On 119 brain stem halves the nerve root fasciculi of the intermediary nerve were examined in respect of their position relative to the facial and vestibulocochlear nerves. The following variants were seen: 1.) the intermediary nerve lies between the facial and vestibulocochlear nerves as stated in the textbooks, in 32.9% of the cases; 2.) in 18.5% of the cases it is entirely apposited to the vestibulocochlear nerve from the beginning and branches off from that nerve only later; 3.) part of the nerve root fasciculi are apposited to the vestibulocochlear nerve and the others originate between th…

The distribution of somatosensory evoked potentials (SEPs) recorded from the brain-stem surface was studied to investigate their generator sources in 14 patients during surgical exploration of the posterior fossa. Two distinct SEPs of different morphologies and electrical orientation were obtained by median nerve stimulation. A small positive-large negative-late prolonged positive wave was recorded from the cuneate nucleus and its vicinity. There was a phase-reversal between the cuneate nucleus and the ventral surface of the medulla, depicting a dipole for dorso-ventral organization. From the

Fibromuscular dysplasia of the basilar artery is a rare and usually asymptomatic vascular disease with only 5 cases reported in the literature. A thirty-eight-year-old man who developed ischemic neurologic symptoms of the brain stem due to dysplasia of the basilar artery is presented, and the clinical features, radiology, treatment, and prognosis of this disease are discussed in correlation with the current data.

Magnetic resonance imaging (MRI) of wallerian degeneration of the corticospinal tract in the brain stem was studied in 25 patients with chronic supratentorial vascular accidents. In the relatively early stages, at least three months after ictus, increased signal intensities in axial T2-weighted images--with or without decreased signal intensities in axial T1-weighted images--were observed in the brain stem ipsilaterally. In later stages, at least six months after ictus, shrinkage of the brain stem ipsilaterally--with or without decreased signal intensities--was clearly observed in axial T1-wei…

A healthy 26-year-old man, without initially presenting fever, rapidly developed a focal right pontomedullary deficit associated with an aseptic lymphocytic meningitis. The diagnosis of Listeria infection was confirmed by blood cultures. CT and MRI demonstrated an abscess extending from the superior cerebellar peduncle to the lateral portion of the medulla. Immunological controls gave no indication of deficiency. With ampicillin therapy, started on the 5th day, clinical recovery was almost complete, but a soft palate right paresis persisted as the unique sequel. Antibiotic therapy was maintain

Changes in the excitatory cardiovascular response (heart rate, arterial blood pressure, left ventricular pressure, and LV dP/dt as an index of myocardial contractility) resulting from electrical stimulation of the cerebellar fastigial nucleus (FN) were recorded after placement of DC or radio-frequency lesions or after microinjections of kainic acid into brain stem areas that receive FN projections and have been shown to be involved in central cardiovascular control. FN-induced increases in heart rate, blood pressure, and contractility were reduced or abolished by lesions made in the restiform …

Antiepileptic drugs probably act by preventing the spread of the abnormal paroxysmal activity from the epileptogenic focus to surrounding normal neurons. An investigation of the mechanism of action of established anticonvulsant drugs on normal neuronal systems may therefore offer useful insights into the pathogenesis of the seizure disorders that these drugs serve to control. Antiabsence drugs (ethosuximide, valproate) depress reticular inhibitory pathways. Drugs effective against generalized tonic-clonic seizures (phenytoin, carbamazepine, valproate) depress reticular excitatory pathways. Dru

A pharmacological study was carried out of a case of severe insomnia following brain-stem lesions; several polygraphic controls were used. Initially total duration of sleep was brief (less than 4 h) with a high REM/NREM ratio and a short paradoxical sleep (PS) latency. In addition, periodic breathing and tremor were observed. Slow injection of delta-sleep-inducing peptide (DSIP) improved sleep both quantitatively and qualitatively, although PS latency remained short. These effects were reversible. The effects of 5-HTP + benzerazide, of L-DOPA + benzerazide (Modopar) and of clonazepam (Rivotril

A case of brain stem infarction after chiropractic manipulation of the cervical spine is presented. Proposed mechanisms and sites of possible arterial injury are discussed. A diagnosis of vertebral artery occlusion was made using conventional brachial angiography. Digital intravenous angiography, a relatively new and less invasive vascular imaging technique which was used as an adjunct for evaluating the remainder of the cervicocephalic vessels, documented the vertebral occlusion. Chiropractic manipulation, which is increasing in popularity, may be a cause of potentially devastating neurologic…

A case of sudden bilateral hearing loss in a diabetic patient is reported. Acoustic-vestibular examination indicated a lesion localized in the brain stem although no other neurological abnormality was found. Postmortem examination demonstrated severe vascular changes of the brain stem. Areas of fresh infarction and necrosis were found localized to the cochlear and vestibular nuclei. The case demonstrates that central vascular disorders may be the cause of sudden severe hearing loss even without other neurological symptoms or signs.

As a part of a continuing study of the relationship between age and neuronal population within the human brain stem, the nucleus of the trochlear nerve was examined in a total of 20 brains from newborn to 87 years of age. The cell count in the newborn was 1,830, and in the 87-year-old 2,010, while the mean number for all the specimens examined was 2,115. It is apparent that there is no cell loss in this nucleus in the normal aging process, which is in accord with the findings in five other brain stem nuclei examined by various investigators. This is in contrast to the findings in cerebral and …

Auditory evoked brain stem potentials were measured in a patient with occlusion of the basilar artery about 7 mm above its origin, resulting in the "locked-in" syndrome due to infarction at the junction of the lower one third and upper two thirds of the pons. The first three waves of the evoked response originating from the acoustic nerve and auditory nuclei in the caudal pons were normal in wave form, peak latency, and voltage level. Waves IV and V, generated in the region of the lateral lemniscus and inferior colliculus in rostral pons and caudal midbrain, demonstrated prolonged peak latency…

The syndrome of persistent facial myokymia and spastic facial contracture is a rare sign of pontine glioma. In none of the previously published cases has this syndrome been reported in association with a nonneoplastic lesion. A patient is described in whom facial myokymia and contracture were caused by a pontine tuberculoma.

In order to obtain basic evidence on the neuronal organization and localization of the central respiratory mechanism in the brain stem of the cat, the following experiments were undertaken: reexamination of the physiological meaning of apneusis and the gasp; differences in stability of spontaneous burst activity of respiratory neurons in the brain stem reticular formation; changes of spontaneous firing pattern of bulbar respiratory neurons in the isolated brain stem of the cat; changes of firing pattern of a pontine and a bulbar respiratory neuron during continuous recording of both unitary ac…
