Alternatives to Transfusion.

OBJECTIVES: We discuss two main categories of blood substitutes: perfluorocarbons and hemoglobin-based oxygen carriers. METHODS: We provide a review of the notable products developed in both categories and include their attributes as well as their setbacks. RESULTS: We contribute a case report tothe growing literature of the successful use of Sanguinate. CONCLUSIONS: We find that artificial oxygen carriers are an attractive field of research because of the practical limitations and the multitude of potential complications that surround human blood transfusions.

Volume replacement and microhemodynamic changes in polytrauma.

Though fluid administration is one of the most basic concepts in resuscitation, there is ongoing controversy and continuing research on the definition of the ideal fluid for resuscitation of trauma and hemorrhage and for intraoperative volume support. In general, crystalloids and colloids, as well as blood, blood substitutes and oxygen therapeutics, are available. This report briefly revisits the physiological mechanisms underlying resuscitation with crystalloids and colloids, emphasizing colloid-supplemented resuscitation with hypertonic saline. Finally, potential applications of oxygen thera…

The status of hemoglobin-based red cell substitutes.

Red cell substitutes are currently under development for use in a variety of surgery and trauma-related clinical conditions. The need for artificial oxygen-carrying fluids continues to be driven by the shortage of donor blood, the complex logistics of blood banking, the risk of virally transmitted diseases, current transfusion practices, and the projected increased demand for blood products in the future. The effort to develop a replacement for the red cell component has evolved over the last century and has presented a number of significant challenges including safety and efficacy concerns. R

[Acute hemorrhage. View on the problem].

Acute hemorrhage is a main cause of reduction of blood oxygen capacity. The main aim of correction of sequels of acute hemorrhage is to maintain effective gas exchange by restoring central circulation and microcirculation, the rate of diuresis, by normalizing water-salt exchange, to eliminate anemia, hypoproteinemia, and acute blood coagulability disorders. The values of oxygen budget with calculated oxygen delivery and consumption and those of hemoglobin and hematocrit which are of great value only after recovery of circulating blood volume are considered to be major indications for hemotrans

Organ engineering--combining stem cells, biomaterials, and bioreactors to produce bioengineered organs for transplantation.

Often the only treatment available for patients suffering from diseased and injured organs is whole organ transplant. However, there is a severe shortage of donor organs for transplantation. The goal of organ engineering is to construct biological substitutes that will restore and maintain normal function in diseased and injured tissues. Recent progress in stem cell biology, biomaterials, and processes such as organ decellularization and electrospinning has resulted in the generation of bioengineered blood vessels, heart valves, livers, kidneys, bladders, and airways. Future advances that may

Surgery in hemophilic patients.

Surgery in hemophilic patients was performed under proper factor-replacement therapy, and a wide variety of orthopaedic procedures (twenty-six in seventeen patients) were done with good results. There were no complications in any of the patients since the advent of concentrates in 1966. Multiple procedures under one anesthetic are recommended because the complications associated with repeated administration of blood substitutes are minimized.

[The use of biochemical characteristics of cadaveric blood in the forensic diagnosis of lethal poisoning].

The purpose of the work is the development of mathematical models in the forensic diagnosis of poisonings by the main groups of toxicologically important substances, on the basis of biochemical characteristics of blood. The most informative forensic and biochemical indicators of cadaveric blood used to detect lethal poisoning are the urea content, total protein content, and the ratio of urea to creatinine. Mathematical models of poisoning can be used to diagnose poisoning with narcotic drugs, psychotropic substances and substitutes of ethyl alcohol.

Clinical and CT Analysis of Lumbar Spine Arthrodesis: β-Tricalcium Phosphate Versus Demineralized Bone Matrix.

BACKGROUND: Bone graft substitutes have been developed to circumvent donor site morbidity associated with iliac crest bone graft, but sparse literature compares the efficacy of various substitutes. Two commonly used bone graft substitutes used in lumbar fusion are β-tricalcium phosphate (BTP) and demineralized bone matrix (DBM). METHODS: A retrospective review of patients who underwent instrumented posterolateral lumbar fusion was conducted by a single surgeon from January 2013 to December 2016. Patients were divided into two groups based on whether DBM or BTP as graft in conjunction with loc

[Limits of hemodilution in the treatment of hemorrhagic shock].

Clinical analysis of 168 cases of hemorrhagic shock of the 230 cases of hemorrhage and injuries admitted to the 2nd Surgical Clinic of Timisora between 1970 and 1973, showed that the administration of blood substitutes (crystalloid and colloid solutions) may be extended with good results in the management of hemorrhagic shock up to a hematocrit of 20%, respectively 7 g Hb. This treatment implies a careful selection of the cases, based upon objective criteria (listed in the text), so that the none administration of blood should not involve any risks. Even in the cases in which blood tranfusion …

Changes of volume substitution for the treatment of hemorrhage within 10 years and their costs.

Whereas 10 years ago blood loss was substituted by whole blood application together with colloidal or crystalloid substitutes, this behavior changed to a distinct therapy according to the changed laboratory parameters with the application of red cell concentrates together with fresh frozen plasma (FFP). By means of two representative operations (resection of sigma and hemicolectomy) the behavior of volume application and the substitution of blood and blood components were controlled for the years 1980 and compared with the behavior of 1990. In addition to that the costs for both behaviors were

Developments in the therapeutic landscape for dry eye.

INTRODUCTION: Dry eye disease (DED) is a chronic condition that can markedly impair visual function and quality of life. While tear substitutes remain the conventional first-line therapy, expanding knowledge of DED has opened the door to treatments designed to act on distinct pathogenic mechanisms. AREAS COVERED: An extensive literature review using PubMed, Scopus, and Web of Science was conducted to evaluate both established and emerging therapies for DED. This review encompasses conventional tear substitutes and their properties, immune-modulating agents, blood derived products and other bi

Effects of a Prolonged-Release Protein Substitute on 24-h Phenylalanine and Tyrosine Profiles in Phenylketonuria: A Randomized Crossover Study.

BACKGROUND/OBJECTIVES: Achieving optimal metabolic control in phenylketonuria (PKU) remains challenging, particularly in adolescents and adults. Conventional free amino acid-based protein substitutes are absorbed rapidly, whereas prolonged-release formulations are designed to provide a more gradual amino acid release profile. Whether this translates into more favorable 24 h Phe and Tyr profiles and improved short-term biochemical parameters of metabolic control remains uncertain. This study evaluated whether a prolonged-release amino acid-based protein substitute (PR-AA) could improve 24 h met

An ex vivo method to evaluate vasoactivity induced by hemoglobin-based oxygen carriers in resistance vessels.

Red blood cell substitutes offer a solution to the problem of blood shortage and side effects of blood transfusion. Hemoglobin-based oxygen carriers (HBOCs) are one of the promising substitutes for red blood cells. Vasoactivity, which refers to the side effect of HBOCs that causes vasoconstriction and subsequent hypertension, limits the clinical application of HBOCs. In this study, an ex vivo method for the evaluation of vasoactivity induced by HBOCs was established based on isolated rat mesenteric artery vessels and the DMT120CP system. The DMT120CP system, equipped with a flowmeter, permits

Extracorporeal Barbotage Detoxification of the Blood Plasma.

We studied the efficiency of bubbling air as a method of detoxification of the blood plasma and its mixtures with plasma substitutes from CCl4. The efficiency of detoxification depended on the characteristics of the obtained foams: foam forming ability, frequency rate, dispersion, stability of a foam skeleton, syneresis, etc. A hardware-software complex for extracorporeal blood plasma detoxification was developed and the programs for the control and regulation of this process were tested. Physical and mathematical models of toxicant excretion from the body during extracorporal blood detoxifica

Rheologic effects of plasma substitutes used for preoperative hemodilution.

This study was designed to compare the influence of various plasma substitutes, administered for preoperative hemodilution, on blood rheology. We studied 40 patients, ASA grade I, who underwent elective facial reconstructive surgery and received 4% albumin (n = 10), 3.5% dextran 40 (n = 10), gelatin (n = 10), or hydroxyethyl starch (HES) (n = 10). Ten patients, undergoing the same surgical procedure without hemodilution, were chosen as controls. After hemodilution, hematocrit was decreased approximately 30%. Fibrinogen decreased in all tested groups except in the gelatin group. Plasma viscosit

[Hemodilution in the treatment of cerebrovascular disorders].

Haemodilution includes intravenous administration of plasma-replacing solutions and/or blood letting for changing the rheological properties of blood, and for improvement of microcirculation. Three methods of haemodilution various plasma substitutes are given: dextran, hydroxyethyl starch (HES), electrolyte solutions (e.g. Ringer's solution, normal saline, 5% glucose, colloidal plasma, protein solutions), plasma, albumins. The controversial results of clinical trials of the method for therapeutic purposes are discussed. In the light of the reports as yet, it seems that the best results are obt
