[The early postoperative period in patients with tumors of the posterior cranial fossa].

The studies carried out during an early postoperative period in patients with posterior fossa tumors were made to analyse the role of the degree and the level of brain stem damages in formation of cerebral reactions. The three groups of cases were selected in accordance with the course of the postoperative period: 1.1-with exitus letalis (irreversible damages); 1.2-rehabilitation in persistent focal damages; 2-restoration with functional safety of brain stem. Differences of the integral cerebral reactions (the state of consciousness, emotional-affective disorders, awakening state) as well as t

Deep Learning for Survival Prediction in Glioblastoma: Time-dependent Model Interpretability Using MRI, Clinical, and Molecular Data.

Purpose To develop a multimodal model for survival prediction and time-dependent model interpretability in glioblastoma by integrating preoperative MRI with clinical and molecular variables. Materials and Methods This retrospective multicenter study included two institutional cohorts (February 2007-December 2024) and two public external test sets. A deep learning-based prognostic index (DPI) was generated from preoperative multiparametric MRI using a vision transformer. The DPI was integrated with clinical variables (age, sex, Karnofsky performance status, extent of resection), molecular marke

Surgical treatment of brainstem cavernous malformations in pregnancy: a retrospective single-center series and systematic review of the literature.

OBJECTIVE: Brainstem cavernous malformations (BSCMs) are rare neurovascular lesions, and they are especially rare during pregnancy. Pregnancy was not considered in the international consensus on patient-related factors that influence surgical treatment decisions for BSCMs. Therefore, the authors sought to analyze the association between pregnancy and surgical outcomes of BSCMs. METHODS: The authors retrospectively identified women who were pregnant and underwent microsurgical treatment for a BSCM during a period of 37 years. Pregnant women who underwent surgery antepartum were compared with t

Surgery of brainstem cavernous malformations: surgical nuances and outcomes of a monocentric series of 34 patients.

BACKGROUND: Brainstem cavernous malformations (BCM) constitute one of the most controversial and challenging neurological pathologies: both natural course and surgical manipulation can lead to severe neurological symptoms by direct compression or following hemorrhage of this highly eloquent brain region. METHODS: The vascular section of the prospectively collected neurosurgical database of our center (2011-2023) was retrospectively reviewed, seeking all patients operated on for a sporadic BCM. Clinical, radiological and surgical data, operative records and videos were analyzed by independent

Dorsally exophytic glioblastoma of the pons.

Brainstem gliomas are rare tumours in adults, accounting for only 1%-2% of all intracranial gliomas. They are recognised as a heterogeneous group, in which most are malignant tumours. Brainstem gliomas are classified into four major groups according to the growth pattern on imaging, namely diffuse, focal, exophytic and cervicomedullary. Such a classification system is also useful for surgical decision making. The exophytic variant is extremely rare having anecdoctal reports in the literature. We report the case of an adult patient affected by an exophytic glioblastoma of the pons, which was su

Gamma Knife surgery for patients with brainstem metastases.

OBJECT: Because brainstem metastases are not deemed resectable, stereotactic radiosurgery (SRS) is the only treatment modality expected to achieve a radical cure. The authors describe their treatment results, focusing particularly on how long patients can survive without neurological deterioration following SRS for brainstem metastases. METHODS: This was an institutional review board-approved, retrospective cohort study in which the authors pulled from their database information on 2553 consecutive patients with brain metastases who underwent Gamma Knife surgery (GKS) at the Mito GammaHouse b

Topotecan combined with myeloablative doses of thiotepa and carboplatin for neuroblastoma, brain tumors, and other poor-risk solid tumors in children and young adults.

Topotecan appears to be relatively unaffected by the most common multidrug resistance mechanisms, may potentiate cytotoxicity of alkylators, has good penetration into the central nervous system, is active against a variety of neoplasms, and has myelosuppression as its paramount toxicity. We present our experience with a myeloablative regimen that includes topotecan. Twenty-one patients with poor-prognosis tumors and intact function of key organs received topotecan 2 mg/m2 by 30-min intravenous (i.v.) infusion on days -8, -7, -6, -5, -4; thiotepa 300 mg/m2 by 3 h i.v. infusion on days -8, -7, -

Brain stem cavernous malformations.

We retrospectively reviewed the clinical experience of 30 patients with brain stem cavernous malformations (BSCM) treated operatively and non-operatively at our hospital between 1983 and 2005 to elucidate the natural history of BSCM and the factors that affect surgical outcome. Inpatient charts, imaging studies, operative records, and follow-up results were evaluated. The average follow up was 48.5 months. Twenty-two patients (73.3%) received surgical extirpation and of these 86.4% improved or stabilized and 13.6% deteriorated with permanent or severe morbidity. There was no mortality. Size, p

[The diagnosis of subtentorial tumours by vertebral angiography (author's transl)].

The diagnosis of space-occupying lesions in the subtentorial space by vertebral angiography is based upon a good knowledge of arteries and veins and topographic points of this region. An imaginary line between the colliculo-central point and the chorioidal point divides the posterior fossa into two imaginary parts. In this way it is possible to localize displacements in the region of the posterior fossa. Of course, the displacement of the veins and arteries is very important in the verification of the tumours. Consequently, vertebral angiography is of vital importance in the investigation of s

Dosimetric predictors of toxicity for brainstem metastases and AVMs treated with stereotactic radiosurgery: An international, multi-institutional analysis.

BACKGROUND: There are limited data on dosimetric predictors of radiation-induced changes (RICs) for brainstem metastases or arteriovenous malformations (AVMs) following single-fraction stereotactic radiosurgery (SRS). METHODS: We examined a multi-institutional cohort of patients with brainstem metastases or AVMs treated with SRS. We evaluated predictors of RICs graded per CTCAE(Common Terminology Criteria for Adverse Events), including D5%, D95%, D0.03 cc, and D0.5 cc (brainstem minus lesion). Univariate logistic regressions were initially performed with independent variables trending towards

Frame-based stereotactic biopsies of brainstem lesions - Monocentric comparison of the transfrontal and the suboccipital-transcerebellar approach over a 16-year period.

Both the transfrontal and the suboccipital-transcerebellar approach are frequently used trajectories for frame-based stereotactic biopsies of brainstem lesions. Nevertheless, it remains unclear which approach is more favorable in terms of complications, diagnostic success and outcome, especially considering the location of the lesion within the brainstem. This study compared the safety and diagnostic yield of these two approaches. Furthermore, a brainstem zone model was created to answer the question, whether there is a favorable approach depending on the location of the lesion in the brainste

Surgery of brainstem cavernous malformations.

BACKGROUND: Cavernomas are vascular hamartomas made up of thin-walled, grossly dilated blood vessels lined with endothelium. Between 4 and 35 % (mean 15 %) of cerebral cavernomas are located in the brainstem making resection of these lesions one of the most challenging tasks in neurosurgery. METHODS: Patients with cavernomas within the brainstem or deep supratentorial structures were chosen from our prospectively collected database of operated patients with brain cavernomas. The timespan of treatment was between January 1998 and June 2012. Primary outcome was defined as percentage of patients

[Value of auditory evoked potentials of the cerebral brainstem in the diagnosis of retrocochlear tumors].

The use of auditory brainstem response (ABR) in the diagnosis of retrocochlear tumoral pathology has been established for years, but the development of magnetic resonance imaging for the same purpose calls for a review of the value of this method. We studied 200 ABR records made to exclude pathology. The results indicate that ABR is useful as a screening method in this type of pathology when carried out in a population with any auditory symptom, although it was not very suggestive. In most positive tests, the presence of pathology is not confirmed.

[Late cerebral radionecrosis after conventional irradiation of cerebral tumors].

It is recognized that excessive irradiation for extra and intracranial tumor may be followed by delayed radiation necrosis of the brain. In recent years this untoward response has also been observed after conventional radiation therapy. Three patients are reported who had this irradiation complication. One had been treated for an ethmoidal sinus adenocarcinoma and another had a craniopharyngioma. The third had an astrocytoma of the pons. All developed their symptoms about a year after irradiation. Marked clinical deterioration of two patients was accompanied by brain edema and shift of midline

Intra-axial dermoid/epidermoid tumors of the brainstem in children.

BACKGROUND: Epidermoid and dermoid cysts are rare intracranial space-occupying lesions, which account for about 1% of all intracranial tumors; they are even rarer in the pediatric age group. Among the various locations, that inside the brain stem is quite exceptional (only 12 cases reported to date in the literature). We report two additional cases occurring in children, underlining their clinical characteristics, the difficulties faced in the diagnostic work-up, and the surgical treatment adopted (which consisted of removal of the cyst contents and partial resection of the cyst membrane, beca
