BACKGROUND: A septal fascicle of the left bundle branch is an anatomical variant present in a substantial number of human hearts. However, the electrocardiographic features of its block remain controversial.
CASE SUMMARY: We describe a case of transitory prominent anterior forces consistent with an intermittent left septal fascicle block, identified on a preoperative ECG of an adult male with a history of diabetes mellitus and systemic hypertension.
CONCLUSION: Clinicians should recognize septal fascicular block as an emerging and underrecognized cause of a prominent R-wave in the right prec

Several publications considering anatomical, histological, pathological, electrocardiographic, vectorcardiographic, and electrophysiologic studies have shown that the left bundle branch splits into three fascicles or in a "fan-like interconnected network" in the vast majority of human hearts. The left His system is trifascicular with a left anterior, a left posterior, and a left septal fascicle (LSF). Consequently, the classic term "hemiblock," to describe the block of one of the fascicles, established several decades ago by the Rosembaum's school, should be updated. Electrovectorcardiographic

The electrocardiographic criteria for the diagnosis of left fascicular blocks have been a subject of debate and speculation. Nevertheless, careful analysis of certain electropathologic studies, extrapolations from animal experiments, and studies resulting from the inadvertent section of the various fascicles and of tracings obtained at the moment that the conduction disturbances appeared or disappeared have led to the acceptance of characteristic patterns. Although the specificity and sensitivity of electrocardiographic criteria of left anterior and left posterior fascicular block still requir

We present the case of a professional soccer player affected by right bundle branch block and symptomatic 2:1 atrio-ventricular block during effort, due to progressive cardiac conduction disease (Lev-Lenegre disease), who received successful left bundle branch area pacing after a failed attempt at His bundle pacing. The electrocardiographic outcome of paced QRS was consistent with a rapid electrical activation of the left ventricle through the Purkinje system. The pursue of physiological pacing was preferred over conventional, given the young age of our patient and the expectedly high burden o
