[Speech therapy for children with cleft lips and palates. I. Basic linguistic considerations].

According to several basic principals of hermeneutic theory (theory of understanding natural languages) we describe the major fields of language acquisition in which anomalies of CPL children often can be found. Besides phonation and articulation there are morphological, psychomotoric, onomatopoetic, and isotopic disorders. Also a delay of understanding by listening is often observed. Additionally some cofactors in psychodynamics of the family have to be respected. Defense mechanisms, somatization, and social symptomatization are described. Some examples from statistics and experimental psycho

A comparison of elective mutism and emotional disorders in children.

The case notes of 24 children diagnosed as elective mutes were compared with those of 24 matched controls with diagnosed emotional disorders. One-third of the elective mutes (but none of the controls) had experienced delayed development of speech or difficulties in articulation. All the elective mutes lived in two-parent families, but marital discord was noted in half the families in both groups. Compared with the controls, the mute children were more often described as 'anxious', 'depressed' and 'manipulative', and their mothers were characterised as 'over-protective', and tending to 'spoil'

A case of trichorhinophalangeal syndrome, type I.

A single case of trichorhinophalangeal syndrome, type I, is reported. The patient manifested the complete triad of the syndrome: thin, sparse, slow-growing scalp hairs; a pear-shaped nose; and cone-shaped epiphysis in some interphalangeal articulations of both hands. In addition, tooth decay, pectus carinatum, lumbar scoliosis, and polycystic ovary were observed. This multisystemic disorder is apparently rare, but its prevalence is probably underestimated in the dermatologic literature.

Familial spastic paraplegia on Mitiaro, Cook Islands.

Familial spastic paraplegia is described in four Cook Island children. Prior to the onset of the disease at age six or seven years, the children were apparently normal. Disease was characterised principally by progressive spastic paraplegia of the lower extremities, in addition to tremor and slow articulation. The disease in these children was probably due to the expression of an autosomal recessive gene. The disorder is of interest because the findings in these children appear to overlap the pure spastic and ataxic heredofamilial syndromes.

Profile of language abilities in a sample of adults with developmental disorders.

This study investigated the profile of language abilities in a sample of high-achieving English speaking adults with developmental disorders. Ninety-seven adult participants were recruited: 49 with a dyslexia diagnosis (dyslexic group), 16 with a diagnosis of a different developmental disorder including dyspraxia, autism and SpLD (non-dyslexic developmental disorder group) and 32 with no diagnosis (non-disordered group). Dyslexic and non-dyslexic developmental disorder groups demonstrated similar impairments across measures of word reading, working memory, processing speed and oral language. D

Speech outcomes after palatal closure in 3-7-year-old children.

INTRODUCTION: One of the main goals of the team approach in management of oro-facial clefts is to help the children with cleft palate have adequate speech development. OBJECTIVE: The present study aimed to investigate the prevalence of articulation and resonance disorders following palate closure in children who were visited for routine examination by the Isfahan Cleft Care Team between 2011 and 2015, and to study the impact of cleft type and age at the time of palatoplasty on speech outcomes. METHODS: Clinical records of 180 preschool children with repaired cleft palate were reviewed. The p

Use of electropalatography in the treatment of speech disorders in children with Down syndrome: a randomized controlled trial.

BACKGROUND: Electropalatography (EPG) records details of the location and timing of tongue contacts with the hard palate during speech. It has been effective in treating articulation disorders that have failed to respond to conventional therapy approaches but, until now, its use with children and adolescents with intellectual/learning disabilities and speech disorders has been limited. AIMS: To evaluate the usefulness of EPG in the treatment of speech production difficulties in children and adolescents with Down syndrome (DS) aged 8-18 years. METHODS & PROCEDURES: A total of 27 children with…

Phonological deficits in specific language impairment and developmental dyslexia: towards a multidimensional model.

An on-going debate surrounds the relationship between specific language impairment and developmental dyslexia, in particular with respect to their phonological abilities. Are these distinct disorders? To what extent do they overlap? Which cognitive and linguistic profiles correspond to specific language impairment, dyslexia and comorbid cases? At least three different models have been proposed: the severity model, the additional deficit model and the component model. We address this issue by comparing children with specific language impairment only, those with dyslexia-only, those with specifi

[Retraining in dento-facial orthopedics. An orthodontist's viewpoint].

In dentofacial orthopedics, rehabilitation concerns the performance of the orofacial functions. Orthodontists will therefore speak of rehabilitation of functions, or functional therapy. Orofacial functions seem to obey a hierarchical system in accordance with their physiological importance; the impact of their disorders on both muscular behavior and facial morphogenesis, proportionally to their physiological importance. Therefore, priority will be given at a very early stage to the rehabilitation of respiration: restoring nasal ventilation is indeed an essential condition to obtain balanced or

A comparison of babbling and speech at pre-speech level, 3, and 5 years of age in children with cleft lip and palate treated with delayed hard palate closure.

Babbling and speech in 21 children with cleft palate were compared at pre-speech level, 3, and 5 years of age. The aims were to study if misarticulations in pre-school speech appear to be articulatorily related to the sound productions in pre-speech, whether the feeding technique influenced the prevalence of anterior articulation, and if there was a relationship between speech and the size of the residual cleft at 3 and 5 years of age. All the children had the soft palate closed, whereas the cleft in the hard palate was left open to be closed later on. Perceptual judgement of speech revealed a

Is covert articulation impaired in developmental stuttering?

Adults who stutter (AWS) have been reported to show difficulties in nonword repetition tasks, where covert articulation supports the maintenance of phonological information during the retention interval. However, it remains unclear whether these difficulties reflect impaired covert articulation, auditory-phonological encoding, overt articulation, or aspects of executive function/attention. It is also unknown whether such difficulties characterize any, some, most, or all AWS. To address these questions, we administered spoken and written versions of a phonological memory task-both using printed

Neuromodulation of the Right Motor Cortex of the Lips With Repetitive Transcranial Magnetic Stimulation to Reduce Phonological Impairment and Improve Naming in Three Persons With Aphasia: A Single-Case Experimental Design.

PURPOSE: Repetitive transcranial magnetic stimulation (rTMS) can enhance aphasia recovery. Most studies have used inhibitory stimulation targeting the right inferior frontal gyrus. However, the motor cortex, observed to contribute to the prediction of aphasia recovery, is involved in word production and could be an appropriate target for rTMS. We aimed to observe behavioral changes in a picture naming task induced by inhibitory rTMS targeting the right motor cortex of the lips in people with poststroke aphasia. METHOD: Using a single-case experimental design, we included three participants wi

Articulation distortion in Mandarin-speaking individuals with complete arch maxillary implant-supported fixed dental prostheses.

STATEMENT OF PROBLEM: Implant-supported fixed dental prostheses (IFPs) have been increasingly used to restore edentulous jaws, yet few studies have utilized acoustic analysis for objective evaluation of postrestoration speech outcomes. PURPOSE: The purpose of this clinical study was to assess speech articulation in edentulous individuals before and after the provision of IFPs by combining the results of subjective evaluations and objective acoustic analysis parameters. MATERIAL AND METHODS: The study included thirty-four individuals who had an edentulous maxilla and had been provided with an

Classification of Articulator Movements and Movement Direction from Sensorimotor Cortex Activity.

For people suffering from severe paralysis, communication can be difficult or nearly impossible. Technology systems called brain-computer interfaces (BCIs) are being developed to assist these people with communication by using their brain activity to control a computer without any muscle activity. To benefit the development of BCIs that employ neural activity related to speech, we investigated if neural activity patterns related to different articulator movements can be distinguished from each other. We recorded with electrocorticography (ECoG), the neural activity related to different articul

Kurdish Speech Test: A validation study for children aged 3-5 years.

OBJECTIVES: Speech disorder in children is the most common disorder reported by speech and language pathologists in all languages and its evaluation and diagnosis requires valid and reliable tools. This study aimed to develop a Kurdish Speech Test for children aged 3-5 years and to determine its psychometric properties. METHODS: The validation included 120 monolingual Kurdish-speaking children aged 3-5 years in the city of Bukan, Iran. Content validity was determined according to expert opinions of Kurdish speech and language pathologists and linguists. Test-retest (one-week interval) and sco

Revision laryngeal framework surgery performed by directly pulling the lateral cricoarytenoid muscle.

BACKGROUND: Revision laryngeal framework surgery is usually performed for medialisation laryngoplasty failure, rather than for failure after arytenoid adduction. We describe a new method for revision arytenoid adduction surgery, performed by directly pulling the lateral cricoarytenoid muscle ('lateral cricoarytenoid muscle pull surgery'). METHODS: We describe a case of revision laryngeal framework surgery, present a literature review and describe the advantages of lateral cricoarytenoid muscle pull surgery over the original method of arytenoid adduction using a posterior approach. RESULTS: M

Relationship between speech motor control and speech intelligibility in children with speech sound disorders.

UNLABELLED: The current study was undertaken to investigate the impact of speech motor issues on the speech intelligibility of children with moderate to severe speech sound disorders (SSD) within the context of the PROMPT intervention approach. The word-level Children's Speech Intelligibility Measure (CSIM), the sentence-level Beginner's Intelligibility Test (BIT) and tests of speech motor control and articulation proficiency were administered to 12 children (3:11 to 6:7 years) before and after PROMPT therapy. PROMPT treatment was provided for 45 min twice a week for 8 weeks. Twenty-four naïve

Two French-speaking cases of foreign accent syndrome: an acoustic-phonetic analysis.

Foreign accent syndrome (FAS) is an acquired neurologic disorder in which an individual suddenly and unintentionally speaks with an accent which is perceived as being different from his/her usual accent. This study presents an acoustic-phonetic description of two Quebec French-speaking cases. The first speaker presents a perceived accent shift to Acadian French (French spoken in the easternmost provinces of Canada), whereas the second acquired an accent identified as Germanic. Speech seems affected by constraints on the coordination of articulatory gestures, expressed by distortions in the pro
