simulations used for treatment planning in transcranial ultrasound stimulation (TUS) currently depend on converting computed tomography (CT) images to acoustic properties. Aiming to reduce subject exposure to ionising radiation, recent literature focused on generating pseudo-CT images from different magnetic resonance (MR) imaging sequences. Here, the pointwise encoding time reduction with radial acquisition (PETRA) sequence was used because of its stronger ability to image bones. A dataset of paired CT and PETRA scans was acquired separately for human subjects and ex vivo skulls. P

Interaural Time Difference Perception with a Cochlear Implant and a Normal Ear.

Currently there is a growing population of cochlear-implant (CI) users with (near) normal hearing in the non-implanted ear. This configuration is often called SSD (single-sided deafness) CI. The goal of the CI is often to improve spatial perception, so the question raises to what extent SSD CI listeners are sensitive to interaural time differences (ITDs). In a controlled lab setup, sensitivity to ITDs was investigated in 11 SSD CI listeners. The stimuli were 100-pps pulse trains on the CI side and band-limited click trains on the acoustic side. After determining level balance and the delay nee

Amygdaloid modulation of mesopontine peribrachial neuronal activity: implications for arousal.

This research sought to determine if activation of the amygdaloid central nucleus (ACe) modulates the activity of arousal-related neurons within the peribrachial (PB) region. Two categories of neurons were identified. Neurons of the 1st category demonstrated low spontaneous rates and responded with a burst of spikes to acoustic stimuli, characteristics similar to those reported for cholinergic ponto-geniculo-occipital (PGO) wave generator neurons. Neurons of the 2nd category demonstrated significant correlations between their spontaneous activity and the power of delta wave activity in the ele

Physical Modalities for the Treatment of Pain in Patients with Fibromyalgia.

Generalised pain is the major symptom in patients with fibromyalgia. The management of pain includes both pharmacological and non-pharmacological options. Exercise, meditative movement therapies and mindfulness-based stress reduction are examples of non-pharmacological treatments. Over the last decades, there is growing evidence regarding the role of physical modalities in the management of fibromyalgia-related pain. Physical modalities demonstrate their effects by using several energy types such as electrical, thermal, acoustic, or radiant energy. They may act through the alteration of blood

Spinal cord neuromodulation for blood pressure control using low-intensity focused ultrasound.

Focused ultrasound (FUS) is an innovative technology that delivers angled acoustic energy to a small target region. Previous FUS technology has demonstrated efficacy in applications such as tumor destruction, nerve modulation, and drug delivery in the brain. We investigated the effects of low-intensity FUS (LIFU) stimulation on the spinal cord and its ability to regulate mean arterial pressure (MAP). We found that LIFU stimulation on exposed rat spinal cord could modulate MAP, causing a decrease when applied at a lower thoracic level and an increase when applied at a lumbosacral level. We also

Animal Models of Hearing Loss after Cochlear Implantation and Electrical Stimulation.

Many hearing-impaired patients may significantly benefit from the Hybrid or electro-acoustic stimulation (EAS) cochlear implant (CI). However, as much as 30-55% of CI recipients lose residual hearing after implantation and the potential for associated benefits of EAS over traditional electric-only stimulation. The cause of this post-implantation hearing loss may be immediate or delayed and result from several factors, including surgical trauma, electric stimulation, and the foreign body response. Clinical and post-mortem studies have helped identify factors effecting EAS performance. Animal CI…

Down-regulation of otospiralin mRNA in response to acoustic stress in guinea pig.

Noise over-stimulation will induce or influence molecular pathways in the cochlea; one approach to the identification of the components of these pathways in the cochlea is to examine genes and proteins that change following different types and levels of stress. Quantitative reverse transcription polymerase chain reaction provides a method to look at differential expression of genes in the acoustic stress response. By using this technique we have revealed a down-regulation of the level of otospiralin mRNA in the cochlea of guinea pigs after white noise over-stimulation for 2 h at 108 dB SPL. Ot
