We report a case of anaphylaxis in a 35+5 week of pregnancy patient who came to the Emergency Room with shortness of breath, hypotension and loss on fetal wellbeing. Due to her medical history and given the clinical picture at that time, an anaphylactic shock was suggested as the most probable diagnose. The administration of dexchlorpheniramine and methylprednisolone resulted in an immediate and positive reaction. Simultaneously, an improvement in the fetus cardiotocographic record was objectified. Flights departing from malarious areas are sprayed with pyrethroids. They are presumed to be safe since reports of adverse responses among passengers or crew were only anecdotal. Benzalkonium chloride (BAC) is commonly used as a bactericidal preservative in nebulizer solutions, and can cause paradoxical onchoconstriction following nebulizing therapy in some asthmatics. We describe a case of anaphylactic shock in a 23-yr-old asthmatic woman following an intradermal skin test with a salbutamol solution containing BAC. Since she complained of cough and dyspnea after inhalation therapy with a nebulizer solution, we conducted an intradermal skin test using the same solution, which contained BAC. About 10 min later, the patient reported dizziness, palpitations, and dyspnea.

A recent change from Haemaccel to Gelofusin as the preferred colloid for resuscitation in our region caused us to review patients who were known to be allergic to Haemaccel. As Gelofusin and Haemaccel are both modified gelatine, it seemed likely that cross reactivity could occur. A great number of parasites have been reported in fish, but only a few of them are capable of infecting human beings. Anisakiasis or anisakidosis is caused by sea nematodes of the genus Anisakis, with the main implicated species being Anisakis simplex. Infection with Anisakis causes a wide spectrum of clinical manifestations, ranging from symptoms related to the upper and occasionally lower digestive tract to allergic manifestations, mainly urticaria and anaphylaxis. We report a case of asymptomatic gastroduodenal anisakiasis presenting as severe anaphylaxis.

The incidence of the hydatid disease is still high in some regions of the Mediterranean. A serious, but rare, peroperative complication is IgE-mediated anaphylaxis, as a result of passing of the highly antigenic hydatid cyst content into the bloodstream. Elevations in tryptase, a recently discovered mast cell enzyme, have been proposed as a postmortem indicator of fatal anaphylaxis. The previous studies had limited numbers of controls and thus the specificity of the test with postmortem samples was not known. Therefore, tryptase was evaluated in postmortem blood samples from 49 autopsy cases where there was no evidence of fatal anaphylaxis. Chlorothalonil, a common fungicide used for agricultural and horticultural purposes is capable of eliciting contact urticaria. In our patient, an anaphylactoid reaction resulted from intact skin contact to diluted chlorothalonil (0.01 percent aqueous). Our patient also experienced immediate respiratory reactions such as a tight chest and throat after entering the nursery greenhouse, without direct skin contact with chlorothalonil.
