Transient Global Amnesia After Cerebral Angiography With Iomeprol: A Case Report.

Transient global amnesia is now considered a very rare complication of cerebral angiography. Various etiological mechanisms have been suggested to account for this complication, but no consensus has been reached yet. This case report documents one of the few reported cases of cerebral angiography-related transient global amnesia associated with magnetic resonance imaging (MRI) evidence of unilateral hippocampal ischemia, most probably as a consequence of a transient reduction in regional hippocampal blood flow. However, the possibility of a direct neurotoxic effect of the nonionic contrast med

Transient global amnesia after ablation of the left lateral accessory pathway.

Transient global amnesia (TGA) could be encountered in many situations even during invasive procedures. In ablation therapy for arrhythmia, there was only one reported case in the ablation of premature ventricular beats. We report a 31-year-old man having paroxysmal supraventricular tachycardia who underwent TGA at the end of ablation and recovered quickly after 8-9 hours later. Long-term follow-up showed no neurologic deficits for 8 months.

Bilateral Hippocampal DWI Lesions in Transient Global Amnesia. A Case Report.

Transient global amnesia (TGA) is characterized by anterograde memory disturbance of sudden onset that lasts for 1 to 24 hours. The etiology and pathogenesis of TGA are still uncertain, although an hypoxic-ischemic origin has been recently suggested on the basis of infarct-like diffusion-weighted imaging (DWI) lesions in the hippocampus of some affected individuals. Recent diffusion-weighted imaging (DWI) studies have indicated the presence of focal hyperintensities involving the hippocampus in TGA patients. These lesions detected by DWI, which peak at 48 to 72 hours, are small and punctate (1

Cerebellar Hypoperfusion during Transient Global Amnesia: An MRI and Oculographic Study.

BACKGROUND AND PURPOSE: Transient global amnesia (TGA) is characterized by sudden anterograde and retrograde amnesia lasting for up to 24 hours. Diffusion-weighted magnetic resonance imaging (DWI) in cases of TGA and ischemia demonstrates a high frequency of high signal intensities restricted to the hippocampus, and this has been proposed as an etiology of TGA. The aims of this study were to characterize the DWI and single-photon-emission computed tomography (SPECT) findings during the acute and recovered phases of TGA and to correlate the findings with oculomotor abnormalities. METHODS: Five

Transient global amnesia and brain lesions: new hints into clinical criteria.

BACKGROUND AND PURPOSE: Transient global amnesia (TGA) is the abrupt onset of anterograde amnesia with repetitive queries. Different hypotheses have been considered for its aetiology, but it still remains obscure. In 1990, Hodges and Warlow proposed clinical criteria for TGA. Applying these criteria, clinical studies looked for an unifying theory, considering TGA almost a primary and benign disorder. However, spare descriptions of TGA in patients with brain structural lesions have been reported. The aim of this study was to evaluate the prevalence of brain structural lesions among TGA patients

Evolution of hippocampal CA-1 diffusion lesions in transient global amnesia.

OBJECTIVE: Selective focal MR-Signal (diffusion-) changes in the CA-1 sector of the hippocampus have been described in transient global amnesia (TGA), but the pathophysiological substrate of these lesions is largely unknown. As several imaging and epidemiological findings point to a vascular origin an analysis of the temporal evolution of the hippocampal apparent diffusion coefficient (ADC) changes may offer new understanding of the pathomechanisms of TGA. METHODS: The time course of the ADC of hippocampal DWI lesions in TGA patients was studied using serial 3 T high-resolution MR-imaging wit

New insights into transient global amnesia: recent imaging and clinical findings.

Transient global amnesia (TGA) is one of the most striking syndromes in clinical neurology. Despite several new hypotheses concerning TGA pathogenesis-including psychological disturbances, personality traits, and hypoxic-ischaemic origin associated with venous congestion in memory relevant structures or small vessel changes-there is no consensus about the cause. New imaging techniques, particularly diffusion-weighted imaging, open up new insights into the location of TGA pathology. Studies with dynamic venous duplex sonography confirmed the importance of jugular-vein-valve insufficiency. We re

Episodic memory in transient global amnesia: encoding, storage, or retrieval deficit?

OBJECTIVES: To assess episodic memory (especially anterograde amnesia) during the acute phase of transient global amnesia to differentiate an encoding, a storage, or a retrieval deficit. METHODS: In three patients, whose amnestic episode fulfilled all current criteria for transient global amnesia, a neuropsychological protocol was administered which included a word learning task derived from the Grober and Buschke's procedure. RESULTS: In one patient, the results suggested an encoding deficit, and in two others, a storage deficit. CONCLUSIONS: The encoding/storage impairment concerning ante

Diffusion-weighted MRI characterizes the ischemic lesion in transient global amnesia.

We present a patient with transient global amnesia (TGA) whose diffusion-weighted MRI (DWI) showed increased signal in the splenium of the corpus callosum and in the left parahippocampal gyrus. The absence of high signal on the corresponding apparent diffusion coefficient (ADC) images supports the diagnosis of an acute infarction. This finding provides a temporal relation between cerebral ischemia and infarction in the territory of posterior cerebral artery and in certain cases of TGA. An early means of detecting ischemia in TGA by DWI may influence clinical decisions made in patient evaluatio

Diffusion-weighted MRI in transient global amnesia precipitated by cerebral angiography.

BACKGROUND: Transient global amnesia is a well-described complication of cerebral angiography. Speculation about the pathophysiology exists but is as yet unsubstantiated. Diffusion-weighted MRI is a new imaging technique that is very sensitive in detecting acute ischemia. Its use in the evaluation of transient amnesia precipitated by cerebral angiography has not previously been reported. CASE DESCRIPTION: A 44-year-old man underwent posterior circulation cerebral angiography for the investigation of episodic vertigo. Shortly after completion of the procedure, he was noted to have symptoms of

Transient global amnesia as a postictal state from recurrent partial seizures.

A 60-year-old patient had a 3-year history of recurrent memory disturbances. The video electroencephalographic (EEG) recording of one attack showed that the amnesic state, which clinically resembled transient global amnesia, was characterized by a normal EEG tracing followed by electroclinical complex partial seizures. We suggest that "epileptic" transient global amnesia is postictal in nature.

Transient global amnesia after clioquinol: five personal observations from outside Japan.

Five personal observations of an acute amnestic episode in younger individuals after intake of clioquinol are described together with three observations from the medical literature. In five of these cases the episode began after an unusually large dose, in three after a therapeutic one with a latency of about 24 hours. The clinical aspect closely resembled classical transient global amnesia but the episode after clioquinol lasted longer (24 hours to three days) and a more or less extensive retrograde amnesia persisted permanently. In one patient after three tablets of Mexase a clioquinol conce

Transient global amnesia with transient anosmia: a curious case suggestive of middle cerebral artery occlusion.

BACKGROUND: Transient global amnesia (TGA) is an enigmatic amnestic syndrome and affects people in middle or older age. During an episode of TGA, a person is not able to make new memories, which indicates hippocampal damage. The symptom anosmia may be associated with memory impairment. CASE PRESENTATION: A 70-year-old woman presented to our emergency room with transient spatial memory loss. She also complained of a sudden loss of smell. Magnetic resonance angiography confirmed occlusion of the right middle cerebral artery. DISCUSSION AND CONCLUSION: The mechanism causing the transient anosmi

Transient global amnesia and transient ischemic attack. Natural history, vascular risk factors, and associated conditions.

BACKGROUND AND PURPOSE: The purpose of the present study was to make an attempt to ascertain the etiology of transient global amnesia (TGA), which is still disputed more than 30 years after the first description of this clinical entity. METHODS: In a case-control study, we compared the prevalence of vascular risk factors in 64 TGA patients with 64 first-ever transient ischemic attack (TIA) control subjects and 108 normal community-based control subjects matched for age and sex. We prospectively studied the vascular events and mortality rates of the TGA cases and of the TIA control subjects. T

Cerebrovascular events - from transient ischemic attack and transient global amnesia to transient ischemic attack with MRI-DWI lesions.

AIM OF STUDY: To compare transient ischemic attack (TIA), transient global amnesia (TGA), and transient ischemic attack with lesions found in magnetic resonance imaging/diffusion-weighted imaging (MRI-DWI) scans, in order to find similarities and differences in their clinical picture. CLINICAL RATIONALE FOR STUDY: Magnetic resonance imaging scans account for a substantial part of the financial burden associa-ted with cerebrovascular events. Finding initial clinical features that differentiate transient brain ischemic events will be useful in developing standardized procedures for selecting pa

[A study of the risk factors in transient global amnesia and its differentiation from a transient ischemic attack].

INTRODUCTION: Transient global amnesia (TGA) is a clinical condition that has been described in many studies, but its pathophysiology is not fully understood. In recent years the theory of valvular insufficiency in the jugular vein has been added to the classical hypotheses that link it to migraine, to epilepsy and -the most widely accepted- to transient ischemic attacks (TIA), although the real origin of the condition has still not been determined. PATIENTS AND METHODS: In a retrospective study we compared 131 patients diagnosed with TGA between 1993 and 2004 with 262 patients who were diagn

Serial 7 T MRI demonstrates reversible hippocampal T2 abnormalities in transient global amnesia.

BACKGROUND: Transient global amnesia (TGA) often shows tiny punctate hippocampal CA1 lesions on diffusion-weighted imaging (DWI). Whether these lesions represent truly transient structural alterations remains unsettled. Ultra-high-field 7-tesla (7 T) MRI increases spatial resolution and lesion conspicuity. However, the evolution of hippocampal T2 signal at 7 T from the acute phase through short-term follow-up has not been systematically examined. OBJECTIVE: To determine, using serial 7 T MRI, whether hippocampal T2-weighted imaging (T2WI) abnormalities visible in acute TGA are consistently de
