Anorectal melanoma. Problems of histopathologic diagnosis.

The present study discusses, in the light of the comparatively restricted data in the speciality literature, two cases of anorectal melanoma emphasizing the histologic diagnostic difficulties linked to appraisal of the limits of the benign naevus formations, predominance of the achromatous zones, alternation of the benign and sarcomatous fascicular proliferations and those of the anaplastic carcinoma type. Stress is laid on the importance for the diagnosis of the histologic examination and of serial sections, in all pedunculate formations in the anorectal region as well as of the tests applied

Perianal endometrioma: report of five cases.

One of the gynecologists' commonest findings at pelvic laparotomy is endometriosis. Despite the fact that the presence of endometrial tissue in ectopic locations is commonplace, the finding of a perianal endometrioma is no more than a surgical curiosity. Very few cases have been reported. The present report describes five such verified cases. Interestingly enough, only two of the five cases were diagnosed preoperatively. The pathogenesis of endometriosis has been, and continues to be, controversial. The prevailing theories include 1) transtubal regurgitation of menstrual blood, 2) the coelomic

Extramammary Paget disease.

In 1874, Sir James Paget first described Paget disease of the nipple, also known as mammary Paget disease. In 1889, extramammary Paget disease (EMPD) of the scrotum and penis was identified. Although mammary and extramammary Paget disease are both characterized by epidermal Paget cells and share a similar clinical presentation, their uniqueness lies in anatomical location and histogenesis. EMPD presents as an erythematous plaque on apocrine gland bearing areas (i.e. vulva, perineum, perianal region, scrotum, and penis) in older men and women. It can be a focal, multifocal, or an ectopic proces

Primary colonic adenocarcinoma diagnosed with cutaneous shave biopsy.

A 67-year-old man presented to the emergency department with chronic weakness, fatigue and failure to thrive. On physical examination, he was found to have multifocal exophytic cutaneous masses in the pubic and scrotal regions. We obtained a shave biopsy, and subsequent histopathology demonstrated non-native tissue consistent with metastasis from a primary adenocarcinoma. We report this novel case of anogenital cutaneous metastases of colorectal adenocarcinoma.

[Human papillomavirus and the risk of anogenital cancer].

Strong evidence suggests that virtually all cases of cervical cancer are caused by infection with oncognenic types of human papillomavirus (HPV). HPV is also found frequently in neighbouring anogenital epithelia and their neoplastic lesions. This review describes the general aspects of the natural history of anogenital HPV infections and summarises the evidence for an association between HPV and cancers of the uterine cervix, vulva, anus and penis.

Outcomes reported in studies of anal high-grade squamous intraepithelial lesions treatments: systematic review.

INTRODUCTION: Anal high-grade squamous intraepithelial lesion (HSIL) is a precursor of anal squamous cell carcinoma (ASCC). Reported outcomes from treatment trials vary widely, limiting understanding of the effectiveness of different interventions. This systematic review aimed to summarize the outcomes reported in studies evaluating treatments in anal HSIL. METHODS: A systematic review of MEDLINE, Embase, CINAHL, and the Cochrane Library (inception to June 2025) was conducted. Eligible studies evaluated any anal HSIL treatment in any population. Verbatim outcomes and associated definitions, m

Malignancy in Long-Standing Perianal Fistulizing Crohn's Disease: Too Little, Too Late.

Perianal fistulizing Crohn's disease (PFCD) is a chronic, aggressive phenotype associated with an elevated risk of malignancy arising within long-standing fistula tracts. Although rare, PFCD-associated cancers carry a poor prognosis and are often diagnosed late due to overlapping symptoms with active inflammatory disease and the absence of established screening guidelines. We present a case of a 71-year-old man with nearly 50 years of untreated PFCD, who developed mucinous adenocarcinoma arising from a chronic perianal fistula. Despite multiple hospitalizations for recurrent abscesses, sepsis,

Is routine histopathological analysis of hemorrhoidectomy specimens necessary? A systematic review and meta-analysis.

BACKGROUND: Hemorrhoidectomy is a common procedure used to treat symptomatic hemorrhoids. However, the necessity and cost-effectiveness of routinely conducting histopathological analysis on excised tissue samples are uncertain. METHODS: A systematic review was conducted using MEDLINE and EMBASE up to December 2023 for studies assessing the histopathological outcomes of hemorrhoidectomy specimens. Meta-analysis was performed on articles with combinable results to determine the pooled proportions of cancer and anal intraepithelial neoplasia (AIN) using the random effects model. RESULTS: From 2

[Microscopic Anal Canal Adenocarcinoma with Pagetoid Spread-A Case Report].

The patient is a 70-year-old woman who presented with a chief presentation of anal itching. Her previous physician diagnosed her with perianal extramammary Paget's disease, and she was referred to our hospital. Physical examination revealed a well-circumscribed red skin rash 6 cm in diameter centered on her anus. An anal skin biopsy showed adenocarcinoma, and immunostaining demonstrated CK7+,CK19+,CK20+,CDX2+, leading to the suspicion of pagetoid spread of anal adenocarcinoma. Lower gastrointestinal endoscopy revealed no obvious lesions; however, based on immunostaining results, she was diagno

Human papillomavirus in the setting of immunodeficiency: Pathogenesis and the emergence of next-generation therapies to reduce the high associated cancer risk.

Human papillomavirus (HPV), a common sexually transmitted virus infecting mucosal or cutaneous stratified epithelia, is implicated in the rising of associated cancers worldwide. While HPV infection can be cleared by an adequate immune response, immunocompromised individuals can develop persistent, treatment-refractory, and progressive disease. Primary immunodeficiencies (PIDs) associated with HPV-related disease include inborn errors of GATA, EVER1/2, and CXCR4 mutations, resulting in defective cellular function. People living with secondary immunodeficiency (e.g. solid-organ transplants recip

Truth or DARE (Digital Anal Rectal Examination): Gynecologist Viewpoints on Anal Cancer Screening.

METHODS: The authors conducted a survey for practicing gynecologists recruited through academic institutions, professional societies, and professional groups on social media resulting in 196 respondents. The survey, fielded between January and June 2022, included questions on knowledge, attitudes, training, and practices regarding anal cancer prevention (ACP). Descriptive statistics and χ 2 analysis were completed. RESULTS: In terms of knowledge regarding ACP, over 80% of respondents identified certain clinical indications for anal cancer screening. However, only 36% respondents selected the

Anal neoplasia and perianal Crohn's disease: myth or reality?

The association between inflammatory bowel disease (IBD) and anal canal squamous cell carcinoma (SCC) has a low prevalence and is mainly supported by articles that include a series of cases. We describe the clinical, endoscopic and histological findings of a patient with Crohn's disease (CD) who developed SCC while undergoing biological therapy with active disease.

A rare case of a superficial squamous cell carcinoma (so-called cloacogenic carcinoma) of the rectum.

A rare case of a squamous cell carcinoma (so-called cloacogenic carcinoma) showing extensive superficial spread to the rectum is presented. A 69-year-old woman had undergone colonoscopy for annual check-up, and a whitish, flat lesion with a central depressed area, 20 mm in size, was identified in the lower rectum. Narrow-band imaging with magnifying observation showed abnormal microvessels without the intrapapillary capillary loop patterns. Endoscopically, the margin of the lesion was unclear. Biopsy was performed, and a histological diagnosis of transitional cell carcinoma was made. Computed

Can Anal Cytology Be a Tool in Following Patients Treated for Squamous Cell Carcinoma of the Anus?

BACKGROUND: Squamous cell carcinoma of the anus (SCCA) is associated with human papillomavirus infection and preceded by high-grade squamous intraepithelial lesions (HSIL). Following successful treatment, the standard of care is to surveille for local recurrence with both anoscopy and digital rectal examination. While high-resolution anoscopy (HRA) has been shown to identify HSIL during the surveillance period, it requires specialized training and resources.1 The burden of these resources may be reduced by conducting surveillance with anal cytology. We studied 2 questions: (1) Can anal cytolog

Pelvic U-Net: multi-label semantic segmentation of pelvic organs at risk for radiation therapy anal cancer patients using a deeply supervised shuffle attention convolutional neural network.

BACKGROUND: Delineation of organs at risk (OAR) for anal cancer radiation therapy treatment planning is a manual and time-consuming process. Deep learning-based methods can accelerate and partially automate this task. The aim of this study was to develop and evaluate a deep learning model for automated and improved segmentations of OAR in the pelvic region. METHODS: A 3D, deeply supervised U-Net architecture with shuffle attention, referred to as Pelvic U-Net, was trained on 143 computed tomography (CT) volumes, to segment OAR in the pelvic region, such as total bone marrow, rectum, bladder,

Anal cancer screening among women with HIV: provider experiences and system-level challenges.

Women living with HIV (WLWH) are at increased risk of anal cancer compared to women without HIV, often due to persistent human papillomavirus (HPV) infections. This paper describes current practices and challenges conducting anal cancer screening for WLWH at an urban integrated safety-net system and a non-profit community-based HIV clinic. We conducted 25 semi-structured interviews with clinical and administrative stakeholders to assess knowledge, clinic practices and procedures, and experiences with anal cancer screening. Interview transcripts and fieldnotes were thematically analyzed using a
