Second-degree atrioventricular block: Mobitz type II.

Acute atrioventricular (AV) block occurs frequently in patients with myocardial infarction. Atrioventricular block is also a common manifestation of sclerodegenerative conduction system disease. Occasionally, heart block results from drug toxicity, hyperkalemia, cardiac valvular calcification, myocarditis, or infiltrative cardiomyopathy. Second-degree AV block is a form of "incomplete" heart block, in which some, but not all, atrial beats are blocked before reaching the ventricles. Mobitz type II second-degree block is an old term, which refers to periodic atrioventricular block with constant

Orthodromic Atrioventricular Reentry Bradycardia and Tachycardia Caused by an Accessory Pathway in Horses.

BACKGROUND: Accessory pathways (APs) are muscular bundles directly connecting the atria and ventricles, bypassing the atrioventricular (AV) node-His-Purkinje system. Anterograde conduction along the AP results in ventricular preexcitation. A retrograde conducting AP allows ventriculo-atrial (VA) conduction, creating a reentry circuit that mediates orthodromic atrioventricular reentry tachycardia (OAVRT). This condition is well described in humans and small animals, but has not been reported previously in horses. OBJECTIVES: Describe clinical and electrocardiographic findings of horses with re

Isolated congenitally complete heart block attributable to combined nodoventricular and intraventricular discontinuity.

Intraventricular together with atrial-axis and nodoventricular discontinuity, in which various parts of the conduction system are replaced b … d by fatty tissue in the second. In contrast to the commoner atrial-axis discontinuity in which the atrioventricular node itself is usually replaced by fibrous or fatty tissue with variable involvement distally, the sinus node, and in particular, the atrioventricular node were normal in both of our cases. There was no family history in either case, whereas tests for th …

Subclinical maternal autoimmune disease leading to congenital high degree atrioventricular block: Case report and review of the literature.

Maternal autoimmune disease is the most common cause of congenital heart block (CHB), a rare illness characterized by fibrosis and calcification of the fetal atrioventricular (AV) node due to maternal autoantibodies anti-SSA/Ro and anti-SSB/La. We report the full autopsy and clinical information on a female neonate with high degree AV block and calcification in the AV node, atrial approaches to the AV node, and both right and left bundle branches, born to a 27-year-old female with subclinical autoimmune disease.

[Anti-arrhythmia effectiveness of phosphobion and finoptin in the treatment of atrioventricular retrograde paroxysmal tachycardia].

Forty-four cases of induced atrioventricular retrograde paroxysmal tachycardia (AVRPT) were studied. AVRPT was provoked by means of frequent esophageal stimulation of the left atrium. During the study, records from the external and esophageal ECG leads were taken. Administration of 10 mg finoptin controlled the AVRPT attack in 86.7% of cases, and the administration of 10 mg fosfobion, in 76.2%. Antegrade block of echo impulse conduction through the atrioventricular node was the electrophysiologic mechanism of AVRPT control in all cases. The time needed to control the attack by the above-mentio…

Incessant non-reentrant supraventricular tachycardia interrupted by apparent simultaneous wenckebach sequences along both nodal pathways.

We report a new case of non-reentrant supraventricular tachycardia, associated with tachycardia-induced cardiomyopathy fully reversible after radiofrequency (RF) ablation, together with striking features of apparent concomitant Mobitz type 1 atrioventricular (AV) block in both AV node pathways. Further analysis of the conduction patterns during the incessant non-reentrant tachycardia raised unresolved hypothesis about the involved mechanisms and further interrogations on AV node physiology.

Antidromic AVRT with changing His activation. What is the mechanism?

We present a case of antidromic AVRT involving an atriofascicular pathway in a patient with an atretic coronary sinus os. This patient had left ventricular dysfunction with rate-related left bundle branch block aberrancy in sinus rhythm. In the electrophysiology lab, there was VA dissociation for a few beats during antidromic tachycardia. The possible mechanisms that could explain this intracardiac finding are discussed.

Potential triggering of repetitive nonreentrant ventriculoatrial synchrony by loss of atrial capture.

BACKGROUND: Data on the factors that trigger repetitive nonreentrant ventriculoatrial synchrony (RNRVAS) are limited. We hypothesize that loss of atrial capture may trigger RNRVAS. We aimed to use an atrial threshold test to observe the development of RNRVAS upon loss of atrial capture in patients with implantable cardiac electronic devices (CIED). METHODS: Patients with DDD mode CIEDs [177 patients, 67.5 ± 14.8 (70) years; 70 women] were included. Atrial threshold test was done in DDD mode at a rate at least 10 beats above the basal heart rate, with an AV delay of 300 ms (range 250-350). A m

[Dual ventricular response - a rare and underdiagnosed condition?].

Dual ventricular response, also called dual atrioventricular [AV] nodal non-re-entrant tachycardia, is a rare arrhythmic condition, characterized by 1:2 anterograde conduction via dual AV-nodal pathways, that is, one P wave is followed by two QRS complexes. The main symptom is palpitations, and it can be treated by catheter ablation. To date, less than 100 cases of dual ventricular response have been published in the scientific literature. In this case report, we present two patients with dual ventricular response (based on ECG findings), both of whom were identified during a 10-year period at

Atrioventricular nodal reentry with retrograde lower common pathway block: a case report.

Retrograde lower common pathway block during AV nodal reentrant tachycardia has rarely been reported. In this report, we demonstrate retrograde ventriculoatrial block during sinus rhythm and during AV nodal reentrant tachycardia in a woman with recurrent supraventricular tachycardia. In addition, evidence of antegrade decremental lower common pathway conduction is presented that produced variable HA intervals despite a constant atrial cycle length. These features may lead to the misdiagnosis of atrial tachycardia if not recognized.

One-to-two atrioventricular conduction causing nonreentrant tachycardia: successful treatment with radiofrequency ablation.

The anatomical substrate for AV nodal reentrant tachycardia (AVNRT) is well known and is due to anterograde conduction through a slow conducting pathway and retrograde conduction using a fast conducting pathway. In this report, we describe a patient with AVNRT who also presented with frequent episodes of paroxysmal nonreentrant tachycardia due to the occurrence of two conducted ventricular beats for each sinus depolarization. Palpitations and arrhythmias were abolished after radiofrequency ablation of the slow pathway.

Analysis of electrophysiological activity in Koch's triangle relevant to ablation of the slow AV nodal pathway.

Atrioventricular junctional reentrant tachycardia (AVJRT) is the most common form of paroxysmal regular supraventricular tachycardia. In patients with disabling, drug refractory AVJRT, catheter ablation has evolved rapidly from a last-resort treatment in the form of interruption of atrioventricular (AV) conduction to selective modification of AV nodal function as an ideal treatment. This article will focus on the frequently unappreciated electrophysiological activities recordable in man in Koch's triangle during ablation of the so-called slow pathway.

Diagnostic approach to narrow complex tachycardia with VA block.

Narrow complex tachycardia with VA block is rare. The differential diagnosis usually consists of (1) junctional tachycardia (JT) with retrograde block; (2) AV nodal reentrant tachycardia (AVNRT) with proximal common pathway block; and finally (3) nodofascicular tachycardia using the His-Purkinje system for antegrade conduction and a nodofascicular pathway for retrograde conduction. Analysis of tachycardia onset and termination, the effect of bundle branch block on tachycardia cycle length, and the response to atrial and ventricular premature depolarization must be carefully done. Atrioventricular conduction: a determinant for the manifestation of ventricular preexcitation in patients with Wolff-Parkinson-White syndrome.

The relation between the atrioventricular conduction properties of the atrioventricular node and the anterograde conduction ability over the accessory pathway in the Wolff-Parkinson-White syndrome has never been studied. Atrioventricular nodal characteristics were studied in 285 patients with manifest and 204 with concealed accessory pathway who underwent radiofrequency ablation, and compared with 146 controls. First and second degree atrioventricular block was observed in 13 (5%) preexcitation patients after ablation, compared with none in concealed accessory pathway (P=0.001) and control pat…

Sinus node dysfunction: an adverse effect of lacosamide.

Lacosamide, a recently introduced antiepileptic drug, acts by enhancing the slow inactivation of voltage-dependent sodium channels. Cardiac conduction disturbances, namely atrial fibrillation and atrioventricular block, have been reported in patients with epilepsy. We report a patient with drug-resistant focal epilepsy who developed asymptomatic sinus node dysfunction following lacosamide use, which resolved on stopping lacosamide. This is the first report of sinus node dysfunction associated with lacosamide therapy.

Surgical Procedure for Right Anteroseptal Accessory Conduction Pathways in Wolff-Parkinson-White Syndrome.

In patients with Wolff-Parkinson-White syndrome, the right anteroseptal accessory conduction pathway is rare, and exists from the atrium to the ventricle in close anatomic proximity to the normal atrioventricular conduction system. Catheter ablation of this lesion is reported to interrupt atrioventricular node-His bundle conduction more easily than that of other lesions. At our institute, there were 10 patients with right anteroseptal accessory conduction pathway among 454 patients (2.2%) who underwent the surgical division of the accessory pathway. Our procedure involved the the endocardial a…
