Extracorporeal Treatment for Chloroquine, Hydroxychloroquine, and Quinine Poisoning: Systematic Review and Recommendations from the EXTRIP Workgroup.

BACKGROUND: Although chloroquine, hydroxychloroquine, and quinine are used for a range of medical conditions, recent research suggested a potential role in treating COVID-19. The resultant increase in prescribing was accompanied by an increase in adverse events, including severe toxicity and death. The Extracorporeal Treatments in Poisoning (EXTRIP) workgroup sought to determine the effect of and indications for extracorporeal treatments in cases of poisoning with these drugs. METHODS: We conducted systematic reviews of the literature, screened studies, extracted data, and summarized findings

Acute human self-poisoning with the N-phenylpyrazole insecticide fipronil--a GABAA-gated chloride channel blocker.

OBJECTIVE: Fipronil, a broad spectrum N-phenylpyrazole insecticide that inhibits GABAA-gated chloride channels, has been in use since the mid-1990s. A high affinity for insect compared to mammalian GABA receptors results in lower animal toxicity than other insecticides blocking this channel. To date, only two accidental cases of fipronil poisoning in humans have been published. CASE SERIES: We report seven patients with fipronil self-poisoning seen prospectively in Sri Lanka together with pharmacokinetics for four patients. Non-sustained generalized tonic-clonic seizures were seen in two pati

Clinical toxinology--where are we now?

Clinical toxinology encompasses a broad range of medical conditions resulting from envenomation by venomous terrestrial and marine organisms, and also poisoning from ingestion of animal and plant toxins. Toxin-related disease is an important cause of morbidity and mortality worldwide, particularly in the tropical and subtropical continents. Snake bite is the single most important toxin-related disease, causing substantial mortality in many parts of Africa, Asia, and the Americas. The most important snake families are Viperidae and Elapidae, causing a range of clinical effects including local n

Cyanide poisoning: pathophysiology and treatment recommendations.

This paper aims to assess and compare currently available antidotes for cyanide poisoning. Such evaluation, however, is difficult. Thus, extrapolation from the results of animal studies has potential pitfalls, as significant inter-species differences in response may exist, and these experiments often involve administration of toxin and antidote almost simultaneously, rather than incorporating a more realistic time delay before initiation of treatment. Direct inference from human case reports is also problematic; either because of uncertainties over the exposure levels involved (and hence the l

Methanol and ethylene glycol poisonings. Mechanism of toxicity, clinical course, diagnosis and treatment.

Methanol and ethylene glycol poisonings share many characteristics both clinically and biochemically. Both alcohols are metabolised via alcohol dehydrogenase to their toxic metabolites. Methanol is slowly metabolised to formaldehyde which is rapidly metabolised to formate, the metabolite mainly responsible for methanol toxicity. Formate metabolism depends upon the folate pool which is small in primates compared with other animals. Therefore, formate accumulates in primates during methanol intoxication and is mainly responsible for the metabolic acidosis in the early stage of intoxication. In l

Ethylene dibromide poisoning with acute renal failure: first reported case with non-fatal outcome.

Ethylene-di-bromide is a popular fumigant. Delayed long-term effects of this chemical include carcinogenicity and genotoxicity in animals. Only a few cases of acute ethylene-di-bromide toxicity have been reported and all of them have resulted in a fatal outcome. We report the first case of ethylene-di-bromide poisoning presenting with acute renal failure and toxic hepatitis that was managed successfully.

Studies of amygdalin (laetrile) toxicity in rodents.

Amygdalin (laetrile), given to Fischer 344 rats in doses of 250, 500, and 750 mg/kg intraperitoneally daily for five days, caused mortalities of 30.8% 44.1%, and 56.8%, respectively. The mode of death and the elevated serum cyanide levels in the dying animals strongly suggested cyanide poisoning as the cause of death. These findings seriously question the use of amygdalin in clinical medicine under any circumstances.

[Selenium toxicity in domestic animals].

The earliest written report of selenium poisoning is thought to be the description by Marco Polo of a necrotic hoof disease of horses that occurred in China in 13. century. However recognition of Se as toxic principle come in the early 1930s. Severity of Se poisoning depends on chemical forms of the element, species of animals and routes of administration. The soluble Se salts (Na2SeO3 and Na2SeO4) appear to be among the more toxic compounds; the Se inherent in grains and selenoamino acids (selenomethionine and selenocystine) appear to have relative moderate toxicity; the poorly soluble forms

[Ciguatera fish poisoning in New Caledonia. Clinical and epidemiological aspects (author's transl)].

The author studies the incidence of ciguatera fish poisoning on the public health in New Caledonia. He gives the results of a large scaled inquiry on the clinical and epidemiological aspects of the disease. The geographical distribution and the toxic species are discussed. It does appear that, though being not an acute medical problem, ciguatera is an important cause of morbidity in some tribus of the Pines and Loyalty Islands primarily, where the reef fish is the chief animal proteic resource.

Description of Acute and Chronic Cases of Poisoning by Oxalis pes-caprae.

Oxalis pes-caprae is one of the most widespread invasive plants in the Mediterranean areas, especially in central and southern Italy, Sicily, and Sardinia. It is an herbaceous species of South African origin, very common in uncultivated areas, in meadows, and at the edges of roads but also in vegetable gardens and fields, where it can become a weed. Its negative impact on the economic sector is significant due to the presence of large quantities of oxalates, which are toxic and dangerous for grazing livestock; the ingestion of high amounts of oxalates causes the accumulation of oxalate crystal

The role of pesticide exposure in the genesis of Parkinson's disease: epidemiological studies and experimental data.

Parkinsons' disease (PD) is the most common neurodegenerative movement disorder that is a consequence of premature death of dopamine-containing neurons in the substantia nigra. A number of observations have led to the hypothesis that environmental factors, including pesticides, play a significant role in the development of PD. Among pesticides, most commonly herbicides (paraquat in particular) and insecticides have been considered. The aim of this study is to address the uncertainties provided by epidemiological studies on the role of pesticide exposures in the development of PD, with the help

Isocapnic hyperventilation increases carbon monoxide elimination and oxygen delivery.

Hyperventilation with mixtures of O2 and CO2 has long been known to enhance carbon monoxide (CO) elimination at low HbCO levels in animals and humans. The effect of this therapy on oxygen delivery (DO2) has not been studied. Isocapnic hyperventilation utilizing mechanical ventilation may decrease cardiac output and therefore decrease DO2 while increasing CO elimination. We studied the effects of isocapnic hyperventilation on five adult mechanically ventilated sheep exposed to multiple episodes of severe CO poisoning. Five ventilatory patterns were studied: baseline minute ventilation (RR. VT),

Use of intravenous lipid emulsions in drug-induced toxicities: a 2025 narrative review.

Intravenous lipid emulsions (ILE) were first proposed in 1998 as a treatment for bupivacaine-induced cardiac arrest. Since then, their use has expanded to include poisonings by various lipophilic drugs such as tricyclic antidepressants, calcium channel blockers, and antipsychotics. This 2025 narrative review explores the evolving pathophysiological mechanisms of ILE therapy, including the lipid sink and lipid shuttle theories, as well as non-scavenging cardiotonic effects such as membrane stabilization, mitochondrial support, and modulation of vascular tone. It summarizes recent findings from

Allopurinol reduces severity of delayed neurologic sequelae in experimental carbon monoxide toxicity in rats.

Approximately half of those who survive severe carbon monoxide (CO) poisoning develop delayed neurologic sequelae. Growing evidence supports the crucial role of free radicals in delayed brain injury associated with CO toxicity. Xanthine oxidase (XO) has been reported to play a pivotal role in the generation of reactive oxygen species (ROS) in CO poisoning. A recent report indicates that allopurinol both attenuated oxidative stress and possessed anti-inflammatory properties in an animal model of acute liver failure. In this study, we aimed to explore the potential of allopurinol to reduce the s

Ecological and toxicological effects of inorganic nitrogen pollution in aquatic ecosystems: A global assessment.

We provide a global assessment, with detailed multi-scale data, of the ecological and toxicological effects generated by inorganic nitrogen pollution in aquatic ecosystems. Our synthesis of the published scientific literature shows three major environmental problems: (1) it can increase the concentration of hydrogen ions in freshwater ecosystems without much acid-neutralizing capacity, resulting in acidification of those systems; (2) it can stimulate or enhance the development, maintenance and proliferation of primary producers, resulting in eutrophication of aquatic ecosystems; (3) it can rea

A review of the metabolism and toxicology of nickel.

Although nickel is an essential element for animal nutrition, the physiological role of nickel is not yet established. Pathological alterations of nickel metabolism are recognized in several human diseases. The diverse clinical manifestations of nickel toxicology include: (1) acute pneumonitis from inhalation of nickel carbonyl, (2) chronic rhinitis and sinusitis from inhalation of nickel aerosols, (3) cancers of nasal cavities and lungs in nickel workers, and (4) dermatitis and other hypersensitive reactions from cutaneous and parenteral exposures to nickel alloys. The toxicity, carcinogenici

DMTS is an effective treatment in both inhalation and injection models for cyanide poisoning using unanesthetized mice.

CONTEXT: Cyanide (CN) is a metabolic poison, halting ATP synthesis by inhibiting complex IV of the electron transport chain. If exposed at high enough concentrations, humans and most animals can die within minutes. Because time is a crucial factor in survival of CN poisoning, a rapidly bioavailable, nontoxic, easy to administer CN medical countermeasure could improve morbidity/mortality in a mass CN exposure scenario. The most likely route of exposure to CN is via inhalation. OBJECTIVE: This study examined the efficacy of a new formulation for dimethyl trisulfide (DMTS), a countermeasure whic
