[Intermittent loss of consciousness during cesarean section under spinal anesthesia: a case report].

Loss of consciousness during spinal anesthesia is a rare but scary complication. This complication is generally related to severe hypotension and bradycardia, but in this case, the loss of consciousness occurred in a hemodynamically stable parturient patient. We present a 31 years-old patient who underwent an emergency cesarean section. She lost consciousness and had apnea that started 10 minutes after successful spinal anesthesia and repeated three times for a total of 25 minutes, despite the stable hemodynamics of the patient. The case was considered a subdural block, and the patient was pro

Bupivacaine vs. Pethidine in Spinal Anesthesia of Old Patients: Haemodynamic Changes and Complications.

OBJECTIVE: Organ changes during the aging are one of the signifi cant events in old patients. rthopedic surgeries are common operations in these patients that accompany with hemodynamic changes as blood pressure decrease, heart rate, and respiratory rate change. On the other hand, pain management of the ancient patients due to negative consequences as tachycardia, blood pressure increase, and myocardial ischemia is vital. Various types of opioid analgesics have been used in order to pain control in these patients. We decided to conduct the present study in order to compare the hemodynamic chan

Intraoperative total spinal anesthesia as a complication of posterior percutaneous endoscopic cervical discectomy.

PURPOSE: Percutaneous endoscopic cervical discectomy (PECD) is an emerging surgical treatment for cervical disc herniation in recent years, but the complications of PECD are rarely reported. In this case report, we aimed to report two cases of total spinal anesthesia, an unusual complication of PECD. METHODS AND RESULTS: One patient became unconscious with unstable vital signs and another patient appeared with numbness of both arms and legs during PECD operation. CONCLUSIONS: From these two cases, we concluded that the operation of anchoring during PECD should be cautious and standardized, i

Intraocular Pressure Changes After Spinal Anesthesia--Acute and Subacute Effects on Surgery Patients.

BACKGROUND: Spinal anesthesia has become appropriate for many surgical procedures. Neuraxial anesthesia can result in acute and late complications like hypotension, bradycardia, local anesthetic toxicity, postdural-puncture headache, backache and spinal nerve damage. Although the body's physiological responses to spinal anesthesia are well understood, its effects on intraocular pressure (IOP) haven't been mentioned before. OBJECTIVES: The aim of this study was to investigate the effects of spinal anesthesia on IOP. MATERIAL AND METHODS: Forty patients receiving spinal anesthesia for subumbli

Conventional Landmark-Guided Midline Versus Preprocedure Ultrasound-Guided Paramedian Techniques in Spinal Anesthesia.

BACKGROUND: Multiple passes and attempts while administering spinal anesthesia are associated with a greater incidence of postdural puncture headache, paraesthesia, and spinal hematoma. We hypothesized that the routine use of a preprocedural ultrasound-guided paramedian technique for spinal anesthesia would reduce the number of passes required to achieve entry into the subarachnoid space when compared with the conventional landmark-guided midline approach. METHODS: One hundred consenting patients scheduled for elective total joint replacements (hip and knee) were randomized into group C (conv

[Spinal anesthesia in high-volume, low-concentration technique for Caesarean sections : Retrospective analysis].

BACKGROUND: Nowadays Caesarean sections are mainly undertaken using spinal anesthesia; therefore, it is important to minimize potential side effects and risks associated with this technique. Currently, many studies have been conducted to optimize the dose of local anesthetics to avoid hypotension, which often occurs during spinal anesthesia. AIM: In a retrospective study design the high-volume, low-concentration technique with up to 12 ml isobaric bupivacain 0.1% (1 mg/ml) and sufentanil (1 µg/ml), which has been used at the University Hospital Würzburg for many years was analyzed with respec

Comparison of percutaneous nephrolithotomy under spinal versus general anesthesia: a randomized clinical trial.

PURPOSE: To evaluate the safety and efficacy of spinal anesthesia compared with general anesthesia in patients who underwent percutaneous nephrolithotomy (PCNL). PATIENTS AND METHODS: One hundred patients with American Society of Anesthesiologists (ASA) score <3 were randomly divided into two groups according to the type of anesthesia. Spinal anesthesia was performed using an injection of 0.25 mg/kg bupivacaine 0.5% in the intrathecal space; no opium (fentanyl) agent was used. All procedures were performed with the patient in the prone position. Stone access was made by using fluoroscopic gui

Spinal cord injury: a rare complication following thoracic epidural anesthesia for percutaneous nephrolithotomy.

Herein, we report a rare instance of paraplegia following percutaneous nephrolithotomy under thoracic epidural anesthesia in a conscious patient. The possible factors include low body mass index, chronic renal failure, and multiple "in and out" needle passes during the procedure. Direct cord trauma with epidural bloody collection resulted in the neurological damage. However, the patient did not show signs of spinal cord trauma during the needle injury, possibly due to a prior accidental subarachnoid block. This not only delayed the recognition of the disorder, but also delayed treatment, conse

Spinal anesthesia in a child with Brachmann-de Lange (Cornelia de Lange) syndrome.

Providing anesthesia to patients with Brachmann-de Lange syndrome (BdLS) may be challenging, mainly because of intubation difficulty, gastroesophageal reflux disease, and aspiration complications. The use of spinal anesthesia (SA) in this population has not been reported. We report the uneventful administration of awake SA to a 7-month-old girl with BdLS who was scheduled for rectal biopsy. The current literature is reviewed to discuss the indications for SA in those patients.

Effect of tetracaine-induced spinal anesthesia on pial microcirculation in pentobarbital anesthetized rats.

Local anesthetic is administrated intrathecally to produce spinal anesthesia. This study examines the effect of spinal anesthesia induced by intrathecal tetracaine on cerebral pial microcirculation in rats. We monitored changes in the mean arterial pressure, internal diameter (ID) of the pial arteriole, intracranial pressure, and red blood cell velocity. The regional cerebral blood flow was calculated from the product of cross sectional area and red blood cell velocity. To induce spinal anesthesia, tetracaine was administered via a polyethylene tube at the L4-5 intervertebral space after lamin

Cauda equina syndrome and profound hearing loss after spinal anesthesia with isobaric bupivacaine.

A 33-yr-old man undergoing anorectal surgery developed cauda equina syndrome and bilateral profound hearing loss after single-injection spinal anesthesia with isobaric bupivacaine. There was no pain on needle placement. Neurologic assessment found impaired sensation to pinprick in the perineal region, lower extremity paralysis, and bowel and bladder incontinence. In addition, he developed a bilateral profound hearing loss involving the low frequencies, with the left side more affected than the right side. Although hearing impairment can occur with cerebrospinal fluid leakage, the etiology of c

[Labor management using epidural anesthesia in a gravida patient with high spinal injury].

Since autonomic hyperreflexia (AH) is a serious complication during labor in a gravida with spinal cord injury, anesthetic measures should be taken for the suppression of AH even in a sensory-loss condition. Several reports have described various methods for the suppression of AH, in which epidural anesthesia has been advocated as a useful means for the prevention or amelioration of AH. However, it is difficult to evaluate the efficacy of epidural anesthesia due to the lack of sensory and motor functions. We report a primipara who had spinal cord injury below the T 3 level at the age of 17 due

[Continuous spinal anesthesia for femoral fracture in two patients with severe aortic stenosis].

Neuraxial blockade is usually not recommended in patients with aortic stenosis. However, neuroaxial blockade techniques such as continuous spinal or epidural anaesthesia can be tailored to minimize potentially dramatic consequences of decrease in systemic vascular resistance, often encountered after standard single shot spinal anaesthesia. We report the cases of two severe aortic stenosis patients (aortic valve area<0.5 cm2) that underwent hip surgery under continuous spinal anaesthesia. Small doses of isobaric 0.25% bupivacaine titrated to limit total dose below 5 mg, injected through the int

Eye problem following foot surgery--abducens palsy as a complication of spinal anesthesia.

BACKGROUND: Paralysis of abducens nerve is a very rare complication of lumbar puncture, which is a common procedure most often used for diagnostic and anesthetic purposes. CASE REPORT: A 38-year-old man underwent surgery for a left hallux valgus while he was under spinal anesthesia. On the first postoperative day, the patient experienced a severe headache that did not respond to standard nonsteroidal anti-inflammatory medication and hydration. During the second postoperative day, nausea and vomiting occurred. On the fourth postoperative day, nausea ceased completely but the patient complained

[Secondary cranial extension after spinal anesthesia with isobaric 0.5% bupivacaine following postural change].

Inadvertent cranial extension of sympathetic and sensory block following posture change during spinal anaesthesia has been reported for isobaric as well as for hyperbaric local anaesthetics. We present the case of a patient who underwent surgical repair of a refracture of the tibia under spinal anaesthesia with 17.5 mg of isobaric 0.5% bupivacaine. The maximum level of sensory block (MLSB) reached T8 after 15 min. Following posture change into a 15 degrees anti-Trendelenburg position 35 min after lumbar puncture, the MLSB increased cranially for 10 segments and reached the C6 level after 10 mi

[Persistent neurological deficit and adhesive arachnoiditis following spinal anesthesia with bupivacaine containing preservatives].

A 75 year-old female with osteoarthritis of bilateral knee joints was scheduled for right total knee replacement. Her medical history included coronary artery disease, bronchial asthma, and previous surgery of lumbar laminectomy, but she had no neurological deficit before the operation. A 22-gauge spinal needle was inserted at the L 4-5 level and 4 ml of 0.5% bupivacaine with preservatives (Marcain 0.5%) was administered. The sensory block level was noted to L 1 and motor blockade of bilateral feet was achieved within 10 minutes. Anesthesia and operative courses were uneventful for 1.5 hours,

[Circulatory reactions under spinal anesthesia. The catheter technique versus the single dose procedure].

UNLABELLED: Life-threatening cardiovascular complications are a serious risk even for healthy patients during spinal/epidural anaesthesia. The incidence of fatal cardiovascular complications for epidural anaesthesia is 1:10000, for spinal anaesthesia 1:7000. In contrast, general anaesthesia has an overall mortality of only 1:28000. Administration of IV fluids to minimise the haemodynaemic reactions of beginning sympatholysis is not always sufficient. In this study, we examined whether fractionated application of local anaesthetics via a spinal catheter would provide better haemodynamic stabili
