FOXP3+ T regulatory cells (Tregs) are considered key players in the maintenance of immune homeostasis. Here we studied the presence and potential role of FOXP3+ Tregs in myositis. CD3 and FOXP3 expression in dermatomyositis, polymyositis and inclusion body myositis was assessed by immunohistochemistry and real-time PCR. We describe an inflammatory myopathy that was symmetrically restricted to both gastrocnemius muscles in a young man. Histopathological findings were typical for polymyositis, but there were neither signs of generalization nor muscle weakness and wasting. This condition was highly sensitive to steroids and has been kept in remission for more than a year using azathioprine. Persistent tumour necrosis factor alpha (TNF-alpha) exposure uncouples proximal T-cell receptor (TCR)-signalling events. In 41 patients aged 24-63 years with acute Flexner's dysentery T-lymphocyte subpopulations CD-3, CD-4, CD-8, IRI and the content of tumor necrosis factor alpha (TNF-alpha) in the blood serum were detected. The suppression of T-cell mediated immunity on account of the presence of CD-4 with decreased immunoregulatory index and an increased content of TNF-alpha were established. Changes in T-cell mediated immunity and TNF-alpha depended on the period and the severity of the diseases.

We examined lymph nodes and tonsils from patients with infectious mononucleosis by combined detection of EBV-encoded RNA and a specific marker of natural killer (NK) cells, PEN5. A small number of Epstein-Barr virus (EBV) latently infected nonneoplastic NK cells were detected. Our data demonstrate that NK cells are natural targets of EBV and that infection of these cells is an early event observed during primary EBV infection.

Ten patients with dermatitis herpetiformis had biopsies taken from involved and uninvolved skin. Thirteen patients with expansion of an unusual subset of T lymphocytes, defined by large size, cytoplasmic granularity and CD3+ CD8+ Leu 7+ surface phenotype, are reported.
