Immune hemolytic anemia due to a drug-adsorption mechanism has been described primarily in patients receiving penicillins and first-generation cephalosporins. We describe a patient who developed anemia while receiving intravenous cefotetan. Cefotetan-dependent antibodies were detected in the patient's serum and in an eluate prepared from his red blood cells. The eluate also reacted weakly with red blood cells in the absence of cefotetan, suggesting the concomitant formation of warm-reactive autoantibodies. We report a case of carbamazepine-induced aplastic anemia. After an initial treatment period of 10 days (600 mg/day), moderate, persisting pancytopenia was observed. Three years later, carbamazepine treatment was resumed at 400 mg/day for 120 days. Subsequently, more severe, irreversible pancytopenia due to bone marrow aplasia was diagnosed. History, course, bone marrow findings and the extensive, negative work-up for other causes of aplastic anemia strongly indicate that carbamazepine was etiologically responsible. Numerous toxic exposures have been implicated in causing aplastic anemia. Sickle-cell anemia is a common disease, affecting more than 50,000 blacks in the United States. Since 1970 the morbidity and mortality have improved, with patients surviving well into their fourth decade. This article discusses the spectrum of serious complications of sickle-cell anemia. Crises and complications that result from the sickling process are presented with recommendations for emergency department evaluation and management. The painful bone crisis and pain control are also discussed.

Glossitis and glossodynia are commonly seen by the dental practitioner. The awareness that these symptoms in conjunction with other oral signs indicate pernicious anemia is important for the clinician. Correct diagnosis can be made, and neurological complications avoided. Two case reports are presented and typical oral and systemic features of pernicious anemia are discussed.

Even when simple iron deficiency is strongly suspected, the child with severe anemia deserves a careful and thorough work-up before treatment is begun. Many other possible causes of anemia need to be excluded. No one test specifically identifies iron deficiency, but the combined use of several tests of iron status can establish iron deficiency with certainty. Hemoglobin level, mean corpuscular volume, serum iron, total iron binding capacity, free erythrocyte porphyrin and serum ferritin are the important studies.

We have described a patient with thymoma, hypogammaglobulinemia, pernicious anemia, and malignant schwannoma. Review of the literature yielded only one other case of the triad of thymoma, hypogammaglobulinemia, and pernicious anemia. We propose that autoimmunity was the common etiology for the coexistence of thymoma, hypogammaglobulinemia, and pernicious anemia in our patient.

Aplastic anemia is characterized by decreased bone marrow function with inadequate production of erythrocytes, granulocytes, and platelets. The short- and long-term effect of multiple versus regularly spaced blood transfusions on cardiac size, electrocardiographic alterations, heart sounds, murmurs, and carotid pulse were studied in 18 children with Cooley's anemia. An operation and anesthesia for patients suffering from true sickle-cell anemia, carries a substantially increased risk for these patients. This article describes the implications for the anesthetist and the measures to be taken during the phases the patient is in his care.
