Vascularized plantar myocutaneous free flap transfer for amputee stump preparation: a proof of concept cadaver study.

Lower extremity amputee stump ulceration, irritation, and pain have been a deterrent to consistent long-term or continuous use of lower extremity prosthetics. This study is the first in a series that hypothesizes that these complications can be minimized through the insetting of a vascularized plantar free flap (VPFF) on the amputee stump. Using three hip disarticulated cadaver specimens, a VPFF was designed, dissected, and implanted on one transfemoral and two transtibial stumps. Using accepted vascular anastomosis techniques, the posterior tibial artery was anastomosed to the distal femoral

Transtibial amputation with plantar flap for congenital deficiency of the tibia.

Disarticulation of the knee has been the preferred treatment for the severe type (Type Ia and Type Ib classification of Jones et al) of congenital deficiency of the tibia because of marked flexion contracture of the knee and loss of quadriceps function. In such cases, the disarticulated stump is often small and poorly covered by soft tissues because of dysplastic femoral condyles and calf muscles. Therefore, stump complications after disarticulation may prevent early aggressive walking exercises and delay independent ambulation. To overcome this problem, a greater weightbearing surface was cre

The effect of adjuvant oxygen therapy on transcutaneous pO2 and healing in the below-knee amputee.

The effects on tissue oxygenation of postoperative adjuvant oxygen have been studied in a group of 20 patients undergoing below-knee (BK) amputation for vascular disease. Ten patients received no therapy, the remainder receiving 28% oxygen for 48 hours following surgery. The results showed that the transcutaneous pO2 in the amputation flaps fell significantly by some 20 mmHg (p less than 0.01) following surgery and that this fall was prevented by the use of adjuvant oxygen. The fall was not observed in the non-amputated limbs. TcpO2 took almost two weeks to reach its pre-operative levels in th

Bilateral Finger Morton Neuroma after Primary Fingertip Amputation.

Morton's neuroma (MN) is a painful, proliferative fibrosis of perineural tissue caused by pressure or recurrent irritation that was previously thought to affect only the common interdigital nerve of the foot. We present a case of a bilateral MN of both the ulnar and radial proper digital nerves of the fifth finger in a 30-year-old patient, following multiple surgeries on the aforementioned finger. The patient was referred to our center by a peripheral hospital after traumatic fingertip amputation distal the DIP joint, where the fingertip was initially simply sutured. Because of progressive neu

An assessment of the relationship between congenital transverse deficiency of the forearm and symbrachydactyly.

PURPOSE: A relationship between symbrachydactyly and transverse deficiency has been suggested but has not been critically investigated or es … l radius and ulna. RESULTS: Two hundred seven patients had soft tissue nubbins at the end of their amputation stumps including 38 with the additional finding of skin invagination at the distal end. Another 36 extremities had a skin invagination alone. Twenty-nine of the extremities without nubbins or skin invaginations had hypoplasia of the proximal radius and uln …

In-situ collagen injection in postamputation symptomatic neuroma: A case report.

RATIONALE: Postamputation pain remains a significant clinical challenge, affecting a substantial proportion of individuals with limb loss. Among its etiologies, symptomatic neuromas-resulting from aberrant peripheral nerve regeneration-are a common source of residual limb pain. These lesions often lead to severe discomfort and a considerable decline in the quality of life. Although surgical treatments are available, they are often invasive and carry a risk of complications, especially in elderly or frail patients. Nonsurgical interventions, such as steroid injections, provide temporary relief

The use of conformal lattice metamaterials for relieving stress in lower limb sockets: A numerical and exploratory study.

Selecting the correct material for each application has always been important. Now, with lattice metamaterials engineers can take advantage of the properties of these metamaterials to best suit a specific application. This paper investigates transtibial lower limb socket stress reduction through the implementation of conformal lattice metamaterials. In this work, a model was obtained with a 3D scanner from a plaster cast taken from a participant with a trans-tibial amputation. Then a 3D socket model was created and two conformal patterns were added to the surface of the socket using nTopology®

Traditional Neuroma Management.

With the development of newer techniques for symptomatic neuroma treatment, such as regenerative peripheral nerve interface and targeted muscle reinnervation, transposition and coverage techniques often have been referred to as "passive techniques." In spite of its negative connotation, these passive techniques yield positive results in a majority of patients treated. The experienced surgeon has more options than ever before in the prevention and management of problematic neuromas. Critical appraisal of the current literature reveals no single, optimal standard of care. Instead, surgeons have

Incorporating a Ferrous Polymer Target into Elastomeric Liners for Socket Fit Sensing in Prosthesis Users.

Liner-to-socket distance measurement using inductive sensing may be an effective means to continuously monitor socket fit in people using trans-tibial prostheses. A practical limitation, however, is a means to incorporate a thin uniform-thickness layer of conductive or magnetically permeable target material into the wide range of prosthetic liner products that people with limb amputation commonly use. In this paper, a method is presented whereby a 0.50-mm thickness ferrous polymer made from a SEEPS polymer and iron powder that is formed adjacent to a 0.25-mm thick non-ferrous layer of SEEPS po

Our experience with modified osteomyoplasty for reamputation of war-related transtibial amputees.

The aim of this study was to examine the efficacy of the operative method of modified osteomyoplasty in relation to primary "guillotine" amputation in Croatian war amputees. From 1995 to 2000 a total of 425 patients underwent transtibial amputation for their war-related injuries. From these group, 52 patients were selected that were unable to tolerate prosthetic wear. Following primary amputation, these 52 patients underwent reamputation surgery using the technique of modified osteomyoplasty. The efficacy of the operative method was assessed in relation to age, presence of complications before

[MRI and leg stump neuroma].

OBJECTIVE: To describe RMI aspects of leg stump neuroma and to evaluate RMI scan interest for neuroma diagnosis and management. POPULATION AND METHOD: During a 2 years period, 224 amputated patients consulting for pain or prostetics problems were studied. In 10 cases, a characteristic pain leads to neurona diagnosis. This is described as a sensation of ascending or descending electric shock induced by the stimulation of an identified point with a reproducible topography. In all these cases, RMI scans were performed. In thirty two other cases, a RMI scan was performed to confirm a pathology (b

A standardised trans-tibial amputation method following chronic occlusive arterial disease.

The histo- and biochemical parameters of leg muscles from patients with chronic occlusive arterial disease were examined. The outcome of these tests indicated that it is not possible visually to determine the amputation level accurately at the time of surgery. These test results therefore encouraged the authors to develop a standardised surgical procedure for trans-tibial amputations. With this standardised technique specific musculature is resected to assure that no pathological tissues remain. This surgical prophylaxis is meant to prevent the development of gas gangrene and thus to obtain pr

Survival rates in dysvascular lower limb amputees.

BACKGROUND: To assess the 5-year and 10-year survival rates of major (above ankle disarticulation level) dysvascular lower limb amputees attending a sub-regional Disablement Services Centre (DSC) specialising in amputee rehabilitation. Also to investigate the association between survival rates, cause of dysvascularity, level of amputation, smoking status and occupational status. SETTING: The study was undertaken in sub-regional DSC for amputee rehabilitation covering a base population of about 3.5million people. Over 80% of lower limb amputations were done for dysvascularity (peripheral vascu

Muscle blood flow after amputation. Increased flow with medullary plugging.

At below-knee amputation for arterial insufficiency in 31 patients, the muscle blood flow of quadriceps and triceps surae was measured by clearance of 99mTc pertechnetate pre- and postoperatively. In 15 patients, myoplastic amputation was performed and in 16 patients the medullary cavity of the tibial stump was plugged with cortex of the removed bone as well. Plugging caused a two-third increase in muscle blood flow.

[Prosthetic reconstruction in high amputations of the upper extremity].

BACKGROUND: Conventional upper arm prostheses are controlled via two surface electrodes that measure motor activity of two separately innervated muscle groups. The various prosthetic joints are chosen by co-contractions and controlled linearly by these two muscles. A harmonious and natural course of movements is not possible in this way. OBJECTIVES: Overview regarding surgical, therapeutical and prosthetic options in high amputations of the upper extremity. METHODS: Selective literature research including the authors' own experience in everyday clinical practice as well as a review of medica

Prosthetic programme after above-knee amputation in children with sarcomata.

A programme for early mobilisation using a temporary prosthesis was evaluated in 17 children who had had an amputation above the knee for sarcomata. The temporary prosthesis had a performed adjustable polypropylene quadrilateral socket which was able to accommodate changes in the size of the stump during the first few months after amputation. The adjustable sockets were assembled onto wooden knee-shank-foot units or onto modular components covered with foam. The wooden units were better for routine use as more adjustment was possible between the socket and the knee and because they were more d
