The transoropalatal approach to the atlantoaxial-clival region: considerations for the head and neck surgeon.

The transoropalatal approach to the atlantoaxial-clival area provides excellent exposure for neurosurgical decompression and fusion procedures. The technique has been effectively applied in the treatment of bony and soft tissue abnormalities of the anterior spinal region. Although this approach is safe and reliable, it is not part of the otolaryngologist's usual surgical armamentarium. In the present study, we describe the case histories of five patients who underwent resection of the odontoid process for cervicomedullary compression. Neurological compromise was manifested by progressive upper

C4 constant vertebra: a novel benchmark of physiological cervical sagittal alignment.

BACKGROUND: Cervical sagittal alignment is essential, and there is considerable debate as to what constitutes physiological sagittal alignment. The purpose of this study was to identify constant parameters for characterizing cervical sagittal alignment under physiological conditions. METHODS: A cross-sectional study was conducted in which asymptomatic subjects were recruited to undergo lateral cervical spine radiographs. Each subject was classified according to three authoritative cervical sagittal morphology classifications, followed by the evaluation of variations in radiological parameters

Posterior screw placement on the lateral mass of atlas: an anatomic study.

STUDY DESIGN: An anatomic study evaluating the feasibility of posterior screw fixation on the lateral mass of the atlas and comparing the pullout force of C1 posterior lateral mass screws with that of occipital and C2 pedicle screws. OBJECTIVES: To study the feasibility, anatomy, and biomechanics of posterior screw fixation within the lateral mass of the atlas. SUMMARY OF BACKGROUND DATA: Occipitocervical fusion is a common choice for atlantoaxial dislocations. After that the movement of occipitocervical region will be restricted. If screws could be placed in the lateral mass of the atlas, 1

Craniovertebral junction: normal anatomy, craniometry, and congenital anomalies.

The craniovertebral junction (CVJ) comprises the occiput, atlas, and axis and is visible in most magnetic resonance (MR) imaging studies of the brain. Craniometric measurements used in radiologic assessment of CVJ anomalies include the Chamberlain line, Wackenheim clivus baseline, Welcher basal angle, and atlantooccipital joint axis angle. Most anomalies of the occiput are associated with decreased skull base height and basilar invagination, the latter being a primary developmental anomaly in which the vertebral column is abnormally high and prolapsed into the skull base. Occiput anomalies inc

Pseudospread of the atlas: false sign of Jefferson fracture in young children.

Jefferson fractures are rare prior to teen-age. Three young children examined after trauma exhibited the characteristic spread appearance of the atlas, but fractures were excluded radiographically and clinically. A retrospective study demonstrated a similar appearance, termed "pseudospread," in most children aged 3 months to 4 years, including over 90% during the second year. Pseudospread results from a discrepancy between the "neural" growth pattern of the atlas and the "somatic" pattern of the axis. An "atlas spread index" is defined and a normal range presented. When an atlas fracture is su

Nonreducible rotational head tilt and lateral mass collapse. A prospective study of frequency, radiographic findings, and clinical features in patients with rheumatoid arthritis.

Of 126 hospitalized, but otherwise unselected, patients with rheumatoid arthritis, 12 (10%) had a persisting nonreducible rotational head tilt deformity (NRRHT). These 12 were grouped with an additional 12 similar subjects, previously found to have the same condition, and all 24 were compared with the remaining 114 who did not have head tilt. Patients with NRRHT differed significantly from the others in that the former were more likely to have a destructive peripheral arthritis, headache, limited neck motion, and various cervical subluxations. All 24 subjects with NRRHT had predominately unila

Long-term corticosteroids and cervical subluxation in non-rheumatoid patients.

Forty-five non-rheumatoid patients with chronic chest disease were examined radiographically for evidence of cervical subluxation. Twenty-six patients had received continuous corticosteroids for periods of between two and 20 years. The remainder (19 patients) had never been treated with steroids and acted as controls. There was no difference in atlanto-axial distance between the steroid-treated and control groups and in no patient did this measurement exceed 3 mm. However, subaxial subluxation was significantly increased in the steroid-treated group, directly related to the duration of steroid

Fractures of the atlas associated with fractures of the odontoid process and transverse ligament ruptures.

One case of fracture of the atlas associated with rupture of the transverse ligament and three cases of atlantal fracture with associated fractures of the odontoid process are reported. In such cases, halo immobilization is recommended until the atlantal fracture has healed, followed by atlanto-axial fusion for instability of the cervical spine.

Atlanto-axial rotatory fixation. (Fixed rotatory subluxation of the atlanto-axial joint).

In seventeen cases of irreducible atlanto-axial rotatory subluxation (here called fixation), the striking features were the delay in diagnosis and the persistent clinical and roentgenographic deformities. All patients had torticollis and restricted, often painful neck motion, and seven young patients with long-standing deformity had flattening on one side of the face. The diagnosis was suggested by the plain roentgenograms and tomograms and confirmed by persistence of the deformity as demonstrated by cineroentgenography. Treatment included skull traction, followed by atlanto-axial arthrodesis

A new method of screening for vertical atlantoaxial dislocation.

A new method for the screening and grading of vertical atlantoaxial dislocation is described. The Sakaguchi-Kauppi method is based on a pathologic process in the atlantoaxial joints and the grading system gives a reproducible (interobserver coefficient of variation = 0.062) tool for the screening of vertical dislocation and is also applicable to patients for whom earlier radiographs are not available. There were differences between the results obtained using Redlund's method and the new Sakaguchi-Kauppi method (chi 2 = 44.7, p less than 0.001). This difference stems from the variations in the

[Neurologic symptoms and their dynamics in contusions of the spinal cord of mild and moderate severity at the level of the 1st and 2d cervical vertebrae].

The results of the study of 154 patients with an acute trauma of the cervical portion of the vertebral column showed that fractures, dislocations and fractures/dislocations of the first-second cervical vertebrae accounted for 10.4% of all the studied cases. In mild contusion of the first-second segments the leading clinical symptoms in the acute period were pareses of the arms and paralyses of hands with retained reflexes and insignificant impairments of sensitivity in fingers, i.e., the level of clinical manifestations did not correspond with the conventional segmento-metameric innervation. High cervical spine and craniocervical junction injuries in fatal traffic accidents: a radiological study.

Postmortem radiographic examinations in 312 victims of fatal traffic accidents were analyzed with respect to injuries to the cervical spine. Most fractures and dislocations in this group were found to involve the craniocervical junction and the upper two cervical segments. Over half were flexion injuries, one-fifth caused predominantly by extension. Flexion and extension views were found to be helpful in demonstrating the instability of many injuries.

Analysis of surgical strategies and efficacy in the treatment of Os odontoideum with atlantoaxial dislocation.

BACKGROUND: There are many classification systems for atlantoaxial dislocation (AAD). Among these systems, the definitions of irreducible AAD remain vague, and its treatments are not unified. OBJECTIVE: To explore the surgical strategies and efficacy for the treatment of os odontoideum (OO) with AAD. METHODS: The clinical data of 56 OO patients with AAD who underwent surgery from January 2017 to June 2021 were retrospectively analyzed. AAD was classified into four types, Type I and type II were treated with posterior fixation and fusion. Type III received posterior fixation and fusion after

Vertebral Artery Dissection in the Setting of Unstable Os Odontoideum: A Case Report.

CASE: A healthy 5-year-old boy presented with a gradual onset of headaches and acute global right-sided weakness over 10 days. The work-up revealed unstable os odontoideum leading to multiple posterior circulation infarcts with vertebral artery dissection. He underwent antiplatelet therapy, cervical collar immobilization, and delayed occiput to C2 posterior spinal fusion and instrumentation with iliac crest autograft. At 2-year follow-up, the patient had a solid fusion mass, appropriate cervical alignment, and was without neurologic sequelae. CONCLUSION: This case adds to a sparse body of lit
