Optimizing acne therapy with unique vehicles.

The science of cutaneous drug delivery is focused on overcoming the major force of resistance to drug penetration and permeation-the stratum … he unique physical and optical properties of micro/nano encapsulation of known therapeutics such as benzoyl peroxide and tretinoin allow for both improved efficacy while minimizing issues of compliance and adverse events. Vehicles that offer both inherent biological reactivity and permeation enhancement have also been shown improvement over the current armament of …

The efficacy of a bioactive moisturizer in mitigating side effects of acne therapies: a randomized, investigator-blind, split-face trial.

Acne treatments often cause mucocutaneous side effects. This study compared a bioactive moisturizer (containing 4-t-butylcyclohexanol, licochalcone A, ceramide, and panthenol) with a traditional moisturizer in Asian patients using adapalene/benzoyl peroxide gel or oral isotretinoin. Forty participants applied each moisturizer to one side of the face. Assessments at weeks 0, 2, and 4 included hydration, trans-epidermal water loss (TEWL), pH, erythema, acne lesion counts, Investigator Global Evaluation of Acne (IGEA), and patient satisfaction. Hydration increased on both sides, with greater impr…

Cosmeceuticals in acne vulgaris: from mechanism of action to clinical application.

The use of cosmeceuticals in acne vulgaris is becoming increasingly prevalent with many over-the-counter formulations becoming part of patients' routine skincare. Cosmeceuticals are often used successfully as an adjunctive therapy to reduce the side effects of traditional prescriptions and improve compliance. This is a review to support the use of cosmeceuticals in acne, including retinol, retinaldehyde, benzoyl peroxide (BPO), azelaic acid, beta hydroxy acids, alpha hydroxy acids, niacinamide, zinc, tea tree oil and green tea. There is most evidence, in human clinical trials, to support the u…

[Canine pyoderma: clinical presentation, diagnosis, and topical treatment - a review].

Canine pyoderma is a common dermatological condition that is still frequently treated with systemic antibiotics in small animal practice. In light of increasing antibiotic resistance, particularly due to methicillin-resistant staphylococci, topical therapy is gaining importance. Numerous studies have now demonstrated the efficacy of topical treatments, especially for superficial pyodermas. Chlorhexidine, sodium hypochlorite, benzoyl peroxide, and ethyl lactate are among the most commonly used agents, with chlorhexidine showing the strongest antibacterial effect. In addition to its antimicrobia…

Hidradenitis Suppurativa: Inside and Out.

Hidradenitis suppurativa is a chronic, disabling, suppurative disease characterized by deep tender subcutaneous nodules; complicated by fibr … good hygiene. Topical antibiotics, like 1% clindamycin, have shown to give good results along with benzoyl peroxide wash. Orally cocktail of antibiotics can be given, though biologicals remain the best treatment option. Surgical excision can be done in later stages and in recalcitrant cases.

Treatment of acne vulgaris during pregnancy and lactation.

Acne vulgaris is a common problem encountered by pregnant and lactating women. Unfortunately, in clinical practice, treatment is often not optimized as a result of the lack of safety data and unified recommendations on the use of the various anti-acne therapies. In this narrative review, current data on their safety is summarized. We recommend the use of topical medications as first-line treatment for acne vulgaris in pregnant and lactating women. These include antibiotics (erythromycin, clindamycin, metronidazole and dapsone), benzoyl peroxide, azelaic acid and salicylic acid. Oral agents and…

Are therapeutic effects of antiacne agents mediated by activation of FoxO1 and inhibition of mTORC1?

Acne pathogenesis has recently been linked to decreased nuclear FoxO1 levels and increased mTORC1 activity. This hypothesis postulates that antiacne agents either enhance nuclear FoxO activity or inhibit mTORC1. Benzoyl peroxide (BPO), by activation of oxidative stress-inducible kinases, increases nuclear FoxO levels promoting Sestrin3-mediated AMPK activation. Furthermore, BPO-derived ROS may activate AMPK via ataxia-telangiectasia mutated. Isotretinoin and all-trans retinoic acid may stimulate FoxO gene expression. Doxycycline may enhance FoxOs nuclear retention by inhibiting the expression …

Selection of therapy for acne vulgaris: balancing concerns about antibiotic resistance.

The widespread use of long-term antibiotics to treat acne and rosacea has resulted in increased Propionibacterium acnes resistance to antibiotics. Although resistance may decrease treatment effectiveness in some cases, it also is clear that antimicrobial activity is not the only role antibiotics play in acne management. The anti-inflammatory activity of antibiotics contributes to antipropionibacterial efficacy. In addition, the use of benzoyl peroxide (BPO) in combination with antibiotics to reduce resistant populations of propionibacteria is widely advocated, as P acnes has not developed resi…

Sulfoxidation of pyrimidine thioate derivatives and study their biological activities.

In a quest to innovate biologically active molecules, the benzoylation of 4,6-dimethylpyrimidine-2-thiol hydrochloride (1) with benzoyl chloride derivatives was employed to produce a series of pyrimidine benzothioate derivatives (2-5). Subsequent sulfoxidation of these derivatives (2-5) using hydrogen peroxide and glacial acetic acid yielded a diverse array of pyrimidine sulfonyl methanone derivatives (6-9). In parallel, the sulfoxidation of pyrimidine sulfonothioates (10-12) yielded sulfonyl sulfonyl pyrimidines (13-15), originating from the condensation of compound 1 with sulfonyl chloride d

Optimizing topical combination therapy for the treatment of acne vulgaris.

Given the multifactorial and complex contributors to acne development, combination therapy is standard of care. By addressing multiple pathogenic factors, combination therapy provides a quicker and more efficacious treatment outcome than monotherapy. Topical retinoids normalize follicular keratinocyte differentiation and are anti-inflammatory. Their use is limited by the potential for cutaneous irritation. Antimicrobials reduce Propionibacterium acnes colonization on the skin and reduce the bacteria's proinflammatory effects. Topical antibiotics and benzoyl peroxide (BPO) are commonly employed…

Clinical evidence for washing and cleansers in acne vulgaris: a systematic review.

PURPOSE: Washing and over-the-counter cleansers are common interventions in acne vulgaris (AV), but the clinical evidence for their benefit is poorly understood. This systematic review presents clinical studies of washing and cleanser efficacy in acne vulgaris to guide treatment recommendations of dermatologists. MATERIALS AND METHODS: We surveyed English-language articles indexed in MEDLINE (1951-March 2017) and EMBASE (1974-March 2017). Articles were required to be prospective studies of a single over-the-counter cleanser or washing intervention in AV with an objective AV outcome measuremen

Antibiotic resistance in the topical treatment of acne vulgaris.

Topical antimicrobial agents are the first line of treatment in mild to moderate acne vulgaris. The primary pathogenic agent implicated in the development of inflammatory acne is Propionibacterium acnes. P acnes also may play a secondary role in noninflammatory acne or comedogenesis. Over the past 20 years, concern has grown about the gradual worldwide increase in the prevalence of antibiotic-resistant P acnes strains. Factors associated with the development of resistant P acnes following treatment with topical antibiotics, clinical relevance of antibiotic resistance, and strategies to reduce
