The rapidity of progression of aortic stenosis in patients with congenital bicuspid aortic valves, and its relation to aging and valve anatomy are not well known. To elucidate these aspects, 75 patients aged 15 to 76 years were examined by echocardiography. Aortic valve sclerosis began from the second decade, the sclerotic index progressing with age (r = 0.72; p < 0.0001). Aortic valve calcium was noted from the fourth decade. Aortic valve pressure gradient increased approximately 18 mm Hg each decade, concomitant with progression of valve sclerosis (r = 0.78; p < 0.0001). Progression of cusp …

In 388 patients with severe isolated aortic valve disease bicuspid aortic valves were found in 45% of patients with aortic stenosis and 24% of patients with aortic regurgitation. There was a history of rheumatic fever in 16% and of endocarditis in 7% of these patients. In a group of 110 patients with severe combined aortic and mitral valve disease, 50% had a history of rheumatic fever and bicuspid valves were found in only 12% of the cases. Severe aortic valve disease based on bicuspid valves was most frequently found in the age group 50-69 years. Bicuspid aortic valve is today the most freque…

Ascending thoracic aortic aneurysm formation differs fundamentally between patients with bicuspid (BAV) and tricuspid (TAV) aortic valve phenotypes, yet current clinical management largely relies on shared diameter thresholds, although clinical and scientific data fail to correlate with diameter and risk of rupture. Over the past two decades, histological, biochemical and biomechanical studies have revealed unique differences between dilated tissue properties from patients with BAV- and TAV-associated idiopathic degenerative ascending aortic aneurysms even at similar aortic diameters. This rev…

When a Transcatheter heart valve (THV) is implanted in a bicuspid aortic valve (BAV), the interventional valve may experience elliptical deformation, thereby reducing its durability. In this study, a novel Asymmetric Artificial Interventional Valve (AAIV) was designed. The deployment of the AAIV in the BAV was simulated and compared to that of a THV. The synergistic effects of the AAIV (0.1, 0.2, and 0.3) eccentricity and annulus eccentricity (0.1, 0.2, and 0.3) on the deployment of the interventional valve were studied. Besides, the influence of native leaflet calcification on the deployment …

OBJECTIVES: Ascending aortic diameter is routinely used for clinical decision-making in bicuspid aortic valve (BAV) patients but is an imperfect predictor of aortic growth and outcomes. This study investigates the potential of additional imaging parameters in predicting ascending aortic diameter growth in BAV patients.
METHODS: Baseline imaging characteristics from cardiothoracic CT angiograms (CTA) performed from Jan 2004 to Apr 2020 were reviewed. The median imaging follow-up duration was 5.4 years (IQR 4.7-6.0 years). Parameters included BAV morphology, aortic valve calcium score (AVCS), a…

BACKGROUND: While athletic patients may exhibit bradycardia, significant exertional symptoms in the setting of a bicuspid aortic valve (BAV) warrant further investigation.
CASE SUMMARY: A 36-year-old male triathlete presented with progressive exercise intolerance, fatigue, and bradycardia. Electrocardiogram demonstrated complete atrioventricular block (junctional ventricular escape: 52 beats/min). Echocardiography revealed a moderately calcified BAV with mild to moderate stenosis and preserved ventricular function. Cardiac imaging demonstrated a Sievers type 0 valve with asymmetric extravalvu…

Having a bicuspid aortic valve (BAV) is the most common congenital heart disease, affecting 0.5%-2% of the population, with significant heterogeneity in clinical presentation, complications, and outcomes. Hemodynamic disturbances, including wall shear stress (WSS), eccentric flow, helical flow and energy turbulence, are critical in the development and progression of BAV-associated aortopathy, which is characterized by ascending aortic dilation, aortic aneurysm, and dissection. The interplay between genetic factors and hemodynamic abnormalities further complicates disease mechanisms, influencin…

BACKGROUND: While bicuspid aortic valve and coarctation commonly coexist, diagnosis in the eighth decade of life is exceptionally rare.
CASE SUMMARY: A 78-year-old male patient with a history of hypertension was admitted with a 2-year history of progressive dyspnea on minimal exertion. After diagnostic workup, he was diagnosed with severe bicuspid aortic stenosis, aortic coarctation, and severely impaired left ventricle systolic function (left ventricle ejection fraction 12%). Due to high surgical risk, a stepwise percutaneous strategy was adopted, starting with coarctation stenting which imp…

Bicuspid aortic valve (BAV) is a congenital malformation that predisposes individuals to thoracic aortic aneurysm (TAA), with endothelial dysfunction playing a pivotal role in its pathogenesis. Endothelial cell senescence is a hallmark of endothelial dysfunction, yet direct evidence linking endothelial senescence to BAV-TAA has not been established. In this study, we generated induced pluripotent stem cells (iPSCs) from both BAV-TAA patients and healthy controls, subsequently differentiating them into endothelial cells (iECs). Our findings revealed that BAV-TAA-iECs exhibited senescence phenot…

BACKGROUND: The aim of this study is to investigate the influence of aortic diameter on hemodynamic environment characteristics in patient with the bicuspid aortic valve (BAV) and dilated ascending aorta (AAo).
METHOD: In this study, an MRI of one BAV patient with 4.5 cm AAo was collected and numerical model was constructed. Based on the images, three additional anatomical models were constructed with different AAo sizes of 4.0 cm, 5.0 cm and 5.5 cm respectively, while the size and the geometry of other parts were fixed. Then hemodynamics in these four models was simulated numerically and the…

Bicuspid aortic valve (BAV) is the most common congenital heart disease, affecting 0.5-2% of the population and often leading to early aortic valve degeneration. While surgical aortic valve replacement (SAVR) remains the gold standard for treating severe bicuspid aortic stenosis (AS), transcatheter aortic valve implantation (TAVI) is emerging as a viable alternative in selected BAV anatomies. Initial experiences with first-generation transcatheter heart valves (THVs) showed the feasibility of this technique, but were associated with lower device success rates and higher complications, such as …

INTRODUCTION: In light of the longstanding assumption that ascending thoracic aortic aneurysms (ATAAs) associated with bicuspid aortic valves (BAVs) exhibit greater biomechanical vulnerability than those associated with tricuspid aortic valves (TAVs), we characterized and compared the delamination resistance of BAV- and TAV-ATAAs.
METHODS: Whole ATAAs were intraoperatively harvested from patients undergoing elective repair. Specimens were sectioned into circumferential and longitudinal strips from all quadrants. Delamination testing, simulating clinical aortic dissection, was performed within…

Aortic valves with bicuspids have two rather than three leaflets, which is a congenital heart condition. About 0.5-2% of people have a bicuspid aortic valve. Blood flow through the aorta is commonly believed to be laminar, although aortic valve disorders can cause turbulent transitions. Understanding the impact of turbulence is crucial for foreseeing how the disease will progress. The study's objective was use large eddy simulation to provide a thorough analysis of the turbulence in bicuspid aortic valve dysfunction. Using a large eddy simulation, the blood flow patterns of the bicuspid and tr…

BACKGROUND: Prior to the recent release of appropriate use criteria for imaging valvulopathies in children, follow-up of valvular lesions, including isolated bicuspid aortic valve, was not standardised. We describe current follow up, treatment, and intervention strategies for isolated bicuspid aortic valve with varying degrees of stenosis, regurgitation, and dilation in children up to 18 years old and compare them with newly released appropriate use criteria.
METHODS: Online survey was sent to members of the American Academy of Pediatrics Section on Cardiology and Cardiac Surgery and PediHear…

Patients with a bicuspid aortic valve (BAV) are at increased risk of valvular regurgitation compared to their counterparts with a tri-leaflet aortic valve. There is now increasing emphasis to offer BAV repair to mitigate the risks of prosthesis-related complications, including thromboembolism, hemorrhage and endocarditis, as well as structural valve deterioration and future reoperation with conventional valve replacement, particularly in younger populations. Furthermore, over the preceding two decades, our greater understanding of the functional anatomy of the BAV, pathophysiological mechanism…
