Blunt arterial injury is usually caused by high velocity trauma and can result in intimal dissection. We present a case of a professional footballer who sustained an intimal tear of the posterior tibial artery following a minor eversion injury of the ankle. The injury was noticed because of the physical demands of this patient's profession. This was confirmed by an arteriogram and was treated with bypass surgery using an arm vein. Ankle fractures are one of the most frequently treated injuries in orthopedic traumatology. The simple injuries often are forgiving and are managed easily and appropriately by closed treatment, but more complex injuries require recognition of possible associated injuries and anatomic internal fixation. Although adherence to general guidelines of fracture management usually suffices, some cases do not fall into a well-defined category, and clinicians are called on to use their experience and clinical knowledge to make treatment decisions that best ensure good clinical outcomes.

The success of cartilage repair in the ankle and other joints rests on multiple factors. Because native cartilage healing fails to produce hyaline, organized cartilage, the challenge lies in activating an appropriate healing response. The combination of an optimal structural matrix, infusion of pleuripotent cells, and gene-modified tissue engineering to activate an appropriate healing response appears most sensible. Until such technology is available for use in humans, however, autologous chondrocyte transplantation and osteochondral transfer procedures are attractive alternatives.

We present a case of type II hyperbetalipoproteinemia in a patient whose diagnosis had been previously unrecognized, and who had previously been misdiagnosed with rheumatoid arthritis and later gout. Radiographic and MR imaging features of the patient's ankles were pronounced but otherwise typical of xanthomatous infiltration. Radiologic assessment can be useful in permitting a specific diagnosis to be made in patients with periarticular and tendinous swelling.

Ankle arthroscopy has grown in popularity over the past decade as a diagnostic and therapeutic procedure. We assessed 24 patients who underwent ankle arthroscopy during the two year period 1991-1993. In 18 cases a mini-arthrotomy was performed based on the arthroscopic findings. BACKGROUND: Cases of old ankle fractures with different delayed treatments in different conditions were collected and evaluated to determine which treatments were optimal for individual type of fractures.
METHODS: Sixteen cases of old ankle fractures were managed by open reduction and internal fixation, osteotomy, or removal of loose fragment according to the condition of the fractures between 1977 and 1987. Arthroscopic examination of the ankle joint permits complete examination of the intra-articular structures. With recent advances in equipment and additional experience, numerous ankle problems can be handled arthroscopically. These problems include treatment of soft-tissue and bony impingement, osteochondral lesions of the talus, removal of loose bodies, treatment of acute and chronic fractures, lateral ankle stabilization, and arthroscopic fusion procedures.

We carried out a retrospective study of the frequency of ankle sprains in basketball players. A questionnaire about previous ankle injuries, time off after such injuries, current ankle problems, personal data, number of practice hours and the use of prophylactic measures was sent out to 102 basketball players in a second division league in Sweden. Ninety-six players answered. 92% of them had suffered an ankle sprain while playing basketball, and of these 83% reported repeated sprains of one ankle. Dislocation of the tibiotalar joint without associated fracture is rare. We have presented a case of open anterolateral ankle dislocation with complete ligamentous disruption. In contrast to reports that internal fixation is not required to maintain reduction of tibiotalar joint dislocation, we found this dislocation to be grossly unstable, requiring both a syndesmotic screw and a calcaneotalotibial transfixing pin for stabilization.
