Topical cidofovir for treatment of resistant viral infections.

Cidofovir is a broad-spectrum antiviral agent with activity against several DNA viruses. An application to approve the use of topical cidofovir for acyclovir-resistant herpes virus infection was denied by the US Food and Drug Administration owing to the lack of phase III clinicla trial data. The manufacturer stated that, as of 1997, no further controlled studies were planned or in progress to evaluate topical cidofovir in herpes simplex infections. It is unknown whether there are plans to conduct controlled studies of topical cidofovir for HPV or MVC infections. However, numerous case reports

Topical therapies for glaucoma: what family physicians need to know.

Medication classes historically used in the management of glaucoma include beta blockers, miotics, sympathomimetics and carbonic anhydrase inhibitors. Because topically applied medications are more site specific, they are preferred in the treatment of glaucoma. Compared with oral medications, topical agents are associated with a decreased incidence of systemic side effects. With topical administration, conjunctival and localized skin allergic reactions are relatively common, whereas severe reactions, including death, are rare. Recently introduced topical agents for glaucoma therapy include dor

Redefining minoxidil therapy for androgenetic alopecia: from advanced drug delivery to precision treatment.

INTRODUCTION: Minoxidil remains a cornerstone of pharmacological therapy for androgenetic alopecia (AGA), yet conventional topical formulations are constrained by pharmaceutical, biological, and patient-related limitations that compromise their real-world efficacy. Meanwhile, the rapid adoption of low-dose oral minoxidil (LDOM) is reshaping the therapeutic landscape and challenging the traditional paradigm of topical drug delivery. AREAS COVERED: This Perspective critically examines the limitations of conventional topical minoxidil, recent advances in topical drug-delivery technologies, and t

Is topical alprostadil an usable and reliable alternative to intracavernous injection for penile dynamic duplex ultrasonography?

No study has yet been done to evaluate topical alprostadil as a less invasive alternative vasoactive agent for Penile Dynamic Duplex Ultrasonography (PDDU) in the diagnosis of erectile dysfunction. The main aim of our study was to evaluate the usability and reliability of topical alprostadil for PDDU compared with standard intracavernous injection. A further objective was to determine the patients' preference between these two different approaches. During session A, patients received injection while during session B, they received topical alprostadil. Each patient underwent both sessions, 1 we

Calcipotriene/betamethasone dipropionate for the treatment of psoriasis vulgaris: an evidence-based review.

While topical medications remain the cornerstone of the psoriasis treatment paradigm, they also come with the risk of multiple side effects. An alternative topical treatment option, calcipotriene or calcipotriol, is a vitamin D derivative that is thought to work by inhibiting keratinocyte proliferation and enhancing keratinocyte differentiation. Multiple studies have demonstrated its efficacy and safety in improving psoriasis when used in combination with topical corticosteroids. Given the effectiveness and side effect profile seen with this combination of topical steroid and calcipotriene, th

Successful treatment with cyclosporin administration for persistent benign migratory glossitis.

We herein describe a 54 year-old female patient with a 5-year history of persistent and painful benign migratory glossitis (BMG), which was remarkably improved by systemic administration of cyclosporin. She had noted some white patches leaving smooth denuded red areas with whitish elevated borders on the dorsum of her tongue, and finally felt strong pain. The lesion was refractory to the previous treatment with topical corticosteroid treatment for the last 2 years. Because clinicopathological findings were compatible with BMG, systemic administration of 20 mg/day prednisolone and topical 0.1%

Real-World Data on Topical Therapies and Annual Health Resource Utilization in Hospitalized Swiss Patients with Ulcerative Colitis.

OBJECTIVES: Topical treatment with aminosalicylates and/or budesonide was shown to be highly effective in patients with ulcerative colitis (UC), while reducing the likelihood of systemic adverse effects. However, previous research has shown that topical treatment is clearly underused. We aimed to evaluate the use of topical therapy in the real-world setting. METHODS: This is an observational study based on claims data of 201 Swiss adult patients who were hospitalized for UC between 2012 and 2014 and who were then followed for 1 year. A variety of factors presumably associated with topical tre

Cutaneous Toxicity in a Laboratory Beagle (Canis lupus familiaris) after Chronic Administration of Doxorubicin Hydrochloride.

An adult female beagle (Canis lupus familiaris) used in a model of doxorubicin-induced cardiomyopathy presented with epithelial desquamation on the shoulders and ventrum after receiving the 8th weekly intravenous dose of the free form of doxorubicin (20 mg/m2; total accumulation, 160 mg/m2). The lesions were empirically treated with topical disinfectants and topical and systemic antibiotics. Despite treatment, the lesions progressed and ulcerated. Bacterial culture revealed Staphylococcus aureus, but trichogram, skin scraping, and fungal culture were negative for microorganisms. Skin biopsies

Review of mupirocin ointment in the treatment of impetigo.

The introduction of mupirocin ointment gives the pediatrician a reliable topical alternative to oral antibiotic therapy for group A streptococcal and staphylococcal impetigo. It is as effective as oral antibiotics and is associated with fewer adverse effects. In superficial skin infections that are not widespread, mupirocin ointment offers several advantages. It is highly active against the most frequent skin pathogens, even those resistant to other antibiotics, and the topical route of administration allows delivery of high drug concentrations to the site of infection.

Metallic taste as a side effect of topical fluorouracil use.

Topical fluorouracil is widely used for the treatment of precancerous and cancerous lesions of the skin. The most common side effect of this medication is localized irritant dermatitis. The authors report a case of dysgeusia with metallic taste as a side effect of this medication. While not previously seen with topical use, this is not an uncommon side effect seen with systemic administration of 5-fluorouracil. The etiology of dysgeusia from chemotherapeutic agents and systemic absorption of fluorouracil is discussed.

Optimal use of topical agents for allergic conjunctivitis.

In recent years, there has been renewed interest in the topical treatment of allergic conjunctivitis, a disorder that is involved in the common red-eye syndrome. Topical treatment for rapid relief of symptoms is most often preferred by the patient, although physicians are more cautious with the choice of an agent that might worsen symptoms due to contact irritation induced by the substance or its preservative. Theoretical advantages of topical over oral administration include a more rapid onset of action, since the active agent is applied directly to the affected tissue, and a reduced potentia

The development of BRL 7660 as an anti-acne agent.

BRL 7660, a 16,16 di-substituted androstene steroid, was initially developed by Beecham Pharmaceuticals as a potential anti-fertility agent for males. It proved inactive in this area, but subsequent studies showed it to be a potent testosterone 5-alpha-reductase inhibitor. It showed little classical steroidal activity but reduced sebum secretion in the rat. In the hamster flank organ, it reduced the stimulant effect of testosterone but not 5-alpha-dihydrotestosterone after topical administration, but had little oral activity. Human studies showed that topical administration significantly reduc

Eye Drop Instillation of the Rac1 Modulator CNF1 Attenuates Retinal Gliosis and Ameliorates Visual Performance in a Rat Model of Hypertensive Retinopathy.

In hypertensive retinopathy, the retinal damage due to high blood pressure is accompanied by increased expression of Glial Fibrillary Acidic Protein (GFAP), which indicates a role of neuroinflammatory processes in such a retinopathy. Proteins belonging to the Rho GTPase family, particularly Rac1, are involved in the activation of Müller glia and in the progression of photoreceptor degeneration, and may thus represent a novel candidate for therapeutic intervention following central nervous system inflammation. In this paper, we have observed that topical administration as eye drops of Cytotoxic
