Effect of Sildenafil Citrate on Exercise Capacity in Athletes With Spinal Cord Injury.

PURPOSE: Ingestion of sildenafil citrate has performance-enhancing effects at high altitudes above 3800 m in able-bodied individuals. It is unknown whether it can improve the performance of athletes with spinal cord injury (SCI) at moderate altitudes (<2200 m), relevant to Paralympic competitions. As most men with SCI suffer from erectile dysfunction of neurologic origin and use sildenafil on a regular basis, it seems important to study the impact of sildenafil on exercise capacity. The outcome of this study is also relevant to the antidoping community. METHODS: Twenty-seven healthy male whee

Pars interarticularis injury in elite athletes - The role of imaging in diagnosis and management.

Injuries of the lumbar neural arch, in particular the pars interarticularis, are widely considered to be due to abnormal bone stress secondary to repetitive loading/shearing, and are a common pathology and a main cause of lower back pain in elite athletes across a range of sports. Medical imaging plays a pivotal role in the diagnosis, monitoring and prognostication of neural arch injury. Early detection is highly desirable in the young elite athlete, as early injuries have been shown to require shorter recovery time and have a higher potential of full healing, whilst accurate grading of injury

Comparison of Psychological Response between Concussion and Musculoskeletal Injury in Collegiate Athletes.

The psychological response to musculoskeletal injuries has been well documented, however, research on the psychological response to concussion is limited. The Profile of Mood States (POMS) and the State-Trait Anxiety Inventory (STAI) have recently been used to assess the psychological recovery of concussions. Although some studies indicate that psychological response is different for musculoskeletal injuries and concussion, there is currently not enough information to indicate this difference occurs at specific clinical milestones. The purpose of this study was to compare the psychological res

Nutrient Intake of Elite Canadian and American Athletes with Spinal Cord Injury.

The nutrient needs of athletes with Spinal Cord Injury (SCI) are dependent on their physiological alterations and training status. Limited research is available regarding dietary intake of elite athletes with SCI and possible nutrient deficiencies. Therefore, the purpose of this study was to examine dietary intake of elite athletes with SCI, and determine dietary intake inadequacies based on the Estimated Average Requirement (EAR) comparisons. Additionally, the average energy and macronutrient (carbohydrate, protein, and fat) intake was compared based on level of injury (C level, T1-T6, T7-T12

Evaluation and management of knee pain in young athletes: overuse injuries of the knee.

Recurrent or chronic activity related knee pain is common in young athletes. Numerous intrinsic conditions affecting the knee can cause such pain. In addition, knee pain can be referred pain from low back, hip or pelvic pathology. The most common cause of knee pain in young athletes is patellofemoral pain syndrome, or more appropriately termed idiopathic anterior knee pain. Although, numerous anatomical and biomechanical factors have been postulated to contribute the knee pain in young athletes, the most common underlying reason is overuse injury. In this paper, we have reviewed selected condi

The Role of Magnetic Resonance Imaging in Athletic Pubalgia and Core Muscle Injury.

Magnetic resonance imaging (MRI) has become the standard of care imaging modality for a difficult, often misunderstood spectrum of musculoskeletal injury termed athletic pubalgia or core muscle injury. Armed with a dedicated noncontrast athletic pubalgia protocol and a late model phased array receiver coil, the musculoskeletal imager can play a great role in effective diagnosis and treatment planning for lesions, including osteitis pubis, midline pubic plate lesions, and rectus abdominis/adductor aponeurosis injury. Beyond these established patterns of MRI findings, there are many confounders

Overuse throwing injuries in skeletally immature athletes--diagnosis, treatment, and prevention.

The number of skeletally immature athletes participating in organized sport is near an all-time high. For nearly half a century, the medical community has investigated the link between shoulder and elbow injuries with repetitive throwing. Despite substantial literature and research, several controversies still exist, including when to begin throwing breaking pitches. Furthermore, despite introduction of pitch recommendations for youth baseball, misconceptions, poor understanding of, and adherence to these guidelines persist. High-velocity and high-volume throwing and throwing while fatigued ar

Core Muscle Injury/Sports Hernia/Athletic Pubalgia, and Femoroacetabular Impingement.

Core muscle injury/sports hernia/athletic pubalgia is an increasingly recognized source of pain, disability, and time lost from athletics. Groin pain among athletes, however, may be secondary to various etiologies. A thorough history and comprehensive physical examination, coupled with appropriate diagnostic imaging, may improve the diagnostic accuracy for patients who present with core muscular injuries. Outcomes of nonoperative management have not been well delineated, and multiple operative procedures have been discussed with varying return-to-athletic activity rates. In this review, we out

Anterior cruciate ligament injuries in the skeletally immature athlete: diagnosis and management.

Intrasubstance anterior cruciate ligament (ACL) injuries in children and adolescents were once considered rare occurrences, with tibial eminence avulsion fractures generally regarded as the pediatric ACL injury equivalent. However, with increased single-sport focus, less free play, and year-round training at younger ages, intrasubstance ACL injuries in children and adolescents are being diagnosed with increased frequency. As in the adult, a knee devoid of ligamentous stability predisposes the pediatric patient to meniscal and chondral injuries and early degenerative changes. Management of ACL

Technique tip: percutaneous fixation of partial incongruous Lisfranc injuries in athletes.

Open reduction with screw fixation is considered the standard surgical approach for injuries of the Lisfranc complex in athletes. However, multiple incisions are required, which increase the risk for postoperative complications. We present a novel percutaneous reduction and solid screw fixation technique that may be a viable option to address partial incongruous injuries of the Lisfranc complex in athletes. At our institution, no intraoperative or postoperative complications have been encountered. Screw breakage did not occur. Reduction of the second metatarsal was considered anatomic across a

Multifragmentary distal crural fracture ski injury in an athlete: a case report.

While alpine skiing, a professional skater fell. He was examined at the nearest hospital and diagnosis was distal crural fracture with multifragmentary tibial metaphysis. The patient left the hospital with only temporary immobilization after the suggested treatment for his injury was crural amputation. Thirteen hours after injury he arrived at his home hospital. He was immediately operated on to perform osteosynthesis on his left lower leg. The healing of the damaged soft tissue as well as the bones was excellent even though Sudeck's syndrome appeared. Six month after injury he returned to ska

Knee popping and clicking in a pediatric athlete: meniscal injury or sports tumor?

This case report presents a teenage patient who initially was thought to have a sports-related injury but ultimately was diagnosed with a primary soft tissue tumor. A previously healthy 16-year-old softball player presented with a history of left knee joint line pain, clicking, and swelling. The patient was presumed to have a lateral meniscus tear. However, magnetic resonance imaging demonstrated an intra-articular mass. Arthroscopy revealed a 2.5- × 1.5-cm firm pedicular mass in the lateral joint. Histological exam demonstrated localized pigmented villonodular synovitis. The patient healed un
