Nasal Mucociliary Clearance in Adenoid Hypertrophy and Otitis Media with Effusion.

Mucociliary clearance (MCC), which exists in many systems, is the first defensive mechanism of the human body. Nasal MCC has an important role in transporting the secretions of the nasal cavity and paranasal sinuses along with the trapped inhaled pathogens to the nasopharynx. Physiologic or pathologic situations that effect nasal MCC, such as temperature, humidity, nasal obstruction, allergic rhinitis, chronic infections, etc., lead to impaired MCC and related local or circumjacent system disorders. With this perspective, when a unified airway with a multiple disease principle is considered, i

A case of adenoid cystic carcinoma associated with IgG4-related disease.

INTRODUCTION: Immunoglobulin G4-related disease (IgG4-RD) is an inflammatory condition associated with elevated serum IgG4 levels and tissue infiltration by IgG4-expressing plasma cells. We present a case of adenoid cystic carcinoma (ACC) of the submandibular gland with possible involvement of IgG4-RD. PRESENTATION OF CASE: The patient was a 59-year-old man presenting with a swollen right submandibular gland. Laboratory tests revealed IgG4 levels of 176mg/dl (reference range: 4.8-105). An initial open biopsy for histological diagnosis showed chronic sialadenitis. The region was monitored on a

Impact of passive smoke and/or atopy on adenoid immunoglobulin production in children.

The adenoids are exposed to a wide number and variety of microbes, environmental pollutants, and food antigens. Atopy and passive smoke may significantly affect immune responses, mainly in children. The aim of the present study was to investigate whether passive exposure to tobacco smoke and/or atopy could affect immunoglobulin production by adenoidal lymphocytes in a cohort of children presenting with adenoid hypertrophy. A total of 277 children (151 males and 126 females; median age 5.5 years), with adenoidal hypertrophy requiring adenoidectomy and or adeno-tonsillectomy, were consecutively …

Metachronous adenoid cystic carcinoma in the peripheral lung and at base of the tongue.

Primary lung adenoid cystic carcinoma (ACC) is extremely rare and accounts for approximately 0.1%-0.2% of all lung cancers. ACC of the head and neck has generally been regarded as a slow-growing, low-grade malignancy which has a tendency for local recurrence and frequent distant metastasis. When ACC of the lung is identified, physicians must determine whether it represents distant metastasis or a primary lung cancer. Thyroid transcription factor-1 staining is one of the most useful methods to differentiate primary from metastatic lesions in lung cancer. Herein we report a case of metachronous,

Relation of mean platelet volume with obstructive adenoid hypertrophy in children.

OBJECTIVE: Adenoid hypertrophy (AH) is a common etiology of chronic upper airway obstruction. Upper respiratory tract obstruction may cause chronic alveolar hypoventilation and pulmonary vasoconstriction. In one previous study in patients with obstructive sleep apnea (OSA), it has been claimed that mean platelet volume (MPV), an indicator of platelet activation is increased and that MPV has an important role in the pathophysiology of cardiovascular diseases. We investigated in our study if MPV can be used as an indicator of obstruction due to adenoid hypertrophy. METHODS: Our study includes 6

Evaluation of a German version of the tonsil and adenoid health status instrument.

BACKGROUND: To create and validate a German version of the Tonsil and Adenoid Health Status Instrument (TAHSI) for evaluation of tonsillectomy outcome in adult patients with chronic or recurrent tonsillitis. SUBJECTS AND METHODS: 46 healthy volunteers were assessed twice in a 6 week interval with the TAHSI questionnaire. Their results were compared with 45 patients suffering from chronic tonsillitis before tonsillectomy and 6 months following surgery. For internal consistency, Cronbach's alpha was calculated; to identify normal score values, the optimum cutoff between healthy and diseased ind

Adenoid cystic carcinoma of the skull base mimicking temporomandibular disorder.

BACKGROUND: Temporomandibular disorder (TMD) involves problems of the temporomandibular joint and its adjacent muscular system. Because TMD prevalence is high among Western populations, complaints in these regions are generally attributed to TMD. However, in rare cases, TMD symptoms are mimicked by malignant tumors of the head. CASE REPORT: Upon first presentation, an 18-year-old female complained about typical symptoms of TMD. After an initial splint therapy and physiotherapy, painful symptoms increased significantly. Twelve weeks after initial diagnosis, further diagnostic imaging revealed

Sinonasal adenoid cystic carcinomas: clinical outcomes and predictive factors.

This is a retrospective study of 11 patients treated for sinonasal adenoid cystic carcinomas (ACCs) between 1989 and 2008. The authors statistically analysed the epidemiological, clinical, histological and therapeutic aspects of this series of patients with sinonasal ACCs and their impact in terms of overall and disease-free survival established using the Kaplan-Meier method. A search for prognostic factors was made using a log-rank test. There were 5 women (45.4%) and 6 men (54.6%). Average age at diagnosis was 52.2 years (24-75 years). Mean follow-up of patients was 63 months (11-142 months)

Objective reduction in adenoid tissue after mometasone furoate treatment.

OBJECTIVE: Chronic rhinitis and adenoid hypertrophy are the main causes of nasal obstruction in children and proper treatment of these factors seem essential for controlling nasal obstructive symptoms. This study aims to evaluate the effects of topical mometasone treatment on symptoms and size of adenoid tissue in children with complaints of nasal obstruction and to compare this approach to continuous nasal saline douching plus environmental control alone. METHODS: Fifty-one children with nasal obstructive complaints were submitted to a semi-structured clinical questionnaire on nasal symptoms

An unusual case of adenoid cystic carcinoma with hard palate perforation.

Adenoid cystic carcinoma is a malignant tumor which occurs frequently in hard palate associated with minor salivary glands. The lesion generally presents as a painful slow growing mass and it is characterized by recurrences and distant metastasis resulting in a poor prognosis for the patient. This paper reports an atypical adenoid cystic carcinoma with palatal perforation which occurred in a young woman. Initial diagnostic hypothesis were necrotizing sialometaplasia and lues. Although adenoid cystic carcinoma is common in hard palate, cases with palatal perforation are uncommon and may lead to

[Videofluoroscopic evaluation of adenoid hypertrophy and velopharyngeal closure during speech].

BACKGROUND: Adenoid hypertrophy is a common cause of upper airway obstruction in children. Traditionally, adenoid size has been evaluated using a conventional lateral skull film. Flexible endoscopy is now the gold standard for the evaluation of the upper airway. In small children, this study is not always adequately tolerated. OBJECTIVE: Determine specificity and sensitivity of videofluoroscopy for evaluating adenoid size and velopharyngeal closure during speech, as compared with lateral skull film, using videonasopharyngoscopy as gold standard. METHODS: Prospective study analyzing 70 patien

[Adenoid cystic carcinoma of the head and neck--a 10 years experience].

INTRODUCTION: Adenoid cystic carcinoma (ACC) was first described in 1853 and 1854 by three french scientists, Robin, Lorain and Laboulbène. ACC is a rare cancer of the head and neck (about 1%) and it comprises from 6% to 10% of all malignant tumors of the salivary glands. 15%–30% of these tumors are located in submandibular gland, 30% occur in small salivary glands (in the palate, tongue, mouth, nose, sinuses, mucous membrane of the cheeks) and about 6% in parotid gland. Because of its specificity ACC is a challenge for the doctor – it is usually recognized at an advanced stage, spreads along

Development and characterization of xenograft model systems for adenoid cystic carcinoma.

Adenoid cystic carcinoma (ACC) is one of the most common malignancies to arise in human salivary glands, and it also arises in the glandular tissue of other organ systems. To address the paucity of experimental model systems for this tumor type, we have undertaken a program of transplanting tissue samples of human ACC into immunodeficient nu/nu mice to create xenograft model systems. In 17 of 23 attempts (74%), xenograft tumors were successfully grown. In all cases, the histologic appearance of the donating tumor was recapitulated in the subsequent xenograft. Characterization of a subset of xe
