Management of Pneumatosis Intestinalis in a Pediatric Burn Patient.

Pneumatosis intestinalis is gas in the wall of the gastrointestinal tract. It is not well described in pediatric burn patients. The authors present the case of a 23-month-old girl who sustained 40% total body surface area deep-partial and full-thickness burns as well as a grade two inhalational injury. On postburn day two, radiographic imaging showed extensive pneumatosis of the colon. She was managed with bowel rest, broad-spectrum antibiotics, and parenteral nutrition. Radiographic resolution of pneumatosis intestinalis occurred several days later and was followed by reinitiation of enteral

High incidence of adult respiratory distress syndrome associated with amphetamine use in the burn population: a retrospective cohort study.

UNLABELLED: A high incidence of adult respiratory distress syndrome (ARDS) is observed in patients with a history of amphetamine use with limited studies addressing this exact topic. The authors sought to understand and compare the clinical features of patients experiencing amphetamine-associated lung injury with features of similar patients naïve to amphetamines, in a population of burn patients. Patients in this population tend to be young with few comorbidities offering a unique opportunity to study the relationship between amphetamine use and ARDS. MATERIALS AND METHODS: A total of 188 pa

Toxicity associated with severe inhalational and dermal exposure to dimethylacetamide and 1,2-ethanediamine.

We present the case of a worker who was accidentally exposed (inhalational and dermal routes) to the chemicals dimethylacetamide and ethylenediamine for 90 minutes in a confined space. Clinical effects included delirium, hallucinations, skin burns, cellulitis, bilateral conjunctivitis, hepatitis, secondary coagulopathy, rhabdomyolysis, and a grade 2 esophagitis. Urinary monomethylacetamide levels 6 days after the exposure were 61 ppm.

The influence of burn injury timing on survival in patients with severe burns.

INTRODUCTION: Various factors can influence the survival of patients with severe burns. One of these factors could be the day or time of the burn injury. There are studies describing worse outcomes and higher mortality, as well as longer hospital stays, for emergency presentations at night and weekends for certain conditions. This study evaluates data from multiple burn centers, to analyse whether the day of the week, time of day or season of the year when a severe burn injury occurs has an impact on patient survival in a single patient cohort. METHODS: Only primary admitted adult patients wi

BURN-OP: A screening tool for identifying a symptomatically distinct cluster of burn patients with the greatest healthcare needs at discharge.

OBJECTIVE: To identify burn patients needing intensive rehabilitation based on discharge symptoms. METHODS: We conducted a retrospective analysis of 1049 adult burn patients recruited to the Burn Injury Model System National Database. Using unsupervised hierarchical clustering, we identified three distinct patient clusters based on discharge symptoms and compared their clinical and demographic profiles, long-term rehabilitative needs, and self-reported quality of life. We also developed a weighted BUrn Rehabilitative Needs - OutPatient (BURN-OP) to prospectively identify patients with highest

Burn Injury Leads to Increased Long-Term Susceptibility to Respiratory Infection in both Mouse Models and Population Studies.

BACKGROUND: Burn injury initiates an acute inflammatory response that subsequently drives wound repair. However, acute disruption to the immune response is also common, leading to susceptibility to sepsis and increased morbidity and mortality. Despite increased understanding of the impact of burn injury on the immune system in the acute phase, little is known about long-term consequences of burn injury on immune function. This study was established to determine whether burn injury has long-term clinical impacts on patients' immune responses. METHODS: Using a population-based retrospective lon

Nitrous oxide tank cold burn to the forearm: a case study and discussion of the literature.

Nitrous oxide (N 2 O) has become one of the most popular recreational drugs in Europe. While N 2 O is often used in medical settings as an analgesic and anaesthetic agent, its recreational use was documented many years before its introduction into clinical practice. The desired effects from inhaling N 2 O for recreational purposes include rapid feelings of relaxation, calmness and euphoria, which can be accompanied by giddiness and laughter. There are various adverse effects associated with N 2 O use, including headache, nausea, vomiting, drowsiness and the development of permanent neurologica

Infections in Burn Patients: Pathophysiology, Prevention, and Contemporary Therapeutic Strategies in the Era of Antimicrobial Resistance.

Burn injuries are associated with significant morbidity and mortality, largely driven by infectious complications. Disruption of the skin barrier, systemic inflammation, and postburn immunosuppression promote microbial colonization and progression to local and systemic infections, a challenge further exacerbated by the increasing prevalence of multidrug-resistant organisms (MDRO). Patients with extensive burns, inhalation injury, prolonged hospitalization, and invasive device use are particularly vulnerable. The microbiological profile of burn wound infections evolves dynamically, shifting fro…

Management of severe thermal burns in the acute phase in adults and children.

OBJECTIVES: To provide recommendations to facilitate the management of severe thermal burns during the acute phase in adults and children. DESIGN: A committee of 20 experts was asked to produce recommendations in six fields of burn management, namely, (1) assessment, admission to specialised burns centres, and telemedicine; (2) haemodynamic management; (3) airway management and smoke inhalation; (4) anaesthesia and analgesia; (5) burn wound treatments; and (6) other treatments. At the start of the recommendation-formulation process, a formal conflict-of-interest policy was developed and enfor

Hyperbaric Oxygen Therapy in Burn Care: A Systematic Review of Current Evidence.

Hyperbaric oxygen therapy (HBOT) involves inhaling 100% oxygen at pressures exceeding one atmosphere within a chamber. It is used across several specialties and has been proposed as an adjunct in burn management to enhance healing and improve outcomes. Despite increasing interest, evidence supporting its efficacy in burn care remains inconsistent, with no clear consensus in practice. A literature search was conducted in September 2025 across PubMed, Cochrane Library, and Embase without date restrictions to identify studies evaluating HBOT for burn treatment. Inclusion criteria were human studi

The Delayed Cortisol Peak Effect After Severe Burn Injury: A Prospective Observational Study.

BACKGROUND/AIM: In contrast to patients with major trauma and septic shock, little is known about the incidence and relevance of acute adrenal insufficiency among patients with burn injuries. Addressing this data gap, the current study was designed to analyze the spontaneous cortisol stress response after severe burn injury. PATIENTS AND METHODS: The study was designed as a prospective observational analysis. From 09/2023 until 06/2024, all patients admitted to the Intensive Care Unit of the BG Tübingen Burn Center were included. The daily cortisol levels were determined in combination with t

[A toxicometric assessment of pneumonias and acute respiratory failure in poisonings].

The author analyzes clinical and morphologic manifestations of pneumonia and the conditions under which acute respiratory failure formed in 572 subjects who suffered poisoning with psychotropic and soporific drugs, chlorinated hydrocarbons, organophosphorus insecticides, caustic poisons, alcohol and its surrogates. Toxicometric (quantitative) assessment of the toxic effects and measurement of the toxins concentrations under which respiratory failure developed helped detect new mechanisms in the patho- and thanatogenesis of pneumonias and acute respiratory failure in poisonings. These data are

Progressive pulmonary insufficiency and other pulmonary complications of thermal injury.

Progressive pulmonary insufficiency appears to be a universal response to the lung to a variety of injuries which damage the pulmonary-capillary emdothelium. Persistent hyperventilation, unresponsive to the administration of oxygen, is the earliest clinical sign of this complication of trauma and should prompt close monitoring of pulmonary function (measurement of arterial blood gas and pH levels, Vd/Vt A-aDo2, minute ventilation, vital capacity and inspiratory force) to assess the severity of the disease, the need for mechanical ventilatory support and the effectiveness of treatment. Integration of biochemical parameters into mortality prediction scores for burn patients: a step toward a new scoring system.

OBJECTIVES: This study aimed to evaluate and compare the performance of five prognostic scoring systems for predicting burn mortality: chronic health evaluation II (APACHE II), abbreviated burn severity index (ABSI), Ryan score, rBaux, and Belgium outcome in burn injury, alongside the influence of initial laboratory analyses. METHODS: A retrospective clinical study analyzed 343 patients hospitalized for moderate to severe burn injuries at a major clinical Center in Serbia. Data were extracted from medical records, and logistic regression assessed the impact of laboratory results on mortality.

Premorbid Mobility Impairments Increase Likelihood of Shelter Discharge After Burn Hospitalization: A Burn Model System Database Study.

Burn injuries are functionally limiting, and premorbid mobility impairments may further complicate recovery. Premorbid mobility impairments are associated with worse outcomes in other populations; however, their role in burn recovery remains unclear. This retrospective cohort study included adult burn survivors injured from 2014 to 2025 extracted from the Burn Model System National Burn Database. Premorbid mobility impairment was defined as self-reported preburn physical problems or impairments affecting mobility (difficulty moving the arms, legs, or body). Outcomes included length of stay, di
