[Arachnoid cyst of the sphenoidal sinus: differential diagnosis of mucocele].

We here report the case of a 45 year old patient, with no previous history of head injury, presenting with isolated intermittent headache which had evolved since the young age. Clinical examination was normal. Cerebral CT scan showed osteolytic expansive hypodense lesion of the sphenoidal sinus extending inferiorly into the infratemporal fossa and superiorly into the temporal fossa (A). This mass wasn't contrast enhanced. A mucocele was suspected. MRI was performed which showed that the mass had the same enhancement as the cerebrospinal fluid (CSF) and was in continuity with the temporal menin

Spinal arachnoid cysts - our experience and review of literature.

OBJECTIVE: Arachnoid cysts are discrete pockets of CSF or CSF-like fluid found adjacent to normal CSF spaces, either spinal or cranial. Spinal arachnoid cysts (SAC) are most commonly extradural, however intradural or perineural are also described. METHODS: All patients admitted to our center and surgically treated with a diagnosis of SAC, were included in the study. The results were analyzed in terms of the clinical symptoms, location of cyst, surgical procedure performed and outcome following surgery. RESULTS: Eleven patients were operated for SAC during the study period and the mean age at

Cerebellopontine angle arachnoid cyst associated with mirror movements.

Arachnoid cysts are benign developmental collections of cerebrospinal fluid (CSF). They constitute approximately 1% of intracranial masses. The cerebellopontine angle (CPA) arachnoid cysts are rare and often asymptomatic. The onset of symptoms and signs is usually due to the compression of the brain, cranial nerves and obstruction of CSF circulation. The major clinical symptoms for CPA arachnoid cysts were reported as headache, ataxia and 8(th) cranial nerve palsy. We report a patient with a CPA arachnoid cyst. He presented with cranial nerve palsies and mirror movements found in upper extremi

Spontaneous Subdural Hematoma and Intracystic Hemorrhage in an Arachnoid Cyst.

We report the case of 22-year-old man with nontraumatic intracystic hemorrhage into a middle cranial fossa arachnoid cyst associated with a contiguous subacute subdural hematoma. Arachnoid cysts are benign intra-arachnoidal fluid collections frequently detected incidentally during neuroimaging. Rare complications of arachnoid cysts such as intracystic hemorrhage or subdural hematomas and subdural hygromas typically occur after head trauma. Our review of the literature identified fewer than 30 cases of arachnoid cysts with complicating intracystic hemorrhage and ipsilateral subdural hematomas.

Trigeminal neuralgia in a paediatric patient caused by arachnoid cyst.

Trigeminal neuralgia (TGN) in the paediatric population is seldom reported and often not considered in the differential diagnosis of paediatric craniofacial pain. Even less common are cerebellopontine angle (CPA) lesions causing TGN in paediatric patients. To the best of our knowledge, this is the first report of a CPA arachnoid cyst causing TGN in a paediatric case.

Arachnoid cyst accompanied by proptosis and unilateral high myopia.

An 8-year-old boy presented with proptosis of the left eye. On ophthalmic examination, unilateral axial elongation of the left eye was evident.Computed tomography and magnetic resonance imaging revealed an arachnoid cyst in the left middle cranial fossa, which expanded the bone and deformed the posterolateral wall of the left orbit. The lesion was observed with serial examinations and reviewed by a neurosurgeon and a radiologist. After 6 months of follow-up, there was no progression of the cyst. We believe this is the youngest reported case of an arachnoid cyst associated with isolated proptosi

Neuronavigation-guided endoscopic and hodotopic approach to an arachnoid cyst.

BACKGROUND: Arachnoid cysts are intraarachnoid benign cystic lesions filled with cerebrospinal fluid and should be treated without incurring further morbidity to the patients. CASE DESCRIPTION: The authors present a case of a 68-year-old elderly female with a large right fronto-parieto-temporal arachnoid cyst who has been suffering from mild left hemiparesis for the past 4 years and presented with sudden onset of seizures. The 3 Tesla MR system with diffusion tensor imaging (DTI) and MR tractography of the brain showed a large right fronto-parieto-temporal cystic lesion measuring 7 × 5 × 5 cm

Inflamed symptomatic sellar arachnoid cyst: case report.

Sellar arachnoid cysts are rare; an infected arachnoid cyst is extremely rare as only one case has been reported to date in the literature. Here, we report a patient with an infected or inflamed sellar arachnoid cyst that was successfully treated with transsphenoidal surgery (TSA). A 53-year-old female with a history of chronic sinusitis developed a headache 5 months ago, and one month before admission polyuria, polydipsia, and abnormal vaginal bleeding occurred. The magnetic resonance imaging (MRI) showed a sellar cystic mass with a thickened pituitary stalk. Preoperative hormonal study revea

Trochlear myokymia secondary to cerebellopontine angle arachnoid cyst.

Superior oblique myokymia (SOM) is a rare disorder with an unclear pathogenesis. We describe a first reported case of chronic disabling SOM in association with a cerebellopontine angle arachnoid cyst, who had a gradual and eventually complete symptomatic resolution 8 months following cyst marsupialisation. Among other aetiologies, SOM may therefore be due to abnormal CSF flow dynamics resulting in structural compromise of the nerve.

Arachnoid cyst masquerading as obstetric brachial plexus palsy.

Obstetric brachial plexus palsy is not uncommon. However, lesions masquerading as obstetric brachial plexus palsy are rare. A child with a cervicothoracic arachnoid cyst masquerading as obstetric brachial plexus palsy is presented, and the relevant literature is reviewed. A girl born by vaginal delivery at full term without any antecedent risk factors for obstetric brachial plexus palsy was noted to have decreased movements of the right upper extremity. After 7 months, there was no improvement. An MRI scan was obtained, which revealed a cervicothoracic spinal extradural arachnoid cyst. During

Intracranial arachnoid cyst on dental radiography: a diagnostic challenge.

Intracranial arachnoid cysts (IACs) can present as congenital asymptomatic lesions that may predispose them to present as an incidental finding during radiographic examination. On the other hand, IACs may also give rise to a series of neurologic symptoms depending on their size and location, such as vomiting, seizures, headache, and ataxia. Skull deformities, including macrocephaly, may occur and become remarkable on dental radiology. We report 2 patients who were identified with IAC before orthodontic treatment. The dental radiologic appearance of IAC is discussed and may constitute a diagnos

Spinal extradural arachnoid cyst: review of surgical techniques.

Extradural arachnoid cysts of the spine are a rare cause of spinal cord and nerve root compression, usually in the mid to lower thoracic spine and at the thoraco-lumbar junction in a posterior position. Local pain and myelopathy occur predominantly in young adults, and MRI reveals a well-demarcated extradural lesion, iso-intense to cerebrospinal fluid. We report a 33-year-old woman with an extradural arachnoid cyst from T11 to L1 and review the surgical techniques reported in the literature.

Congenital intradiploic arachnoid cyst presenting as painless proptosis.

A 12-year-old girl presented with several years of progressive painless proptosis and orbital fullness. On imaging, she was found to have marked expansion and remodeling centered in the right greater wing of the sphenoid bone with an adjacent middle cranial fossa arachnoid cyst. A clinical diagnosis of intradiploic arachnoid cyst was made. This entity was benign, and other ophthalmic and neurologic signs or symptoms were absent. Our patient was observed without surgical intervention. To our knowledge, this is the first such case reported in a child. Although this cyst has been described in the

Cleft palate, arachnoid cyst and posterior urethral valve.

A 5-month-old infant presented with cleft palate, febrile urinary tract infection and abnormal movements. His urinary stream was poor and investigations revealed posterior urethral valve and grade V vesicoureteric reflux. This baby was investigated for seizure activity and on MRI of the brain was found to have a large arachnoid cyst, splaying the occipital horns and compressing the posterior aspect of third ventricle leading to dilatation of the ventricles. This seems to be a very unusual coexistence of three abnormalities in one patient for which no plausible explanation can be given. It appe

Electroconvulsive therapy for catatonia in a boy with hydrocephalus and an arachnoid cyst.

We report on the successful use of electroconvulsive therapy in a 13-year-old boy with congenital hydrocephalus, a history of multiple shunt revisions, and a stable prepontine arachnoid cyst, who experienced profound catatonic deterioration. After initial shunt placement at age 20 months, the patient had followed normal motor, cognitive, and social developmental trajectories. Two uncomplicated shunt revisions were performed at ages 10 and 13 years. Three months after the last revision, the patient demonstrated multiple psychomotor signs, culminating in hospital admission for rigidity, posturin

Microsurgical and endoscopic anatomy of the supratentorial arachnoidal membranes and cisterns.

OBJECTIVE: A limitation of previous studies of the arachnoid cisterns and membranes is that the act of opening the sylvian and interhemispheric fissures and basal arachnoid often led to destruction of the cisternal compartments and their membranous walls. The goal of this study was to overcome this limitation by combining the surgical microscope and endoscope for the examination of the cisternal compartments and their membranous walls. METHODS: The supratentorial cisterns were examined in 22 cadaveric brains using both the operating microscope and the endoscope. RESULTS: There are 2 types of
